HighlightsIntraoperative ruptures of trachea during thoracoscopic esophageal resection are very serious complications.A 52-year-old woman had cancer of middle third of esophagus T3N1M0, stage IIIB.The woman underwent thoracoscopic esophageal resection with gastric tube plasty.Successful management of intraoperative tracheal and bronchial rupture with use of the intracorporeal suture.The patient had an uneventful, standard recovery and was discharged 12 days after the surgery.
AIM: to assess the effectiveness of micronized purified flavonoid fraction (MPFF) in conservative treatment for chronic hemorrhoids according to high-resolution anoscopy (HRA).PATIENTS AND METHODS: the study included 192 patients with chronic hemorrhoids, stage III. The general recommendation for all patients was dietary fibers for soft stools. The main group included 96 patients treated with MPFF for 2 months, 96 controls used dietary fibers only. The treatment effectiveness was evaluated due to high-resolution anoscopy data and clinical manifestation of the disease. The calculation of the drug intake compliance was carried out as well.RESULTS: patients of the main group showed significant clinical improvement after treatment in prolapse correction (66% vs 27%; p=0.001), in pain intensity decrease (5 times vs 1.5 times; p=0.03), in bleeding incidence (4 times vs 1.5 times; p=0.001). HRA showed significant reduction of inflammation (from type 2 to type 1) in main group in 50% and 20% in controls (p=0.02). Compliance with the MPFF in main group 1 was 74%, the total compliance of dietary fiber intake in both groups was 66.3%.CONCLUSIONS: combination of MPFF with dietary fiber intake significantly reduces clinical manifestations of chronic hemorrhoids stage III and HRA shows significant reduction of inflammation in hemorrhoid piles in these cases.

Определить роль и оценить клиническую эффективность миниинвазивных вмешательств в лечении пациентов с инфицированными формами острого деструктивного панкреатита (ОДП).Материал и методы. В исследование включено 167 пациентов с инфицированными формами ОДП, из которых у 48 (28,7%) при лечении были применены миниинвазивные вмешательства. У 32 пациентов были выполнены транскутанные пункционно-дренирующие вмешательства (ТПДВ) под ультразвуковым контро-лем, у 3 под видеолапароскопическим контролем, у 6 пациентов некрсеквестрэктомия путем видеолапаро-скопии и/или ретроперитонеоскопии и у 7 пациентов -некрсеквестрэктомия из мини-доступа с помощью набора «Мини-Ассистент». Выбор метода миниинвазивного вмешательства определялся строго индивидуаль-но с учетом характера, объема и локализации гнойной полости, размеров секвестров, соотношения жидкост-ного и тканевого компонентов, а также исходной тяжести состояния пациентов по шкале SAPS.Результаты. Из 48 пациентов с инфицированными формами ОДП, которым выполнялись миниин-вазивные вмешательства, положительный клинический эффект был достигнут у 41 (85,4%) оперированных. У этих пациентов миниинвазивные вмешательства явились окончательной хирургической манипуляцией. В 7 (14,6%) случаях после применения миниинвазивных вмешательств в дальнейшем потребовалось тради-ционное хирургическое лечение. Послеоперационная летальность в группе пациентов с инфицированными формами ОДП, в лечении которых применялись миниинвазивные вмешательства, составила 12,5%.Заключение. Миниинвазивные вмешательства являются эффективными методами хирургического лечения пациентов с инфицированными формами острого деструктивного панкреатита при условии тща-тельного отбора пациентов и строгого индивидуального подхода к выбору способа оперативного пособия. To determine the role and estimate clinical efficiency of minimally invasive interventions in treatment of patients with the infected forms of the acute destructive pancreatitis (ADP).Methods. The study included 167 patients with infected forms of ADP, 48 (28,7%) patients underwent minimally invasive interventions. In 32 patients by ultrasonically guided percutaneous puncture and drainage interventions (PPDI) have been performed, in 3 patients -under video laparoscopic control, 6 patients underwent necrosectomy by video laparoscopy and/or retroperitoneosopy and 7 patients underwent necrosectomy from mini approaches with application of set "mini-Assistant". The choice of a minimally invasive method was defined strictly individually taking into account the character, volume and localization of a purulent cavity, the sizes of sequesters, the ratio of fluidic and tissue components, and also initial severity of a patient condition according to SAPS scale.Results.
