The final stage of the diagnostic of primary hyperaldosteronism is to identify the causes of excessive secretion of aldosterone and determination of its variants. Based on the analysis of literature data, the diagnostic value, sensitivity and specificity of the methods of radiation diagnostics for primary hyperaldosteronism were assessed: ultrasound, computed tomography, magnetic resonance imaging, photon emission tomography, magnetic resonance spectroscopy, scintigraphy with iodine radiopharmaceuticals. The causes of false-positive and false-negative evaluations of changes in adrenal glands in the application of these diagnostics have been analyzed. There are many genetic and morphological studies when searching the literature data on the principles and methods of distinguishing the nosological forms of primary hyperaldosteronism based on the results of the aldosterone level estimation in the separated blood from the central veins of both adrenal glands or segmental veins of one gland with subsequent determination of the concentration gradient. It was noted that topical diagnostics and, especially, the determination of nosological forms of primary hyperaldosteronism are complex and expensive, but their results allow choosing an appropriate treatment approach for each particular case.
La thérapie medicamenteuse de la thiroidite auto-immune Introduction. La thyroïdite auto-immune est l'un des problèmes non résolus de l'endocrinologie moderne, car la question de son étiologie et de sa pathogénie est insuffisamment connue. Objectifs. Etudier l'effet de la substitution, de la thérapie antioxydante et de drainage lymphatique chez les patients atteints d'AIT avec hypothyroïdie sur la structure et l'état fonctionnel de la glande thyroïde et le cours des processus auto-immuns. Méthodes Pour évaluer l'efficacité de la substitution, de la thérapie antioxydante et du drainage lymphatique afin de stabiliser l'état fonctionnel de la thyroïde et de supprimer l'activité des processus auto-immuns, nous avons identifié 2 groupes de patients atteints de thyroïdite auto-immune avec hypothyroïdie subclinique. Dans le groupe I (de contrôle) ont été inclus 20 patients qui ont reçu un traitement de substitution avec de la thyroxine. Dans le groupe II ont été inclus 33 patients qui ont reçu un traitement complet avec l'utilisation de la thyroxine, l'acide alpha-lipoïque et Lymphomyosot conformément au schéma élaboré.
Changes in the severity of heart failure in patients with toxic goiter after surgery are insufficiently studied.The objective of the research was to study changes in cardiac disorders in the remote period after surgical treatment of toxic goiter.Material and methods. The study included 48 patients with toxic goiter and moderate thyrotoxicosis and 102 patients with severe thyrotoxicosis. Cardiac disorders and changes in cardiac function after surgery were studied using the following gradation: good, satisfactory, unsatisfactory results and ineffective treatment.Results. Good and satisfactory results of surgical treatment in patients with moderate thyrotoxicosis were obtained in 47 (98%) cases and in patients with severe thyrotoxicosis they were achieved in 45 (44%) cases. Unsatisfactory outcome was determined by the severity of thyrotoxicosis, its duration, and low exercise tolerance during the 6-min walking test.Conclusions. Persistent severe thyrotoxicosis leads to atrial fibrillation and heart failure, which in most cases (66%) after surgical treatment of toxic goiter do not undergo reversible changes.
