Определить роль и оценить клиническую эффективность миниинвазивных вмешательств в лечении пациентов с инфицированными формами острого деструктивного панкреатита (ОДП).Материал и методы. В исследование включено 167 пациентов с инфицированными формами ОДП, из которых у 48 (28,7%) при лечении были применены миниинвазивные вмешательства. У 32 пациентов были выполнены транскутанные пункционно-дренирующие вмешательства (ТПДВ) под ультразвуковым контро-лем, у 3 под видеолапароскопическим контролем, у 6 пациентов некрсеквестрэктомия путем видеолапаро-скопии и/или ретроперитонеоскопии и у 7 пациентов -некрсеквестрэктомия из мини-доступа с помощью набора «Мини-Ассистент». Выбор метода миниинвазивного вмешательства определялся строго индивидуаль-но с учетом характера, объема и локализации гнойной полости, размеров секвестров, соотношения жидкост-ного и тканевого компонентов, а также исходной тяжести состояния пациентов по шкале SAPS.Результаты. Из 48 пациентов с инфицированными формами ОДП, которым выполнялись миниин-вазивные вмешательства, положительный клинический эффект был достигнут у 41 (85,4%) оперированных. У этих пациентов миниинвазивные вмешательства явились окончательной хирургической манипуляцией. В 7 (14,6%) случаях после применения миниинвазивных вмешательств в дальнейшем потребовалось тради-ционное хирургическое лечение. Послеоперационная летальность в группе пациентов с инфицированными формами ОДП, в лечении которых применялись миниинвазивные вмешательства, составила 12,5%.Заключение. Миниинвазивные вмешательства являются эффективными методами хирургического лечения пациентов с инфицированными формами острого деструктивного панкреатита при условии тща-тельного отбора пациентов и строгого индивидуального подхода к выбору способа оперативного пособия.
