In this large, prospective, multinational cohort, more than one half of all cases of non-HACEK gram-negative bacillus endocarditis were associated with health care contact. Non-HACEK gram-negative bacillus endocarditis is not primarily a disease of injection drug users.
The immediate results of treatment for endocarditis are comparable between elderly and younger patients. The long-term prognosis for the elderly patients is worse, mainly related to a higher comorbidity index. Surgery in elderly patients may be a reasonable option, and should be considered in selected elderly patients.
Echocardiography is still underused in elderly patients with HF. Use of echocardiography appears to be associated with more intensive medical therapy and improved outcome.
