Este estudo buscou compreender a experiência de vulnerabilidade da família da criança internada em Unidade de Cuidados Intensivos Pediátricos (UCIP). O Interacionismo Simbólico e o Conceito de Vulnerabilidade da Família foram utilizados como fundamentação para a compreensão dessa experiência. A Análise Qualitativa de Conteúdo foi o referencial metodológico aplicado. Os dados foram coletados por meio de entrevista e observação com 11 famílias de crianças hospitalizadas em uma UCIP de um hospital universitário, do Município de São Paulo. Emergiram seis categorias analíticas da experiência da família que, ao serem comparadas às categorias conceituais da Vulnerabilidade da Família, revelam os elementos definidores do conceito nesse contexto. Para a família, a internação de um filho em UCIP desencadeia sofrimento intenso, pois remete a possibilidade de uma perda definitiva. Assim, o poder e a autonomia da família em relação ao filho são diminuídos, intensificando o sentimento de vulnerabilidade.
Objective: To identify the issues occurred with nursing workers through a Health Monitoring System for Nursing Workers (SIMOSTE) and to describe the consequences of those problems. Method: This is a quantitative, exploratory and descriptive study realized in a teaching hospital in the west region of the city of São Paulo. Results: From the SIMOSTE, 1.847 occurrences were registered in a six month period. Within the main occurrences, medical licenses, work related accidents with and without removals; psychiatric consultations and psychotherapy were highlighted. Conclusion: The data points out to the need for the development of new health vigilance actions to notify accidents and illness related to work, besides the prevention of issues.
OBJETIVO: conhecer as crenças e ações da enfermeira pediatra em relação à autonomia da criança hospitalizada durante a realização de procedimentos terapêuticos. MÉTODOS: estudo de abordagem qualitativa, em que foram realizadas observações e entrevistas com sete enfermeiras de um hospital público de São Paulo. RESULTADOS: foram evidenciados dois temas que revelam as crenças de enfermeira e que fundamentam suas ações em relação à autonomia da criança hospitalizada: acreditando no potencial da criança para exercer a autonomia e considerando a criança incapaz de exercer a autonomia. CONSIDERAÇÕES FINAIS: tem sido dada pouca oportunidade à criança para desenvolver sua autonomia. Como regra, em situações de conflito, o profissional não permite a participação da criança, permanecendo uma distância entre seu discurso e a prática.
Objectives To evaluate whether the presence of pleural abnormalities was a reasonable marker to predict mesothelioma among workers with asbestosis in Hong Kong. Method This is a historical cohort study comprised of 99 male asbestosis workers registered in the Pneumoconiosis Clinic under Hospital Authority of the Hong Kong Government during 1981-2008 who had records of chest radiograph at the time of diagnosis of asbestosis. All asbestosis workers were followed up till 31/12/2008 and the rate of follow-up was 97%. We calculated the sensitivity and specificity of the presence of benign pleural abnormalities (i.e., the presence of benign plaques and/or thickenings at the initial chest radiograph)" using mesothelioma deaths as the "good standard"; meanwhile, the positive predictive value (PPV) and negative predictive value (NPV) were also calculated. Results Benign pleural abnormalities appeared in the initial radiograph for 54 asbestosis workers. We observed 15 mesothelioma deaths and 4 of them had benign pleural abnormalities at the initial chest radiographs. The sensitivity, specificity, PPV, and NPV for using the baseline benign pleural abnormalities to predict mesothelioma deaths was 0.27 (95% CI: 0.078-0.55), 0.63 (95% CI: 0.52-0.73), 0.11 (95% CI: 0.032-0.27), and 0.83 (95% CI: 0.71-0.91). These results remained unchanged when workers with co-presenting cancer at the baseline were excluded from the analyses. Conclusions This study suggests a relatively limited value for using benign pleural abnormalities as markers to predict mesothelioma deaths in workers with asbestosis.
