The use of the anterior technique of the abdominal wall components separation combined with ‘onlay’ aloplasty (ACST + onlay) in giant incisional hernias (IH) may pose a surgical challenge as it does not exclude increased intra-abdominal pressure (IAP) and the occurrence of abdominal compartment syndrome (ACS). There remains a high incidence of the surgical site complications. In our view, the use of intra-abdominal aloplasty combined with the anterior separation of the anterior abdominal wall components (ACST + IPOM) will contribute to the improvement of surgical outcomes in giant IH. Purpose: to improve the results of surgical treatment of giant IH by the use of ACST + IPOM. Materials and methods. Analysis of surgical treatment of 164 patients with giant IH aged 30 to 75 years (mean age 54.7 ± 3.3). Depending on the surgery, the patients were divided into 2 groups. Group I (82 patients) consisted of patients who underwent our modified technique, including ACST + IPOM. The surgery in group II (82 patients) involved ACST + onlay. Results and discussion. As compared with ACST + onlay, ACST + IPOM surgery contributes to a significantly reduced incidence of ACS [6.1% (group II) versus 0 (group I), (p <0.05)], seroma [25.6% versus 7.3%, p <0.05], surgical site infection (SSI) [4.9% versus 2.4%, p> 0.05], meshoma [3.7% versus 0] and hernia recurrences [6.5% versus 1.6%, p> 0.05]. Conclusions. IAP value equal or exceeding 9.1 mmHg (1.2 kPa) during the surgery in approximated rectus muscles is prognostic for ACS occurrence and requires intraoperative preventive measures. Utilization of ACST + IPOM in giant IH ensures an optimal volume of abdominal cavity without a substantial increase in IAP and reduces the probability of ACS, whereas the use of ACST + onlay results in ACN in 6.1% (p <0.05) patients. A reduced contact of the mesh with the subcutaneous tissue in ACST + IPOM contributes to a significantly lower incidence of seroma [7.3% vs 25.6% (p <0.05)], surgical site infection (SSI) [2.4% vs 4.9% (p> 0.05)], postoperative wound infiltrate [2 (2.4%) vs 11 (13.4%) (p <0.05)], chronic postsurgical pain [1 (1.6%) vs 5 (8.1%) (p> 0.05)] and recurrent IH [1 (1.6%) vs 4 (6.5%) (p> 0.05)] as compared with ACST + onlay technique.

Матеріали і методи. Проведено аналіз хірургічного лікування 215 пацієнтів з післяопераційними грижами живота (ПГЖ) великих та гігантських розмірів віком від 30 до 75 років (середній вік 54,7±3,3) за період з 2012 по 2019 рр. Жінок було 137 (63,7%), чоловіків -78 (36,3%). Вибір варіанту CST у поєднанні з алопластикою при ПГЖ великих та гігантських розмірів виконували під час операції з урахуванням середніх показників ВЧТ при контактному зведенні прямих м'язів. Групу порівняння складали 82 пацієнти, яким в період з 2008 по 2011 рр. виконувалася ACST+onlay без врахування рівня ВЧТ. Результати досліджень та їх обговорення. Оптимізація вибору варіанту CST при ПГЖ великих та гігантських розмірів може бути досягнута шляхом визначення ВЧТ під час операції при контактному зведенні прямих м'язів живота. Так, при ВЧТ (5,4±2,1) мм рт. ст. (0,7±0,3 кПа) оптимальним варіантом є ACST, при ВЧТ (9,1±2,3) мм рт. ст. (1,2±2,3 кПа) -PCST, при ВЧТ (14,1±2,3) мм рт. ст. (1,9±0,3 кПа) -TAR. Використання диференційованого підходу до вибору варіанту CST у поєднанні з алопластикою при великих та гігантських ПГЖ з урахуванням ВЧТ сприяє покращенню результатів лікування, а саме зменшується частота АКС до 0,5 % проти 4,9 % у групі порівняння, сероми -до 15,8 % проти 25,6 %, інфікування післяопераційної рани -до 1,4 % проти 4,9 %, хронічного післяопераційного болю -до 4,2 % проти 8,1 %, рецидиву ПГЖ -до 1,4 % проти 8,1 %.Ключові слова: післяопераційна грижа живота; методика розділення анатомічних компонентів; внутрішньочеревний тиск; алопластика.
Мета роботи: покращення результатів лікування хворих із рецидивом пахвинних гриж після операції Ліхтенштейна. Матеріали і методи. Проведено аналіз хірургічного лікування 105 пацієнтів із рецидивною пахвинною грижею після операції Ліхтенштейна. Всі хворі були чоловіки. У 31 (29,5 %) хворого рецидив виник через 3 місяці після первинної пластики, після 6 міс. – у 59 (52,3 %), після 12 – у 19 (18 %), 26 хворих (24,7 %) мали супутню патологію. В першій групі 52 пацієнтам виконувалось класична ТАРР. Під загальним знеболенням накладали карбоксиперитонеум, встановлювали 3 троакари, основний етап ТАРР, розрізання парієтальної очеревини над пахвинними ямками, після чого преперитонеально встановлювали сітку 10×12 см, з наступною фіксацією її до зв’язки Купера і м’язів черевної стінки. В другій групі – 53 хворі – виконували удосконалену ТАРР. Суть удосконалення – додаткова мобілізація верхнього клаптя парієтальної очеревини краніально на 3-4 см, заводилась ширша сітка 15×15 см. Комбінована фіксація імплантату герніостеплером і клейова фіксація клеєм Сульфакрилат. Результати досліджень та їх обговорення. В першій групі, у 5 хворих (9,6 %), були сероми на стороні герніопластики. В більш пізньому періоді хворі спостерігались у строках 6, 24 та 36 місяців. Так, у 4 (7,7 %) було діагностовано хронічний пахвинний біль. Повторний рецидив спостерігали в 5 (9,6 %). В другій групі сероми – у 6 (11,3 %) хворих, у 3 (5,6 %) хворих діагностований хронічний пахвинний, повторний рецидив 1 (2 %). Використання удосконаленої ТАРР, порівняно з класичною методикою, при рецидивах пахвинної грижі після операції Ліхтенштейна, за рахунок використання більшого сітчастого імплантату 15×15 см та його клейова фіксація по нижньому краю сприяють зменшенню частоти повторного рецидиву з 9 до 2 %.
The aim: The aim of the article is to increase the effectiveness of the treatment of postoperative ventral hernias of giant size by a differentiated approach to the selection of the posterior technique of dissection of the anatomical components of the anterior abdominal wall in combination with alloplasty. Materials and methods: An analysis of the surgical treatment of 312 patients with giant postoperative ventral hernias (PVH). The main group consisted of 232 patients who underwent a differentiated approach to the selection of component separation in combination with alloplasty, taking into account intra-ab¬dominal pressure (IAP) when simulating hernioplasty during surgery. In the second control group (80 patients), surgical treatment of PVH of giant sizes was performed using only the posterior Carbonell technique. Results: In 78 patients of the 1st subgroup, IAP was 9.1±1.2 mm Hg. the posterior technique of separating anatomical components according to Carbonell was performed in combination with sublay alloplasty. In 80 patients of the II subgroup with IAP from 11.1 to 14.1±1.2 mm Hg – the TAR technique was performed with sublay alloplasty. In 74 patients of III subgroups with IAP from 16 to 20.1±1.2 mm Hg, performed the TAR technique, we improved in combination with IPOM alloplasty. Conclusions: An increase in the effectiveness of surgical treatment was achieved in patients of subgroup I, reducing the probability of intra-abdominal hypertension to 5.1% versus 11.2%, in patients of subgroup II to 5% versus 11.2%, and in patients of subgroup III to 1.4% versus 11.2% of the control group.
