МЕЛЬНИКОВ ФГБУ «Научно-клинический центр оториноларингологии» ФМБА России, Москва, Россия Цель исследования -совершенствование хирургического лечения пациентов с храпом и синдромом обструктивного апноэ сна (СОАС) легкой и средней степени. Пациенты и методы. В исследование вошли 75 больных СОАС легкой и средней степени, сопровождающимся храпом, оперированных на базе ФГБУ «Научно-клинический центр оториноларингологии» ФМБА России. Пациенты были разделены на три клинические группы, каждая из которых состояла из 25 лиц с СОАС легкой и средней степени. В 1-й группе (контрольная) исследуемым была выполнена установка имплантатов мягкого неба с холодноплазменной резекцией небного язычка и задних дужек, во 2-й -холодноплазменная передняя палатопластика, в 3-й -холодноплазменная передняя палатопластика с резекцией слизистой оболочки задних дужек и язычка. Результаты. На основании данных, полученных с помощью визуально-аналоговой шкалы (ВАШ) оценки храпа постоянным партнером, опросника Эпворта, контрольной полисомнографии (ПСГ) и повторной контрольной ПСГ, проведен сравнительный анализ эффективности методов хирургического лечения через 6 мес и через 1 год. Выводы. Для хирургического лечения пациентов с храпом и СОАС легкой и средней степени разработаны и рекомендованы следующие виды операций: холодноплазменная передняя палатопластика и холодноплазменная передняя палатопластика с резекцией слизистой оболочки задних дужек и язычка. Выбор конкретного вида хирургического лечения осуществляется индивидуально в зависимости от наличия гипертрофии слизистой оболочки задних небных дужек и язычка, а также порога болевой чувствительности.Ключевые слова: синдром обструктивного апноэ сна, холодноплазменная передняя палатопластика, холодноплазменная передняя палатопластика с резекцией слизистой оболочки задних дужек и язычка, установка имплантатов мягкого неба с холодноплазменной резекцией небного язычка и задних дужек.Aim. The objective of the present study was to improve the surgical treatment of the patients presenting with snoring and obstructive sleep apnoea (OSA) syndrome. Patients and methods. The study included 75 patients presenting with OSA syndrome in the mild and moderately severe form accompanied by snoring. All the patients were given the surgical treatment based at the Research and Clinical Centre of Otorhinolaryngology, Federal Medico-Biological Agency of the Russian Federation. Цель-изучить корреляцию между клиническими проявлениями храпа и синдромом обструктивного апноэ сна (СОАС) и функциональными характеристиками верхних дыхательных путей (ВДП). Пациенты и методы. В течение 2015-2018 гг. 113 пациентам с храпом и СОАС проведены полисомнография (ПСГ) и слип-эндоскопия на базе ФГБУ «Научно-клинический центр оториноларингологии» Федерального медико-биологического агентства Российской Федерации. Данные слип-эндоскопии оценивались по системам NOHL и VOTE, а также по собственной системе оценки МРКН. Результаты. По системе VOTE обструкция на уровне мягкого неба отмечалась у 88 пациентов, причем у 25 больных она была тотальной. На уровне мягкого неба было выявлено переднезаднее (n=60), концентрическое (n=25), боковое (n=3) cужение (по форме). На уровне ротоглотки у 68 пациентов сужения не отмечалось. На уровне корня языка у 56 больных обструкция отсутствовала, у 8 наблюдалась тотальная, у 25-частичная. У 79 пациентов уровень надгортанника оставался интактным, у 6 отмечалась тотальная обструкция, у 4частичная. По классификации МРКН обструкция и форма сужения на уровне мягкого неба не отличались от системы VOTE. На уровне ротоглотки у 68 пациентов сужения не отмечалось, у 15 обструкция была значимой, у 6-незначимой. На уровне корня языка у 56 пациентов обструкция отсутствовала, у 10 наблюдалась незначимая, у 23-значимая (из числа последних у 15 обструкция возникла за счет гипертрофии язычной миндалины, у 8-за счет гипертрофии мышц языка). У 79 пациентов уровень надгортанника оставался интактным, у 8 отмечалась значимая обструкция, у 2-незначимая. Вывод. Отмечена корреляция степени тяжести СОАС с уровнем и формой сужения. Для СОАС легкой степени наиболее характерно изолированное переднезаднее сужение на уровне мягкого неба. При СОАС средней степени чаще всего встречается комбинация переднезадней обструкции на уровне мягкого неба и частичной на уровне корня языка. Концентрическое сужение на уровне мягкого неба и тотальное на уровне корня языка характерно для СОАС тяжелой степени.
Introduction. Patients with nasal polyposis are predisposed to diffuse intraoperative bleeding, that could highly impair surgical field visualization. The absence of a universal technique of improving surgical field visualization became a reason for searching the specific hemostatic methods.Objective: to evaluate the efficiency of sphenopalatine artery and/or its branches coagulation as a method of hemostasis in endoscopic surgery for nasal polyposis and to describe the surgical anatomy of sphenopalatine artery.Materials and methods. Endoscopic coagulation of sphenopalatine artery and/or its branches was performed among 30 patients with nasal polyposis. Surgical field visualization was graded by Boezaart and Wormald scales before and after coagulation.Results and discussion. Intraoperative attempts to achieve the hemostasis using warm (490С) saline irrigation and then application of xylometazoline (0.1%-10ml) and lidocaine (10%-4ml) were ineffective among 28 patients with initial grade 8 and among 2 patients with grade 9 according to Wormald scale. Sphenopalatine artery and/or its branches coagulation was effectively performed in these patients, obtaining the hemostasis in 100% of cases (р < 0,001). The initial surgical field grade improved to grade 5 by Wormald scale in 9 patients (30%), to grade 6 in 17 patients (56,7%) and to grade 7 in 4 patients (13,3%). Only one artery (trunk of sphenopalatine artery) was observed in 18 patients (60%), crossing the sphenopalatine foramen, and 2 arteries (posterior septal artery and posterior lateral nasal artery) – in 12 patients (40%). Endoscopic examination revealed no signs of mucosal atrophy in postoperative period.Conclusion: The appropriate choice of hemostatic technique depends on grades of surgical field visualization. If there is a grade 8–9 by Wormald scale, endoscopic coagulation of sphenopalatine artery and/or its branches is an effective and safe method to improve surgical field visualization.
Background. To study the efficiency of irrigation and topical corticosteroids in the treatment of patients with PNDS in allergic and non-allergic (vasomotor) rhinitis. Methods. Two groups, twelve patients each, at the age of 16 till 65, with allergic and non-allergic rhinitis participated in the investigation. All patients have firstly undergone the irrigation therapy with isotonic marine solution, after that the course of intranasal corticosteroid therapy (ICST) with fluticason propionate was provided during 14 days. Results. The levels of subjective sensations and objective symptoms were the same during and after the irrigation therapy in both groups (p>0,05).
