A pandemia do SARS-CoV-2 provocou e ainda provoca um grande impacto em diversas esferas da população mundial. O impacto da pandemia da COVID-19 no Sistema Único de Saúde foi evidenciado pela alta exigência de internações, gerando a falta de equipamentos e leitos, uma vez que o SUS já enfrentava dificuldades no manejo de determinadas doenças, tendo que se adaptar de forma repentina para atender à demanda de pacientes com COVID-19. A epidemiologia é ferramenta fundamental na proteção de grandes populações e a coleta de dados e suas interpretações auxiliam a definir manejos mais adequados as doenças. Este trabalho objetivou identificar o perfil epidemiológico de pacientes internados por COVID-19 no Hospital Regional da Asa Norte (HRAN). Trata-se de estudo transversal, quantitativo e comparativo com dados coletados de prontuários eletrônicos de pacientes diagnosticados com infecção por COVID-19 no HRAN, entre janeiro e junho de 2020. Foram Incluídos pacientes ≥ 18 anos portadores de COVID 19, comprovados por exame de RT-PCR positivo, de ambos os sexos, internados nas unidades de clínica médica, clínica cirúrgica e cirurgia plástica. Foram avaliados 106 pacientes, sendo 71 adultos e 35 idosos. Foram investigadas as variáveis sexo, idade, teste RT-PCR confirmatório para COVID-19, tempo de internação, desfecho clínico, comorbidades, exames de imagem, exames laboratoriais e principais medicamentos. A amostra total foi composta por 106 pacientes, sendo 67,0% adultos (n=71) e 33,0% idosos (n=35). As altas representaram 98,1%. Idosos tiveram média de dias de internação e número de comorbidades proporcional maiores que os adultos. Houve presença de hipertensão arterial sistêmica (HAS) e doença renal crônica, estatisticamente maiores entre idosos. Exames de hemoglobina, AST, ALT, CPK e Gama GT foram estatisticamente maiores entre adultos. Foram encontradas grandes elevações em exame de ferritina nos 2 grupos. A tomografia computadorizada (TC) de tórax revelou padrão de vidro fosco em todos os pacientes, com predominância de acometimento bilateral. O tratamento farmacológico revelou o uso de associação de Azitromicina e ceftriaxona em 94,33% dos pacientes sendo que destes 34% (n=34) apresentaram Azitromicina + ceftriaxona + Oseltamivir, 17% (n=17) Azitromicina + ceftriaxona + cloroquina/hidroxicloroquina e 12% (n=12) Azitromicina + ceftriaxona + Oseltamivir + cloroquina/hidroxicloroquina. Os corticoesteroides foram utilizados em 21,6% e os anticoagulantes em 80,1% dos pacientes. A COVID-19 é uma doença nova, potencialmente fatal e de alta infectividade, com pior prognóstico para pacientes de idade avançada, com comorbidades e maior acometimento pulmonar. Entendemos que este trabalho, pode colaborar com a consolidação do perfil epidemiológico, fatores de risco para o desfecho clínico, principais alterações laboratoriais relacionadas à infecção por COVID-19 e ainda a terapia farmacológica instituída por um hospital referência no tratamento da COVID-19.
As doenças infecciosas são um grande problema de saúde pública no mundo atualmente, em especial as transmitidas por meio de vetores. O Aedes aegypti é um mosquito capaz de transmitir algumas doenças virais, entre elas a Dengue, a Zika e a Chikungunya, sendo importante alvo de políticas públicas. A confirmação diagnóstica clínica dessas doenças é insidiosa, entretanto todas necessitam de intervenção precoce para redução de danos subsequentes, principalmente nas fases agudas. A espectrometria de massas é uma técnica analítica capaz de fornecer um diagnóstico seguro e rápido ao identificar características moleculares semelhantes em pacientes infectados, que podem ser relacionadas a caracteres específicos da interação com o hospedeiro. Há moléculas presentes nos vírus bem como outras produzidas no momento da invasão ao organismo que podem ser utilizadas para identificar padrões específicos de cada uma das infecções e confirmar o diagnóstico no momento da atividade viral da doença. Deve-se, para isso, identificar moléculas diferenciais em cada uma das doenças virais e utilizá-las para a resposta diagnóstica precoce. O presente trabalho demonstra que, nas infecções pelos vírus da Dengue, Zika e Chikungunya, existem íons marcadores específicos que apresentam diferentes espectros e podem ser utilizados para definição de rápido diagnóstico. Foram utilizadas quinze amostras de pacientes infectados com dengue, quinze com Zika, quinze com Chikungunya e quinze pacientes hígidos como grupo controle. Para uma melhor eficácia da análise estatística, foram realizados testes para determinar melhores condições para execução dos experimentos da pesquisa avaliando métodos de extração, matriz, concentração de amostra e potência do feixe de laser utilizado. O trabalho pode determinar que na amostra de sangue de pacientes distintos infectados por Zika vírus houve reprodutibilidade no método para o íon marcador 650 m/z do espectro, além de importantes relações de proporcionalidade entre outros íons. Da mesma forma, pacientes infectados com Chikungunya vírus apresentaram reprodutibilidade do íon marcador 633 m/z da mesma relação, bem como o sorotipo II da dengue que apontou os íons 879 m/z e 991 m/z. Além disso, foram observadas as porções polares com bons resultados para dengue tipo II em 725 m/z, assim como os peptídeos com reprodutibilidade de íons marcadores nas bandas 567 e 672 o m/z para a Chikungunya. Foram analisados pacientes coinfectados para observação de reprodutibilidade de fingerprint por esta técnica. Conclui-se, portanto, que há uma especificidade apresentada por esses íons referentes a diferentes moléculas que possibilitam identificar de maneira rápida o fingerprint metabólico destas doenças. Essa utilização de biomarcadores para rápido diagnóstico molecular de arboviroses representa uma das melhores alternativas para o futuro de seu diagnóstico
