Early mobilization is beneficial for critically ill patients because it reduces muscle weakness acquired in intensive care units. The objective of this study was to assess the effect of functional electrical stimulation (FES) and passive cycle ergometry (PCE) on the nitrous stress and inflammatory cytometry in critically ill patients. This was a controlled, randomized, open clinical trial carried out in a 16-bed intensive care unit. The patients were randomized into four groups: Control group (n=10), did not undergo any therapeutic intervention during the study; PCE group (n=9), lower-limb PCE for 30 cycles/min for 20 min; FES group (n=9), electrical stimulation of quadriceps muscle for 20 min; and FES with PCE group (n=7), patients underwent PCE and FES, with their order determined randomly. The serum levels of nitric oxide, tumor necrosis factor alpha, interferon gamma, and interleukins 6 and 10 were analyzed before and after the intervention. There were no differences in clinical or demographic characteristics between the groups. The results revealed reduced nitric oxide concentrations one hour after using PCE (Po0.001) and FES (Po0.05), thereby indicating that these therapies may reduce cellular nitrosative stress when applied separately. Tumor necrosis factor alpha levels were reduced after the PCE intervention (P=0.049). PCE and FES reduced nitric oxide levels, demonstrating beneficial effects on the reduction of nitrosative stress. PCE was the only treatment that reduced the tumor necrosis factor alpha concentration.
Objective: To evaluate the impact of inspiratory muscle training (IMT) on the quality of life, immune response, inspiratory and lower limb muscle strength of older adults. Method: A randomized clinical trial was conducted with 30 institutionalized older adults. They were allocated into two groups: the IMT group (n=15), which underwent IMT with PowerBreathe Classic, using a load of 60% of maximal inspiratory pressure (MIP). This was performed using a 30 repetition protocol, three times a week, for six weeks. The second group was the control group (n=15) which did not perform any type of therapeutic intervention. In both groups, MIP, lower limb strength by sit-up test, quality of life by the SF-36 questionnaire and C-reactive protein (CRP) were evaluated. Results: The results demonstrated the homogeneity between the groups regarding the demographic and clinical variables. The IMT group showed an increase in the variation of MIP (9.20±7.36 cmH2O) compared to the control (0.93±8.79 cmH2O). Improvement was also observed in the sitting and standing test (p<0.05) (Tukey Test) in the difference between the values before and after the IMT. In terms of quality of life, two of the eight SF-36 domains were influenced by the IMT, namely: functional capacity and limitations due to physical factors. There were no changes in CRP in either group. Conclusion: IMT improved the inspiratory muscle strength, lower limb strength and quality of life of institutionalized older adults. These findings reinforce the contribution of this therapy to reducing the deleterious effects of aging.
OBJETIVO: O treinamento muscular respiratório vem sendo considerado uma das principais estratégias para amenizar a sarcopenia em idosos, portanto, o presente estudo teve por objetivo verificar quais protocolos de treinamento muscular respiratório são mais utilizados em idosos e os seus principais benefícios encontrados na literatura. MÉTODOS: Foi realizada pesquisa bibliográfica nas bases de dados eletrônicas PubMed, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Scientific Electronic Library Online (SciELO). Para tanto, foram utilizados os termos: treinamento muscular respiratório, idosos e fraqueza muscular. Foi estudado um total de 80 artigos, dos quais apenas 8 preencheram os critérios de inclusão deste estudo, cuja metodologia, variáveis estudadas e desfecho foram analisados. RESULTADOS: Dos oito artigos analisados, podemos observar uma importante diversidade dos protocolos estudados; e em todos os artigos foram encontrados ganhos dos mais variados possíveis com o treinamento muscular respiratório. CONCLUSÃO: Os protocolos utilizados nos diversos estudos dependem diretamente do objetivo a ser alcançado com o treinamento muscular respiratório; e os principais desfechos foram a melhora na força, na função pulmonar, no nível de aptidão física, na qualidade de vida, em marcadores inflamatórios e no consumo da glicose.
Aims: To analyze oxidative cellular stress and monitor the evolution of hematological parameters before and after of neuromuscular electrical stimulation (NMES) in critically ill patients. Methods:A controlled and randomized clinical trial, composed of a sample of 19 patients, admitted to the Agamenon Magalhães Hospital intensive care unit. The patients were divided into two groups: NMES group (n=9), patients that underwent only one NMES in the quadriceps muscle for 20 min, and the other, control group (n=10) that did undergo any therapeutic intervention. Results:In relation to the demographic and clinical variables, the groups were homogeneous at the beginning of the study. For the nitric oxide (NO) analysis, we perceived a reduction, when comparing the before and after analyses, in NO in the stimulated cell (p=0.0188) and non-stimulated cell (p=0.0258) in the NMES group. Also in relation to NO, when comparing the two groups, we observed a significant reduction in the NMES group compared to the control one. For the hematological parameters we did not observe any difference when comparing before and after in the two groups studied. Conclusion:We can conclude that the use of NMES causes a reduction in cellular NO levels, showing the beneficial effects in reducing oxidative stress. With relation to complete blood count, we observed that its application was not able to causes any alterations.Keywords: Nitric oxide; Reactive species of oxygen and nitrogen; Physiotherapy in intensive care and physical exercise
Introdução: O conhecimento do perfil físico/funcional dos pacientes em cuidados paliativos é essencial para adequação e elaboração do plano de tratamento fisioterapêutico e de condutas específicas à essa população. Objetivos: Caracterizar a condição física e funcional dos pacientes em cuidados paliativos do Hospital Universitário de Juiz de Fora, unidade Santa Catarina. Material e Métodos: Estudo transversal. Foram inicialmente triados 31 pacientes e 27 pacientes concluíram o estudo. Todos foram submetidos às seguintes avaliações: nível de consciência (Escala de Coma de Glasgow); sintomas (Edmonton Symptom Assessment System); proteção de vias aéreas (Peak Flow); força muscular periférica (handgrip e Medical Research Council) e respiratória (pressão inspiratória máxima e pressão expiratória máxima); mobilidade funcional e status de performance (Functional Status Score scale; Palliative Performance Scale e Karnofsky Performance Status). Resultados: Fraqueza muscular inspiratória e expiratória foram identificadas, respectivamente, em 69% e 85% dos pacientes e fraqueza muscular periférica em 63% da amostra. Prejuízo na proteção de vias aéreas ocorreu em 79% dos pacientes. Prejuízo da mobilidade funcional foi identificada em 64% da amostra, enquanto 63% (Palliative Performance Scale) e 67% (Karnofsky Performance Status) apresentaram status de performance reduzida. Algum sintoma de dor, fadiga e dispneia foi relatado por 100% dos pacientes que responderam ao Edmonton Symptom Assessment System. Observou-se correlação muito forte entre medidas de avaliação do status de performance e avaliação de mobilidade funcional. As avaliações de status de performance e mobilidade funcional foram correlacionadas fortemente e/ou muito fortemente com a força muscular periférica, força muscular respiratória e proteção de vias aéreas. Conclusão: Grande parte dos pacientes tiveram status de performance reduzida nos testes. Os pacientes apresentaram, ainda, graves prejuízos na proteção de vias aéreas; perda de força muscular respiratória e periférica; acentuados déficits funcionais; e presença de sintomatologia física.
Relação entre a competência funcional da memória episódica e os fatores associados à independência funcional de idosos saudáveis Resultados: Foi encontrada associação estatisticamente significante entre escolaridade e desempenho em tarefa de memória episódica e entre esta e três itens da escala de avaliação das atividades instrumentais de vida diária, embora inconclusiva. Conclusões: Acredita-se que esses resultados se devam ao fato de existir muitas diferenças entre os estudos com relação à metodologia empregada e a escolha dos instrumentos que avaliam tanto a memória episódica, quanto a capacidade funcional de idosos. Além disso, a hipótese de que no envelhecimento saudável existe uma capacidade de, frente às perdas graduais na memória episódica, compensá-las utilizando outras estratégias e recursos, deve receber atenção de futuras pesquisas. AbstractObjective: This study aimed to describe the relationship between the functional competence of episodic memory and its associations with functional independence of healthy elderly in the city of Juiz de Fora, Minas Gerais state, Brazil.
