StreszczenieWstęp: Zaburzenia równowagi ciała są jednym z wielu czynników negatywnie wpływa-jącym na codzienną aktywność, co skutkuje pogorszeniem jakości życia człowieka. Celem pracy była cena różnic wyników stabilometrycznych w dynamice i statyce u kobiet młod-szych, starszych i z dysfunkcją narządu wzroku.Materiał i metody: Materiał badany obejmował 67 kobiet podzielonych na 3 grupy: 26 kobiet młodszych (20,2 ± 1,75) i 26 starszych (68,7 ± 7,55) oraz 15 osób z dysfunkcją narządu wzroku (19,2±1,78). Wykonano badania stabilometryczne na dwóch platformach; Accusway i Balans System SD.Wyniki: Wyniki różniące się istotnie statystycznie odnotowano pomiędzy grupą osób młodszych a starszych w wynikach parametrów z platformy Accusway badanych z kontrolą wzrokową tylko w polu powierzchni stabilogramu oraz w maksymalnych wychyleniach na boki. W powyższych grupach na platformie BBS obserwuje się znamienne różnice we wszystkich ocenianych próbach. Największe różnice obserwuje się na poziomie p<0,0001 w protokołach z niestabilnym podłożem. Najlepsze wyniki długości COP w warunkach oczu zamkniętych w statyce osiągnęły osoby z dysfunkcją narządu wzroku (37,01±6,73), najsłabsze kobiety po 60 r. ż. (49,59±21,04). Natomiast badania na niestabilnym podłożu bez kontroli wzrokowej osoby starsze nie wykonały, a kobiety z dysfunkcją narządu wzroku osiągnęły istotnie gorsze rezultaty od grupy osób normalnie widzących.Wnioski: Większe zaangażowanie systemu kontroli równowagi obserwuje się w badaniach dynamicznych, które wydają się być bardziej odpowiednie i mogą wcześniej wykrywać zaburzenia niż badania w statyce. Szczególnie przydatne wydają się protokoły badań ze zmiennym ustawieniem niestabilności platformy lub dodatkowo bez kontroli wzrokowej, które najbardziej różnicują badanych. Aim: evaluation of the differences in static and dynamic postural stability in young, elderly and with vision loss women.Material and methods: Eighty-three female volunteers were divided into three groups: 26 young women (20,2 ± 1,75), 26 elderly (68,7 ± 7,55) and 15 pupils with vision loss (19,2±1,78). The prevalence of temporomandibular disorder (TMD) in the general population is difficult to identify. Depending on the source, values range from 3% to 48.9%. These large discrepancies occur because of two main reasons. The first one is connected with the evolution of the definition and diagnostic criteria of TMD, while the second one concerns the use of different diagnostic tools, protocols, etc. Temporomandibular joint (TMJ) diagnosis is both quantitative and qualitative. Quantitative measurements used in the field of scientific research are highly accurate motion capture systems used for kinematic analysis, while an electronic caliper is applied in a clinical setting. MRI seems optimal in TMJ disc and its dysfunction imaging. In more difficult cases, ultrasound imaging can be used. On the other hand, CT is the best examination for joint imaging. In recent years, qualitative measurements have been dominated by vibroarthrography (VAG), which shows good diagnostic value. Research Diagnostic Criteria for Temporomandibular Joint Disorders (RDC/TMD) is the most complex systematized diagnostic tool for TMD. The proposed procedure allows us to differentiate a real TMJ dysfunction from other systemic problems that can manifest themselves in this region of TMJ, such as depression, chronic pain or psychosomatic disorders. RDC/TMD are constantly evaluated and modified in terms of their diagnostic or clinical value by experts associated in the International RDC-TMD Consortium. Currently it is the best tool available for TMD diagnostics. The RDC/TMD questionnaire was translated into Polish, culturally adapted and officially approved by the Consortium. The aim of the study is to review selected TMD diagnostic tools and to assess their diagnostic value.
Streszczenie Wstąp. Inwolucyjne zmiany organizmu w procesie starzenia są wieloczynnikowe. a niektóre z nich mają wpływ na pogorszenie jakości lokomocji. Wyróżnia się wśród nich między innymi: spadek sił}' mięśniowej czy zmniejszenie zakresu ruchomości w stawach. Objawy tych zaburzeń manifestują się zmianą niektórych parametrów czasowo-przestrzennych chodu, przede wszystkim: zmniejszeniem prędkości chodu i długości kroku oraz zwiększeniem szerokości kroku. Cel. Celem pracy była ocena wpływu różnych form treningowych na czasowo-przestrzenne parametry chodu osób starszych. Materiał. Badaniem objęto 59 słuchaczy Uniwersytetu Trzeciego Wieku AWF Warszawa. Badania lekarskie i wysiłkowe pozwoliły wykluczyć wszelkie przeciwwskazania zdrowotne. Dokonano podziału na dwie grupy treningowe: TW - trening wszechstronny i TN - trening w pozycjach niskich. Badania ukończyło po 14 osób z każdej z grup. Metody. Pomiary wykonano przy użyciu platformy FDM Zebris. która rejestrowała parametry czasowo-przestrzenne badanych. Ich zadaniem było trzykrotne przejście po platformie z naturalną dla siebie prędkością. Cykl treningów trwał 12 tygodni, zajęcia odbywały się dwa razy w tygodniu i trwały 45 minut. Zasadnicza różnica pomiędzy treningami polegała na eliminacji ćwiczeń lokomocji i ćwiczeń w pozycjach wysokich w grupie TN. Analizę statystyczną przeprowadzono przy użyciu oprogramowania Statistica. Wyniki. W wyniku treningu wszechstronnego wśród badanych zaobserwowano istotną zmianę sześciu parametrów chodu:. długość kroku prawej kończyny dolnej. długości kroku prawej kończyny dolnej (jako % długości kończyny), czas kroku prawej i lewej kończyny dolnej, czas cyklu chodu oraz kadencji. Pozostałe parametry wykazały pewną tendencję, lecz ich zmian nie można przypisać przeprowadzonemu treningowi. Badani grapy TN nie wykazali istotnych zmian w analizowanych parametrach. Wnioski. Zmiany jakości chodu są wieloczynnikowe. istnieje więc potrzeba dalszych dociekań. w celu ich rozpoznania oraz modyfikacji w ramach treningu czy terapii. Dobór ćwiczeń zastosowanych w treningu powinien strukturalnie odzwierciedlać docelowy element, któiy ma być kształtowany.
Current research concerning the repeatability of the joint’s sounds examination in the temporomandibular joints (TMJ) is inconclusive; thus, the aim of this study was to investigate the repeatability of the specific features of the vibroarthrogram (VAG) in the TMJ using accelerometers. The joint sounds of both TMJs were measured with VAG accelerometers in two groups, study and control, each consisting of 47 participants (n = 94). Two VAG recording sessions consisted of 10 jaw open/close cycles guided by a metronome. The intraclass correlation coefficient (ICC) was calculated for seven VAG signal features. Additionally, a k-nearest-neighbors (KNN) classifier was defined and compared with a state-of-the-art method (joint vibration analysis (JVA) decision tree). ICC indicated excellent (for the integral below 300 Hz feature), good (total integral, integral above 300 Hz, and median frequency features), moderate (integral below to integral above 300 Hz ratio feature) and poor (peak amplitude feature) reliability. The accuracy scores for the KNN classifier (up to 0.81) were higher than those for the JVA decision tree (up to 0.60). The results of this study could open up a new field of research focused on the features of the vibroarthrogram in the context of the TMJ, further improving the diagnosing process.
