BackgroundPulmonary arterial hypertension (PAH) is a severe and progressive disease characterized by increased pulmonary vascular resistance, ultimately leading to right heart failure and death. Epidemiological data from national registries are growing worldwide, but are still unavailable in Eastern Europe.MethodsA PAH registry was initiated in January 2007 using a nationwide network of echocardiographic centers and four diagnostic centers that specialize in PAH. All patients aged above 18 years, diagnosed with PAH and monitored between January 2000 and December 2007 were included. Patients diagnosed with PAH between January and December 2007 were classified as incident. The survival analyses were performed up to the end of 2010. Prognostic factors at the time of diagnosis were identified using uni- and multivariable Cox proportional hazard models.ResultsOverall, 191 patients were included (100 prevalent cases, 91 incident cases). Patients were predominantly female (n = 125) and had a mean age of 51.9 ± 16.9 years. Incident patients were significantly older at the time of diagnosis than prevalent patients (p < 0.001). Most patients (60.7%) had idiopathic PAH; 20.4% had PAH associated with congenital heart disease and 11.4% had PAH associated with connective tissue disease. Estimates of prevalence and incidence of PAH in adults were 22.4 cases per million and 10.7 cases per million per year, respectively. The 1-, 2- and 3-year survival rates in the incident PAH cohort were 89% (95% confidence intervals [CI] 83–95%), 78% (95% CI 70–87%) and 74% (95% CI 65–83%), respectively. Lower survival rates were significantly associated with higher age (hazard ratio [HR] 6.6 95% CI 1.4–30.9) and lower creatinine clearance (HR 3.3 95% CI 1.1–9.7).ConclusionThis is the first study in Eastern Europe to describe the prevalence, incidence and survival of patients with PAH from a national representative registry. This registry from the Czech Republic highlights that diagnosis of PAH is frequently made late in the disease continuum when patients have significant functional impairment.
Background: Mechanical heart valves carry signifi cant risk during pregnancy. Aim: To assess the risk of pregnancy and delivery for mother and child in women with mechanical heart valve prosthesis. Methods: Multicenter retrospective observational study of women with mechanical heart valve prosthesis. The regimen with low molecular weight heparin throughout pregnancy (LMWH) was compared to warfarin since the second trimester (W). Results: Out of 419 pregnant women with cardiovascular diseases we assessed 14 women with mechanical heart valve prosthesis (3.3%) who experienced 23 pregnancies. There were 13 pregnancies with aortic valve prosthesis, 8 with prosthesis of atrio-ventricular valves and 2 pregnancies with both aortic and mitral valve prosthesis. LMWH regimen was used in 18 pregnancies, W in 5 pregnancies. Major maternal complications occurred in 65% of pregnancies, including 3 strokes and 3 urgent cardio-surgical procedures, all in women with LMWH regimen. Prosthetic valve thrombosis occurred in 26%, all in the LMWH group; no thrombosis was found in W group, (p = 0.166). Major bleeding occurred in 30% with no difference between LMWH and W groups (p = 0.596). Fetal loss represented 26% of all pregnancies with no difference between LMWH and W group (p = 1). The birth weight was not signifi cantly different between LMWH and W groups (2496 ± 327 g vs. 3132 ± 592 g, p = 0.12). Conclusion:The rate of maternal and fetal complications in pregnant women with mechanical valve prosthesis is still high. The anticoagulation regimen using warfarin since the second trimester appears to be the safest one. The best prevention of complications is to avoid the implantation of mechanical valve prosthesis in girls and women in fertile age.
Bolesti na hrudi jsou jedním z vedoucích důvodů návštěv urgentního příjmu a hlavním symptomem akutního infarktu myokardu (AIM). Základem diagnostiky AIM je stanovení hodnoty biochemických markerů srdeční ischemie v krvi. Aktuálním zlatým standardem je srdeční troponin. Rozšíření vysoce senzitivních metod umožňujících detekci velmi nízkých hodnot srdečního troponinu představuje výzvu pro správnou interpretaci výsledků. Doporučené postupy Evropské kardiologické společnosti upřednostňují použití validovaných algoritmů rule out/rule in k rychlému a bezpečnému vyloučení AIM a/nebo k časné identifi kaci a zahájení léčby pacientů s vysokým rizikem fi nální diagnózy AIM. Článek předkládá srozumitelné informace o využití vysoce senzitivní metodiky stanovení srdečních troponinů v klinické praxi. Přináší algoritmy preferované Evropskou kardiologickou společností a užitečné informace k interpretaci výsledků v některých specifi ckých skupinách pacientů.

Výjimečně se postižení jedinci dožívají vyššího věku, aniž by byla stanovena a řešena diagnóza ALCAPA. V kasuistice popisujeme případ asymptomatického 45letého muže, u nějž byla diagnóza stanovena v rámci došetření blokády levého Tawarova raménka a mírně snížené systolické funkce levé komory. Ačkoliv klíčovou diagnostickou metodou ALCAPA je koronarografi e a výpočetní tomografi e, v našem případě byla přesná diagnóza stanovena echokardiografi cky. Původně byl pacient veden u ambulantního kardiologa s nesprávnou diagnózou mnohočetného defektu komorového septa. Ve skutečnosti se však jednalo i dezinterpretaci ultrazvukového obrazu interkoronárních spojek. Současná přítomnost samostatného odstupu ramus diagonalis z aorty činí kasuistiku ještě zajímavější. ABSTRACT ALCAPA syndrome (anomalous origin of left coronary artery from the pulmonary artery) is an extremely rare congenital cardiac anomaly associated with high mortality rate at young age. If undiagnosed and uncorrected, the affected individuals rarely survive beyond infancy. This article reports on a 45-year-old asymptomatic man with the ALCAPA diagnosed thanks to a detailed cardiac examination that followed the finding of the left bundle branch block and a decreased left ventricular systolic ejection fraction. Although coronary angiography and computed tomography are considered to be the key diagnostic methods for ALCAPA, in this case the diagnosis was established based on echocardiography. Previously, the patient has been under regular review by a cardiologist with the incorrect diagnosis of multiple ventricular septal defects. In fact, this diagnosis resulted from a misinterpretation of the ultrasound image of the intercoronary connections.

Většina postihuje výtokový trakt pravé komory a plicní tepny, a tak ovlivňuje funkci pravé komory. Nejčastějším reziduem je objemové přetížení pravé komory v důsledku rozvoje závažné pulmonální regurgitace. Přestože již existují doporučení pro správné načasování náhrady pulmonální chlopně, zůstává ještě v tomto ohledu mnoho kontroverzních otázek. Zachování funkce pravé komory a prevence arytmií vycházejících z pravé komory je pro dlouhodobé přežití a morbiditu pacientů po korekci Fallotovy tetralogie zásadní.Klíčová slova: Fallotova tetralogie, pulmonální regurgitace. Tetralogy of FallotPatients after corrective surgery of tetralogy of Fallot develop residual findings in both early postoperative course and long-term follow-up in adulthood. Most residual findings affect the right ventricular outflow tract and pulmonary arteries and thus indirectly influence the function of the right ventricle. The most frequent residual finding is volume overload of the right ventricle due to severe pulmonary insufficiency. In spite of the fact that guidelines on the correct timing of pulmonary valve replacement exist, there are still a lot of controversies. Preservation of the function of the right ventricle and prevention of arrhythmias arising from the right ventricle are crucial for the long-term survival and morbidity of tetralogy of Fallot patients.
