Mục tiêu: Đánh giá kết quả bước đầu của sinh thiết màng hoạt dịch (MHD) dưới hướng dẫn siêu âm trong chẩn đoán một số bệnh lý khớp háng. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang các bệnh nhân có chỉ định và được sinh thiết MHD khớp háng dưới hướng dẫn siêu âm. Mẫu bệnh phẩm sinh thiết sẽ được đánh giá về mô bệnh học, cấy vi khuẩn và xét nghiệm PCR lao. Kết quả: Từ tháng 09/2020 đến tháng 10/2022, có 17 bệnh nhân có chỉ định và được sinh thiết MHD khớp háng dưới hướng dẫn siêu âm tại bệnh viện Đại học Y Hà nội. Có 1 trường hợp không lấy được bệnh phẩm. Số còn lại có 1 bệnh nhân u MHD, 3 bệnh nhân NK sinh mủ, 2 bệnh nhân lao khớp và 10 bệnh nhân viêm MHD không do nhiễm khuẩn. Không có biến chứng xảy ra trong hoặc sau khi sinh thiết. Kết luận: Sinh thiết MHD khớp háng dưới hướng dẫn siêu âm là phương pháp tương đối dễ thực hiện, an toàn, có hiệu quả chẩn đoán cao trong trường hợp các phương pháp khác chẩn đoán chưa rõ ràng.
Mục tiêu: Đánh giá giá trị của CHT trong chẩn đoán lao khớp. Đối tượng và phương pháp nghiên cứu: Phân tích đặc điểm hình ảnh CHT của 07 trường hợp lao khớp được sinh thiết MHD dưới hướng dẫn siêu âm và đối chiếu các dấu hiệu cộng hưởng từ với kết quả sinh thiết. Kết quả: Từ tháng 09/2020 đến tháng 10/2022, có 7 bệnh nhân lao khớp được chụp CHT sau đó được sinh thiết MHD dưới hướng dẫn siêu âm tại bệnh viện Đại học Y Hà nội. Trong số đó có 01 khớp vai, 1 khớp khuỷu, 1 khớp cổ tay, 2 khớp háng và 2 khớp cổ chân. Tuổi trung bình của bệnh nhân là 60±15.5, cao nhất là 75 tuổi, thấp nhất là 35 tuổi. Có 5 bệnh nhân nữ và 2 bệnh nhân nam. Hình ảnh CHT cho thấy độ dày MHD trung bình là 8.9±6.7 mm, độ dày lớn nhất là 24mm, nhỏ nhất là 5.6 mm. Màng hoạt dịch dày đều và ngấm thuốc mạnh trên tất cả các trường hợp. Dịch ổ khớp chỉ có ở 1/7 các trường hợp. Ăn mòn và phù tủy xương thấy trên 6/7 các trường hợp. Không có trường hợp nào có mảnh xương rời. Phù nề kèm áp xe phần mềm quan sát thấy trên 3/6 các trường hợp. Kết quả phân tích mẫu sinh thiết cho thấy toàn bộ các mẫu có nuôi cấy vi khuẩn âm tính. Có 6 BN dương tính trên phân tích mô bệnh học, 4 BN dương tính với PCR lao. Kết luận: Các dấu hiệu hay gặp nhất trên CHT của lao khớp là ăn mòn và phù tủy xương. Các dấu hiệu khác ít gặp hơn và có thể thấy ở nhiễm khuẩn khớp do vi khuẩn sinh mủ.

Mẫu bệnh phẩm sinh thiết sẽ được phân tích mô bệnh học và xét nghiệm PCR lao. Kết quả: Từ tháng 09/2020 đến tháng 10/2022, có 7 bệnh nhân lao khớp được chẩn đoán bằng sinh thiết MHD dưới hướng dẫn siêu âm tại bệnh viện Đại học Y Hà nội. Trong số đó có 01 khớp vai, 1 khớp khuỷu, 1 khớp cổ tay, 2 khớp háng và 2 khớp cổ chân. Tuổi trung bình của bệnh nhân là 60±15.5, cao nhất là 75 tuổi, thấp nhất là 35 tuổi. Có 5 bệnh nhân nữ và 2 bệnh nhân nam. Thang điểm đau khi sinh thiết của bệnh nhânVAS từ 2-4, không có biến chứng xảy ra. Số mẫu sinh thiết từ 5-8 mẫu/bệnh nhân, thời gian sinh thiết từ 10-25 phút. Kết quả phân tích mẫu sinh thiết cho thấy toàn bộ các mẫu có nuôi cấy vi khuẩn âm tính. Có 6 BN dương tính trên phân tích mô bệnh học, 4 BN dương tính với PCR lao. Kết luận: Sinh thiết MHD được chỉ định để chẩn đoán lao khớp khi xét nghiệm và hình ảnh không điển hình. Thủ thuật an toàn, bệnh nhân chấp nhận được. Mục tiêu: Đánh giá giá trị của sinh thiết màng hoạt dịch (MHD) trong chẩn đoán nhiễm khuẩn (NK) khớp. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang các bệnh nhân có chỉ định và được sinh thiết MHD dưới hướng dẫn siêu âm. Sau đó, nhóm bệnh nhân NK khớp sẽ được đối chiếu giữa kết quả sinh thiết và chẩn đoán cuối cùng nhằm xác định độ nhạy, độ đặc hiệu của phương pháp này. Kết quả: Từ tháng 09/2020 đến tháng 10/2022, có 43 bệnh nhân có chỉ định và được sinh thiết MHD tại bệnh viện Đại học Y Hà nội. Trong số này, có 18 bệnh nhân có NK khớp (bao gồm cả NK sinh mủ và lao). Đối chiếu với chẩn đoán cuối cùng, sinh thiết MHD có giá trị chẩn đoán đúng đối với NK khớp là 94.4%. Kết luận: Sinh thiết MHD dưới hướng dẫn siêu âm là phương pháp có giá trị chẩn đoán cao đối với NK khớp trong trường hợp các phương pháp khác chưa rõ ràng.
Mục tiêu: Đánh giá giá trị của cộng hưởng từ (CHT) trong chẩn đoán nhiễm khuẩn (NK) khớp có đối chứng với kết quả sinh thiết màng hoạt dịch (MHD) dưới hướng dẫn siêu âm. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang các bệnh nhân được chụp CHT khớp và được sinh thiết MHD dưới hướng dẫn siêu âm. Sau đó, các dấu hiệu CHT sẽ được đối chiếu với kết quả sinh thiết MHD nhằm đánh giá độ nhạy, độ đặc hiệu của CHT đối với chẩn đoán NK khớp. Kết quả: Từ tháng 09/2020 đến tháng 10/2022, có 43 bệnh nhân được chụp CHT khớp, được sinh thiết MHD dưới hướng dẫn siêu âm tại bệnh viện Đại học Y Hà nội. Trong số này, có 18 bệnh nhân có NK khớp (bao gồm cả NK sinh mủ và lao). Các dấu hiệu CHT của các bệnh nhân này sẽ được đối chiếu với kết quả sinh thiết MHD dưới hướng dẫn siêu âm. Độ nhạy, độ đặc hiệu trong chẩn đoán NK khớp của sự kết hợp bộ 4 dấu hiệu, 3 dấu hiệu và 2 dấu hiệu CHT lần lượt là 50% và 96%, 72% và 84%, 83% và 56%. Kết luận: Khi đối chiếu với kết quả sinh thiết MHD dưới hướng dẫn siêu âm, CHT là phương pháp có giá trị chẩn đoán cao đối với NK khớp.
