The consequences of changes in the oculomotor system on the three-dimensional eye movements are difficult to grasp. Although changes to the rectus muscles can still be approximately understood with simplified geometric models, this approach no longer works with the oblique muscles. It is shown how SEE++, a biomechanical model of the oculomotor plant that was built on the ideas of Miller and Robinson (1984) can improve the understanding of the effects of changes to the oblique eye muscles. By displaying only selected muscles, and by illustrating the relative contribution of these muscles through color-coding the bulb surface, the functional properties of the oblique muscles can be presented in a much clearer way. Investigating the effects of a hyperactive inferior oblique muscle shows that this type of model can help to clarify the functional cause of a pathology, which can otherwise be unclear, even for common pathologies.
Purpose: The goal of this paper is to give an overview of the latest developments in the construction and application of our interactive diagnostic software system for simulating ocular positioning defects.Methods: Object-oriented design techniques in the field of Software Engineering were used to build component based abstractions of our biomechanical model. Our biomechanical model is comprised of geometric representation, muscle force simulation and a kinematical model, enabling interactive substitution of system behavior. Therefore, our system is ideal for training and education purposes, since different model predictions can be evaluated on demand.Results: The implementation of our model is partially based on the work from J. Porrill, P. A. Warren and P. Dean. However, modifications to system behavior and mathematical representation of different types of models were applied. This paper gives a detailed explanation of these modifications, including implementation of rotations using quaternion representation, deployment of two different optimization algorithms for solving the forward and the inverse kinematics and adjustment of these calculations to geometrical properties of the biomechanical model. Model predictions were compared to basic clinical experience and other existing implementations.Conclusions: Finally, this paper gives an outlook on future developments and applications in training and education, extending our system towards an authoring tool, giving the user the possibility to illustrate pathologies and calculations. Moreover, we indent the separation of the "mathematical" kernel from the rest of the system in order to enable the development of clients which can remotely connect to the kernel and use it exclusively for performing calculations. Our aim is to demonstrate the benefits of using a computer model to support the clinical diagnosis of complex eye motility disorders. For diagnosis and differential diagnosis we compared the clinical data of a patient with suspected monocular elevation deficiency (MED) and the corresponding computer simulation with the simulations of rectus superior palsy, vertical Duane miswiring syndrome and two simulations of asymmetric gaze palsy. We used our biomechanical eye model SEE-KID for the computer simulations, which is partly based on ideas and concepts of the software system Orbit™ by Joel Miller. A young patient with the clinical characteristics of congenital MED, unilateral limitation of up-gaze above midline, with accompanying ptosis on the affected right side, mild head posture, chin-up position, partial binocular functions and Bell's phenomenon was examined. Pupillary situation, cover test, version and duction movements, saccadic test, Parks-Bielschowsky phenomenon and head tilt test, stereopsis test, Bagolini striated lens test and forced duction test were assessed. Up to the age of 5 years we used the prism cover test in the nine main gaze positions; later we switched to the Hess-Lancaster test for analyzing deviations. We also used our computer model for evaluating the diagnosis and for differential diagnosis of our patient. The simulation results from the SEE-KID model support the diagnosis of supranuclear MED, which can be achieved in the model by varying central innervations, contrary to the modification of muscle forces in the simulation of a rectus superior palsy. It is necessary to distinguish between supranuclear, nuclear, interstitial or peripheral lesions with regard to monocular elevation deficit. Simulations of patients with similar pathologies in a way that the simulations correspond to the patient-measured values support (beside the clinical signs) the diagnosis of supranuclear or infranuclear lesions.
Zusammenfassung. Zweck: Für die Unterteilung vertikaler Schielformen, hervorgerufen durch schräge Augenmuskel, werden in der englischsprachigen und in der deutschen Literatur verschiedene Kriterien verwendet. Daraus ergeben sich eine unterschiedliche Nomenklatur und teilweise auch unterschiedliche Behandlungsvorschläge. Im Folgenden wird versucht, diese unterschiedlichen Nomenklaturen zu vergleichen und die zugrunde liegende Pathologie mit einem computergestützten Augenmodell zu analysieren. Material und Methode: Die englischsprachige Literatur benützt die Beschreibung des Pathomechanismus für die Unterteilung der vertikalen Schielformen, im Deutschen basiert die Unterteilung in erster Linie auf der Beschreibung der Klinik. Diese beiden unterschiedlichen Klassifikationen verursachen in manchen Fällen ein unterschiedliches Ver -s t ä n d n i s besonders im Fall von Fehlfunktion von schrägen Augenmuskeln oder Pulleyverlagerungen. Der Strabismus sursoadductorius, eine in der deutschen Literatur klar definierte Schielform, ist in der englischen Literatur nahezu unbekannt oder wird mit der primären Obliquus-inferiorÜberfunktion gleichgesetzt. Eine ähnliche Situation besteht beim Strabismus deorsoadductorius. In der vorliegenden Arbeit werden diese verschiedenen Schielfonnen mit Hilfe eines biomechanischen Computermodells analysiert und verglichen. Die Ergebnisse werden mit dreidimensionalen Abbildungen dargestellt und alle Abweichungen in den neun Hauptblickrichtungen im üblichen Schema präsentiert. Schlussfolgerung: Die beiden unterschiedlichen Klassifikationen können nicht komplett verglichen werden. Beide Unterteilungen, aus der englischen und der deutschen Literatur, erlauben jede für sich eine ausreichende Einteilung. Häufig kann im Vergleich kein optimales Gegenstück einer Schielform in der anderssprachigen Literatur gefunden werden, die Einteilungen überlappen sich teilweise bei der Beschreibung der klinischen Symptome.
