T-wave inversions in leads V1-V4 appear to represent an ethnic variant of 'athlete's heart'. Conversely, T-wave inversions in the lateral leads may represent the initial expression of underlying cardiomyopathy and merit further evaluation and regular surveillance.
Objetivo: Descrever reações adversas após tratamento radioterápico em pessoas com cânceratendidas em um ambulatório de radioterapia. Metodologia: Trata-se de um estudo documental pormeio da coleta de dados em prontuários clínicos. Foram avaliados 436 pacientes que realizaramtratamento radioterápico em busca de episódios de reações adversas registrados nos prontuários.Foram elegíveis os pacientes maiores de 18 anos com diagnóstico de neoplasia (confirmado porlaudos citopatológicos e/ou histopatológico) que realizaram cinco ou mais sessões de radioterapiaentre 2015 e 2016. Resultados: 353 prontuários clínicos foram incluídos (83,6%) e 1.390 reaçõesassociadas ao tratamento radioterápico foram descritas nestes pacientes, sendo 53,2% mulheres. Asreações adversas mais frequentes relacionadas à radioterapia foram dor (14%), radiodermite (9%) efalta de apetite (8%). 69% dos pacientes não apresentavam sinais ou sintomas antes do diagnóstico,sendo 25% diagnosticados com adenocarcinoma. A técnica de radioterapia mais utilizada foi a 3D(67%), com cerca de 30 frações (14%) e 5000 Gy como dose total planejada (17%). 70% dospacientes que apresentaram reações adversas possuíam ao menos uma comorbidade. Conclusão: Aradioterapia pode estar associada à diversas reações adversas, variando os sinais e sintomas nospacientes acometidos. A prevenção e o manejo destas condições podem potencializar o tratamento,reduzindo a morbidade e a mortalidade, além de proporcionar qualidade de vida aos pacientes comcâncer.
O objetivo do estudo foi descrever o perfil clínico-epidemiológico de mulheres em tratamento oncológico para carcinoma ductal invasivo da mama. Trata-se de um estudo transversal, descritivo e quantitativo por meio da coleta de dados em prontuários clínicos de mulheres em tratamento oncológico do câncer de mama em um serviço privado de oncologia, considerando variáveis sociodemográficas e clínicas. Foram identificadas 125 mulheres que se adequaram aos critérios de inclusão propostos. A maioria das pacientes incluídas eram sergipanas (71,2%), pardas ou negras (65,6%), com companheiro (56%) e com média de idade de 53,5±11,4 anos. Destas, 80,8% não possuíam ensino superior e 77,6% possuíam renda familiar entre um e três salários mínimos. Todas se submeteram à quimioterapia, 64,8% à mastectomia total e 36% à radioterapia. Além disso, 32,8% apresentaram mudanças na autoimagem, 24,8% na autoestima, 64,8% sinais de ansiedade e 49,6% sintomas depressivos. Além disso, 40% eram portadoras de hipertensão arterial sistêmica. O perfil clínico-epidemiológico das mulheres com carcinoma ductal invasivo da mama descrito é amplo e possui componentes sociodemográficos importantes. O tratamento oncológico pode desencadear alterações clínicas relevantes para a saúde, bem-estar e qualidade de vida destas pacientes.
ObjectivesThe aim of this study was to investigate the relationship of echocardiographic parameters, laboratory findings and clinical characteristics with in-hospital mortality in adult patients with COVID-19 admitted to the intensive care units (ICU) in two large collaborating tertiary UK centres.DesignObservational retrospective study.SettingThe study was conducted in patients admitted to the ICU in two large tertiary centres in London, UK.ParticipantsInclusion criteria were: (1) patients admitted to the ICU with a COVID-19 diagnosis over a period of 16 weeks. and (2) underwent a transthoracic echocardiogram on the first day of ICU admission as clinically indicated.No exclusion criteria applied.Three hundred patients were enrolled and completed the follow-up.Primary and secondary outcome measuresThe outcome measure in this study was in-hospital mortality in patients admitted to the ICU with COVID-19 infection.ResultsOlder age (HR: 1.027, 95% CI 1.007 to 1.047; p=0.008), left ventricular (LV) ejection fraction<35% (HR: 5.908, 95% CI 2.609 to 13.376; p<0.001), and peak C reactive protein (CRP) (HR: 1.002, 95% CI 1.001 to 1.004, p=0.001) were independently correlated with mortality in a multivariable Cox regression model. Following multiple imputation of variables with more than 5% missing values, random forest analysis was applied to the imputed data. Right ventricular (RV) basal diameter (RVD1), RV mid-cavity diameter (RVD2), tricuspid annular plane systolic excursion, RV systolic pressure, hypertension, RV dysfunction, troponin level on admission, peak CRP, creatinine level on ICU admission, body mass index and age were found to have a high relative importance (> 0.7).ConclusionsIn patients with COVID-19 in the ICU, both severely impaired LV function and impaired RV function may have adverse prognostic implications, but older age and inflammatory markers appear to have a greater impact. A combination of echocardiographic and laboratory investigations as well as demographic and clinical characteristics appears appropriate for risk stratification in patients with COVID-19 who are admitted to the ICU.
Background: Germline mutation screening of BRCA1 and BRCA2 genes is performed in suspected familial breast cancer cases, but a causative mutation is found in only 30% of patients. The development of additional methods to identify good candidates for BRCA1 and BRCA2 analysis would therefore increase the efficacy of diagnostic mutation screening. With this in mind, we developed a study to determine molecular signatures of BRCA1—or BRCA2—mutated breast cancers. Materials and Methods: Array-cgh (comparative genomic hybridization) and transcriptomic analysis were performed on a series of 103 familial breast cancers. The series included 7 breast cancers with a BRCA1 mutation and 5 breast cancers with a BRCA2 mutation. The remaining 91 cases were obtained from 73 families selected on the basis of at least 3 affected first-degree relatives or at least 2 affected first-degree relatives with breast cancer at an average age of 45 years. Array-cgh analyses were performed on a 4407 BAC-array (CIT-V8) manufactured by IntegraGen. Transcriptomic analyses were performed using an Affymetrix Human Genome U133 Plus 2.0 chip. Results: Using supervised clustering analyses we identified two transcriptomic signatures: one for BRCA1-mutated breast cancers consisting of 600 probe sets and another for BRCA2-mutated breast cancers also consisting of 600 probes sets. We also defined cgh-array signatures, based on the presence of specific genomic rearrangements, one for BRCA1-mutated breast cancers and one for BRCA2-mutated breast cancers. Conclusions: This study identified molecular signatures of breast cancers with BRCA1 or BRCA2 germline mutations. Genes present in these signatures could be exploited to find new markers for such breast cancers. We also identified specific genomic rearrangements in these breast cancers, which could be screened for in a diagnostic setting using fluorescence in situ hybridization, thus improving patient selection for BRCA1 and BRCA2 molecular genetic analysis.
OBJETIVO: Identificar o perfil clínico de pacientes oncológicos e reações de hipersensibilidade aos agentes antineoplásicos sistêmicos. MÉTODO: Trata-se de um estudo documental e retrospectivo, com dados obtidos de prontuários clínicos de pacientes oncológicos em tratamento quimioterápico. Foram analisados 249 prontuários clínicos entre janeiro de 2013 e janeiro de 2014 para identificar reações de hipersensibilidade e extrair dados demográficos e clínicos. RESULTADOS: Foram identificados seis prontuários de pacientes com episódios de hipersensibilidade aos quimioterápicos. Na amostra 66,7% foram pacientes do sexo feminino, com idade média de 58,4 anos (DP: ± 14,9) e câncer em estágio III (66,7%), sendo os tumores de cólon e ovário os tipos mais prevalentes (33,3%). A incidência de reações de hipersensibilidade foi 2,4%. Nos 12 episódios estudados, o desconforto respiratório foi o sintoma mais frequente (58,3%) e hiperemia foi o sinal mais frequente (50%). O rituximabe foi o agente antineoplásico mais associado a tais reações (33,3%), seguido pela combinação de FOLFOX e bevacizumabe (25%). A maioria dos episódios ocorreram no segundo ciclo quimioterápico (25%). CONCLUSÃO: As reações de hipersensibilidade à quimioterápicos sistêmicos dependem dos fármacos selecionados e das respostas desenvolvidas pelos pacientes, com ampla variação de sinais e sintomas.
