Стаття присвячена актуальній проблемі неонатології і дитячої неврології, зокрема перинатальної неврології-морфофункціональним особливостям розвитку головного мозку в онтогенезі. Наведено формування патології нервової системи у новонароджених з урахуванням дії шкідливих чинників. При систематизації вроджених порушень розвитку велике значення надається часу ушкодження, а також характеру дії, що ушкоджує. При дії шкідливих чинників на 23-28-й день внутрішньоутробного розвитку (дорзальна індукція), коли відбувається утворення нервової пластинки і відособлення головного і спинного мозку, виникають такі грубі вади розвитку, як аненцефалія, енцефалоцеле, менінгомієлоцеле. При терміні дії на 30-32-й день гестації (вентральна індукція), коли відособляється передній мозок, нюхові цибулини, мозкові гемісфери, бічні шлуночки і базальні ганглії, виникає голопрозенцефалія. При терміні гестації 45 днів-5 місяців внутрішньоутробного розвитку (проліферація нейронів), коли відбувається клітинний мітоз і подальший розвиток тканин мозку, формується істинна мікро-і макроцефалія. При дії шкідливих чинників на 5-му місяці гестації (пік клітинної міграції) внаслідок змін міграції нейронів у певні зони мозку (наприклад, кору головного мозку) можливе виникнення таких порушень розвитку мозку, як пахігірія, мікрогірія, полімікрогірія, гетеротопія. При терміні дії 6-9 місяців внутрішньоутробного розвитку (стадія організації), коли відбувається пошарове розташування кіркових нейронів, формування синаптичних контактів, гліальна проліферація і диференціація, виникають такі порушення, як розлади клітинного метаболізму, порушення або затримка мієлінізації, що в подальшому можуть призвести до наслідків органічного ураження головного мозку, а також до таких станів, коли мозок не здатний перенести природне навантаження, пов'язане з актом пологів, а саме до зриву кардіореспіраторної та метаболічної адаптації. В огляді літератури висвітлено основні питання нейрогенезу, гліогенезу, формування гематоенцефалічного бар'єра та системи васкуляризації на різних етапах розвитку плода. Дія патогенних чинників на різних етапах розвитку головного мозку призводить до особливостей: міграції та диференціації клітинних елементів, синаптогенезу і мієлінізації, порушень судинної архітектоніки і регуляції мозкового кровообігу, стану лікворної системи і гематоенцефалічного бар'єра, що визначає в кінцевому підсумку своєрідність морфологічних особливостей ураження центральної нервової системи (ЦНС) та клінічну різноманітність неврологічної симптоматики у новонароджених дітей, зокрема недоношених. Практичне значення вищевказаних особливостей розвитку головного мозку в онтогенезі полягає в оцінці характеру морфофункціональних змін у ЦНС новонароджених дітей, що дозволяє своєчасно визначати тактику лікування наслідків.
Стаття присвячена актуальній проблемі неонатології, перинатальної неврології та імунології, зокрема нейроімунології,-питанням щодо нейроімунних механізмів у формуванні перинатального ураження головного мозку у новонароджених. Показано ембріональний період розвитку імунної системи плода, який характеризується формуванням і дозріванням основних ланок неспецифічного (вродженого) та адаптивного (специфічного) імунітету. Зокрема формування неспецифічних механізмів резистентності імунної системи, які відіграють головну роль у захисті організму дитини на ранніх етапах онтогенезу. Визначено, що взаємодія імунної та нервової систем має комплексний характер, починаючи від індукування їх аферентних відділів на ранніх етапах імуногенезу і закінчуючи наступною активацією еферентних ланок зазначених систем. В основі цієї взаємодії лежить здатність цитокінів функціонувати і як імунорегулятори, і нейропептиди одночасно. Висвітлено сучасні дані літератури про імунозахисну, нейроцитотоксичну та нейропротекторну функції мікрогліальних клітин ЦНС. Показано походження і розвиток мікроглії. Проаналізовано гетерогенність цих клітин, показано їх фізіологічну роль у здоровому організмі, забезпечення контролю за діяльністью живих нейронів, а також її реакція на патологічні стани. Наведено дані літератури про протизапальну та ремієлінізуючу дію мікроглії та її гуморальних чинників. У статті висвітлено основні принципи взаємодії нервової та імунної систем, а також деякі питання щодо ролі нейроімунних механізмів у формуванні перинатального ураження головного мозку; зазначено, що у формуванні та прогресуванні постгіпоксичної енцефалопатії тригерним фактором є локальне запалення з подальшим накопиченням антитіл і вторинним пошкодженням гематоенцефалічного бар'єру. Наведено дані літератури щодо нейроімунних механізмів у формуванні дитячого церебрального паралічу. З перивентрикулярною ділянкою пов'язані мітоз, міграція нейронів до кори і підкіркових структур, а також, що особливо важливо у період онтогенезу, аксональний синаптогенез із клітинами мішенями і формування функціональних систем. Прогредієнтність патологічних змін, зокрема перивентрикулярної ділянки, обумовлена імунологічним дисбалансом. Інтерес до розгляду системи цитокінів можна пояснити залученістю зазначених медіаторів міжклітинної взаємодії у патогенез перивентрикулярної лейкомаляції як одного з провідних патоморфологічних субстратів при ДЦП у передчасно народжених дітей. Вивчення популяційного складу імунокомпетентних клітин, медіаторів їх міжклітинної взаємодії, маркерів проникності гематоенцефалічного бар'єру при гіпоксично ішемічному ураженні ЦНС різного ступеня важкості дозволить висвітлити нові ланки нейроімунного конфлікту у патогенезі неврологічних порушень у новонароджених, зокрема недоношених і дітей раннього віку. Статья посвящена актуальной проблеме неонатологии и детской неврологии — вопросу ранней диагностики перинатального повреждения головного мозга гипоксически-ишемического генеза у новорожденных, в частности преждевременно рожденных детей. В работе проанализирована научная литература o механизмах перинатального повреждения головного мозга гипоксически-ишемического генеза. Представлены новые данные об особенностях функционирования, повреждения, а также о механизмах гибели клеток нейронального и глиального происхождения в развивающемся мозге. Показано, что в реализации механизма повреждения клеток головного мозга участвуют эксайтотоксичность, окислительный стресс и асептическое воспаление, в результате которых клетки гибнут путем некроза и патологического апоптоза. Подчеркнуто, что в незрелой нейрональной ткани гибель нейронов протекает не только вышеуказанными путями, но и по комбинированному некротически-апоптозному (некроптозный) механизму. Проанализированы особенности функционирования глутаматных рецепторов в развивающемся мозге. Приведены данные литературы о том, что выше-описанные патологические механизмы на фоне особенностей функционирования митохондрий развивающегося мозга приводят к дебюту патологического апоптоза. Определено, что наиболее перспективным в ранней диагностике гипоксически-ишемического повреждения центральной нервной системы у новорожденных детей, в частности преждевременно рожденных, является изучение уровня нейроспецифических белков и антител к ним, а также белков, связанных с плазматической мембраной клеток, в частности клеток мозга — молекул межклеточной адгезии. В статье проанализирована роль маркеров нейрональной и глиальной природы, в частности глиофибриллярно-кислого протеина (GFAP), убиквитин-С-концевой гидролазы L1 (UCHL1), основного белка миелина (MBP), с учетом роли провоспалительных цитокинов в механизмах повреждения клеток развивающегося мозга. Показана роль мембранного белка эндотелиоцитов мозговых капиляров (молекулы межклеточной адгезии 1 — sICAM-1) как одного из маркеров повреждения клеток гематоэнцефалического барьера при различных патологических процессах, в частности гипоксии и ишемии.
The article deals with the current problem of neonatology and pediatric neurology — the issues of early diagnosis of perinatal hypoxic-ischemic brain injury in newborns, particularly, in prematurely born children. The work considers modern literature data on the mechanisms of hypoxic-ischemic perinatal brain damage. New data on the functioning, injury, as well as the mechanism of cell death of neuronal and glial origin in the developing brain are presented. It was shown that excitotoxicity (glutamatergic system), oxidative stress and aseptic inflammation are involved in the realization of this mechanism, the final result of which is cell death by necrosis and pathological apoptosis. It was emphasized that in immature neuronal tissue, the death of neurons occurs not only by the above paths, but also due to the combined necrotic-apoptotic (necroptotic) mechanism. The ambiguous role of glutamate receptors in the developing brain is analyzed. Literature data are presented that excitotoxicity, oxidative stress and inflammation against the background of peculiarities mitochondrial functioning in the brain lead to the onset of pathological apoptosis. It has been determined that the most promising in the early diagnosis of hypoxic-ischemic damage to the central nervous system in newborns, in particular premature babies, is the study of the level of neuron-specific proteins and antibodies to them, as well as proteins associated with the plasma membrane — intercellular adhesion molecules. The article analyzes the role of neuronal and glial markers, in particular glial fibrillary acidic protein, ubiquitin C-terminal hydrolase L1, myelin basic protein, as well as the role of pro-inflammatory cytokines in the mechanisms of damage to cells of the developing brain. The role of the membrane protein of cerebral capillary endotheliocytes, an intercellular adhesion molecule 1, as one of the markers of damage to the blood-brain barrier cells in various pathological processes, in particular hypoxia and ischemia, was determined.
The article is devoted to the urgent problem of neonatology and pediatric neurology — seizures in newborns and young children. The work analyzes the scientific literature on the pathogenetic mechanisms of the development of the epileptic process, which is based on the anatomical and physiological mechanisms of the functioning of brain cells, in particular, in newborns and young children. New pathophysiological data on the increased excitability of the developing brain are described. It has been shown that the implementation of the mechanism of increased excitability of the fetal brain may be due to the peculiarities of the functioning of NMDA, AMPA, kainate receptors, and the peculiarities of the localization of ion channels in different brain structures. The paradoxical (exciting, depolarizing) role of gamma-aminobutyric acid, which is due to the peculiarities of the activation of chlorine co-transporters, is emphasized. The features of the epileptic process in newborns and young children have been determined. The reasons for the development of seizures in newborns are emphasized. The classification and clinical phenotypes of neonatal seizures are considered. It is emphasized that the convulsions in newborns rarely have a deployed clinical picture and are often represented by abortive or focal seizures, which is associated with the ontogenetic features of the fetal brain, namely: incomplete by the time of the birth of a cortical-neural organization, synaptogenesis and myelination of its structures, insufficiency of the development of commulectral intermetrous bonds , uneven representation in the cortex of ion channels with a relatively well-formed limbico-reticular system and its bond with a brain barrel. It was noted that despite various clinical manifestations, the newborn has four main types of seizures: subtitle (erased, abortive, fragmentary), tonic, clonic and myocloniс.
