Objectives: To determine the percentage of Helicobacter pylori (H. pylori) carrying the outer inflammatory protein A (oipA), duodenal ulcer promotion (dupA) genes, and their relationship with endoscopic imaging, histopathology, and the risk of gastric cancer (GC). Subjects and methods: A cross-sectional study on 89 GC patients, compared with 90 patients with chronic gastritis (CG) with H. pylori infection, diagnosis based on histopathological results at Military Hospital 103 and Central Military Hospital 108 from August 2019 to May 2022. Determination of oipA, dupA genes of H. pylori by realtime-PCR technique on biopsy tissue samples of the gastric mucosa. Results: The percentage of H. pylori carrying the oipA and dupA genes was 30.2% and 10.1%, respectively. The relationship between bacterial infection harboring these 2 genes and endoscopic images and histopathology of GC has not been recorded. There was no difference in the prevalence of H. pylori carrying the oipA and dupA genes infection between the GC and CG groups (p > 0.05). Conclusion: The infection of H. pylori with oipA and dupA genes positive may not increase GC risk in Vietnamese.
Đặt vấn đề: Tính nguyên vẹn của đảo bệnh sinh cag (cagPAI) của H. pylori được nhiều nghiên cứu cho thấy có liên quan với nguy cơ ung thư dạ dày (UTDD). Tuy nhiên, các kết quả nghiên cứu còn chưa thống nhất. Mục tiêu: Nghiên cứu mối liên quan giữa cagPAI của H. pylori với UTDD tại Việt Nam. Đối tượng & phương pháp: Nghiên cứu cắt ngang 179 bệnh nhân (89 bệnh nhân UTDD và 90 bệnh nhân viêm dạ dày mạn) nhiễm H. pylori. Chẩn đoán UTDD và viêm dạ dày mạn (VDDM) dựa trên mô bệnh học. Xác định sự hiện diện của cagPAI thông qua ba gen cagA, cagE, cagT bằng kỹ thuật Realtime-PCR. Kết quả: 100% các chủng H. pylori mang gen cagPAI dương tính, trong đó tỷ lệ H. pylori mang gen cagPAI nguyên vẹn là: 126/179 (70,4%). Nhóm UTDD có tỷ lệ cagPAI nguyên vẹn cao hơn so với nhóm VDDM (79,8% so với 61,1%), sự khác biệt có ý nghĩa thống kê (OR = 2,5; p < 0,01). Ở bệnh nhân UTDD: Không có sự khác biệt về tỷ lệ nhiễm H. pylori mang gen cagPAI theo nhóm tuổi, theo giới, đặc điểm đại thể trên nội soi (theo phân loại Borrmann) và trên mô bệnh học (theo phân loại Lauren). Kết luận: Tỷ lệ cagPAI của H. pylori nguyên vẹn ở bệnh nhân UTDD cao hơn so với tỷ lệ cagPAI ở bệnh nhân VDDM.
Tinh trùng lệch bội nhiễm sắc thể (NST) có thể trực tiếp gây ảnh hưởng xấu tới sức kết quả thai kỳ. Hiện nay, phương pháp lai huỳnh quang tại chỗ (FISH) là một trong những kỹ thuật di truyền tế bào phổ biến nhất và có nhiều ưu điểm nổi trội trong việc xác định lệch bội các NST ở tế bào tinh trùng. Mục tiêu: Hoàn thiện quy trình kỹ thuật lai huỳnh quang tại chỗ trong phân tích lệch bội nhiễm sắc thể từ tế bào tinh trùng. Đối tượng và phương pháp: 15 mẫu tinh dịch được thực hiện lai huỳnh quang tại chỗ để phân tích tỉ lệ lệch bội NST tinh trùng tại Bộ môn Y sinh học – Di truyền, Trường Đại học Y Hà Nội và Trung tâm Xét nghiệm Quốc tế Gentis - Công ty Cổ phần Dịch vụ Phân tích Di truyền Gentis. Kết quả: 15/15 mẫu tinh trùng thực hiện thành công kỹ thuật lai huỳnh quang tại chỗ để phân tích lệch bội cho 5 NST (13, 18, 21, X, Y). Kết luận: Kỹ thuật lai huỳnh quang tại chỗ (FISH) được thực hiện qua nhiều giai đoạn, mỗi giai đoạn đều có những đặc điểm riêng phù hợp với từng đặc điểm cấu tạo, tính chất màng tế bào và DNA tinh trùng để đảm bảo quy trình thành công và kết quả phân tích rõ ràng, cho phép phát hiện chính xác tỉ lệ lệch bội NST từ tế bào tinh trùng.
Mục tiêu nghiên cứu: Mô tả đặc điểm lâm sàng y học cổ truyền của bệnh nhân đau thắt lưng do thoái hóa cột sống tại Trạm y tế phường 1, quận Tân Bình, thành phố Hồ Chí Minh. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 32 bệnh nhân từ 38 tuổi trở lên được chẩn đoán xác định đau thắt lưng do thoái hóa cột sống thắt lưng tại trạm y tế phường 1 quận Tân Bình từ tháng 1/2022 đến hết tháng 5/2022. Kết quả: Tuổi mắc đau thắt lưng do thoái hóa cột sống của bệnh nhân nghiên cứu chủ yếu ở độ tuổi trên 60 tuổi (53%), nữ nhiều hơn nam (66% so với 34%), hầu hết đều là lao động mang vác nặng và lao động chân tay nhẹ (34% và 34%), thời gian mắc bệnh từ 2 6 tháng chiếm tỷ lệ cao nhất (63%). Triệu chứng lâm sàng y học cổ truyền thường gặp nhất là đau vùng thắt lưng và đau tăng khi vận động và giảm khi nghỉ ngơi (100%), sợ lạnh thích ấm (84%), lưng gối đau mỏi, ù tai (69%). Kết luận: Bệnh nhân đau thắt lưng do thoái hóa cột sống thường là nữ, trên 60 tuổi, hầu hết đều là lao động mang vác nặng và lao động chân tay nhẹ, thời gian mắc bệnh trên 6 tháng. Triệu chứng lâm sàng y học cổ truyền thường gặp nhất là đau vùng thắt lưng; sợ lạnh, thích ấm; lưng gối đau mỏi, ù tai.
Đặt vấn đề: Việt Nam là nước có tỷ lệ lưu hành EBV cao, tuy nhiên chưa có nghiên cứu nào về tình trạng nhiễm EBV ở bệnh nhân UTDD. Nghiên cứu này nhằm mục tiêu xác định tình trạng nhiễm EBV và đặc điểm UTDD có EBV dương tính. Đối tượng và phương pháp: Nghiên cứu cắt ngang trên 179 bệnh nhân: UTDD (n = 89) và viêm dạ dày mạn (n = 90) được chẩn đoán bằng mô bệnh học. Tình trạng nhiễm EBV được xác định bằng kỹ thuật realtime-PCR. Kết quả: Tỷ lệ nhiễm EBV nhóm nghiên cứu là 133/179 bệnh nhân (74,3%), trong đó nhóm UTDD cao hơn nhóm VDDM (80,9% so với 67,8%), sự khác biệt có ý nghĩa thống kê (p < 0,05). UTDD có tỷ lệ nhiễm EBV cao nhất ở nhóm tuổi trẻ (dưới 45), vị trí khối u hay gặp ở tâm vị và thân vị (100% và 92,9%, tương ứng). Kết luận: Nhiễm EBV có thể là “nguy cơ” UTDD tại Việt nam. UTDD có EBV dương tính gặp nhiều ở bệnh nhân trẻ tuổi và vị trí khối u ở phần trên của dạ dày.
