Bệnh lý thiếu hụt carnitin nguyên phát (CDSP) gây ra bởi một đột biến lặn trên gen SLC22A5 mã hóa cho protein vận chuyển carnitin (OCTN2) trong huyết thanh và được đặc trưng bởi triệu chứng hạ đường huyết giảm lượng ketone máu (hypoketotic hypoglycemia), bệnh lý về xương và nhược cơ tim có thể dẫn tới tử vong nếu không được điều trị kịp thời [1,2]. Bệnh nhân mắc bệnh thiếu hụt carnitin nguyên phát thường đáp ứng tốt với việc bổ sung L-Carnitin đường uống và cải thiện đáng kể các triệu chứng lâm sàng của họ [3]. Do đó, chẩn đoán sớm và can thiệp kịp thời có ý nghĩa rất quan trọng với bệnh nhân thiếu hụt carnitin nguyên phát. Xét nghiệm phân tích gen SLC22A5 mã hóa protein vận chuyển carnitin có giá trị chẩn đoán xác định bệnh thiếu hụt carnitin nguyên phát bởi kỹ thuật này xác định được chính xác các đột biến trên gen SLC22A5 ảnh hưởng trực tiếp tới việc tổng hợp protein vận chuyển carnitin. Ở Việt Nam hiện nay để xác đột biến gen SLC22A5 chủ yếu sử dụng các bộ mồi được tham khảo từ nghiên cứu trước đây trên thế giới tuy nhiên vẫn gặp phải sự không đặc hiệu về mồi dẫn đến khó xác định được đột biến. Nghiên cứu này tập trung vào thiết kế và tối ưu hóa bộ mồi đặc hiệu trong xác định đột biến gen SLC22A5 gây bệnh thiếu hụt carnitin nguyên phát ở người Việt Nam.
Chiến tranh biên giới Tây Nam là cuộc chiến gần đây nhất mà dân tộc Việt Nam trải qua, nhưng dấu ấn của nó trong văn học hiện đại không sâu đậm như hai cuộc chiến tranh chống Pháp và chống Mỹ. Có thể vì nó chỉ diễn ra trong thời gian ngắn, có thể cuộc chiến tranh ấy xảy ra phần lớn là trên đất bạn Campuchia, cũng có thể giới văn nghệ sĩ và công chúng thuở ấy đã bắt đầu thấm mệt với đề tài chiến tranh vì phải đối mặt với nhiều vấn đề phức tạp của cuộc sống thời bình. Tuy nhiên, từ cuộc chiến ấy vẫn cất lên những tiếng thơ vừa hào hùng vừa u uẩn. Bài viết này phân tích một số tác phẩm thơ của Phạm Sỹ Sáu, Lê Minh Quốc, Đoàn Tuấn, Anh Ngọc - những người đã trực tiếp góp mặt trong chiến tranh biên giới Tây Nam - khai thác cái nhìn hướng ngoại của họ khi quan sát kẻ thù Khmer Đỏ, đất nước và nhân dân Campuchia, cùng cái nhìn hướng nội khi họ chiêm nghiệm về bản thân, đồng đội và tính chất của cuộc chiến tranh mà họ tham gia. Với nhãn quan lịch sử-cụ thể, phương pháp phân tích thi pháp thơ kết hợp với phương pháp so sánh, bài viết cho thấy người lính Tây Nam đã kế thừa tư tưởng và tâm thế của những thế hệ chiến đấu đi trước nhưng đồng thời cũng có những cảm xúc riêng bắt nguồn từ những trải nghiệm thực tế trong một cuộc chiến tranh mang đặc thù khác.
Mục tiêu: Xác định các căn nguyên nhiễm trùng thường gặp gây viêm màng não nước trong tăng lymphocyte trên bệnh nhân điều trị tại Bệnh viện Bệnh nhiệt đới Trung ương. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 144 bệnh nhân được chẩn đoán viêm não và viêm màng não nước trong tăng lymphocyte điều trị tại Bệnh viện Bệnh nhiệt đới Trung ương từ tháng 01/2020 đến tháng 06/2022. Kết quả: Tỷ lệ xác định được nguyên nhân là 61,1%. Nguyên nhân chính gây nên tình trạng viêm màng não nước trong tăng lymphocyte là virus (48,9% trong đó Herpes 28,4%, EV 18,2%, VNNB 2,3%) và lao (38,6%), còn lại là các nguyên nhân khác (C.neoformans 10,3%, Rickettsia 2,3%). 95,3% căn nguyên virus được phát hiện nhờ phương pháp PCR. Kết luận: Vi rút và lao là 2 căn nguyên chính ở bệnh nhân viêm màng não nước trong tăng lymphocyte. Cần áp dụng kỹ thuật PCR dịch não tủy để xác định căn nguyên vi rút gây bệnh.
