OBJECTIVE To evaluate the nursing care regarding the intramuscular medication administration procedure in pediatrics. METHODS It was an observational and quantitative study performed in a pediatric hospital located in the city of Fortaleza, in Ceará, Brazil. The data collection was carried out through the observation of 327 intramuscular medication administration procedures. An observational checklist composed by 25 steps was used to collect data. The performance of the 25 steps of the checklist was divided as follows: (1) five steps were applied before the procedure of intramuscular medication administration; (2) fourteen steps during the procedure; (3) six steps after the procedure. RESULTS It was found that none of the five steps carried out before the intramuscular medication administration procedure were satisfactory performed. Regarding the fourteen steps applied during the procedure, six of them were satisfactory performed (42.8%). With regards to the six steps applied after the procedure, three of them were satisfactory performed (50%). CONCLUSIONS Fragilities that negatively impact the intramuscular medication administration procedures in pediatric patients were identified in this study.
Objective: To compare the self-care performed by hypertensive patients monitored in primary and secondary health care. Methods: Cross-sectional study with 189 patients with arterial hypertension in a Basic Health Unit and in an integrated center for diabetes and hypertension in Fortaleza, from March to June, and from September to November of 2013. An interview with a guide based on Orem's self care theory was conducted. For statistical analysis, percentage and chi square test distributions were used. Results: Adequate fluid intake (77.6%), attending nurse consultation (88.0%), and changes in lifestyle (54.3%) had better results with patients monitored in primary care. Appropriate salt consumption (100.0%) and abstinence from alcoholic beverages (88.7%) were more common in patients monitored in secondary care. Conclusion: Hypertensive patients from both primary and secondary care showed a self-care demand, without significant differences.
RESUMO: Objetivou-se avaliar a atuação da equipe de Enfermagem na organização do ambiente, no preparo e na diluição de medicamentos administrados por via intramuscular na pediatria. Estudo observacional, quantitativo, desenvolvido mediante 327 observações do preparo de medicamento intramuscular, no período de dezembro de 2014 a maio de 2015, em um hospital pediátrico, Fortaleza-Ceará. Para observações utilizou-se um checklist composto pelas etapas: organização do ambiente com quatro ações; preparo e diluição de medicamentos com 12 ações. Na organização do ambiente, alcançou-se desempenho satisfatório na ação organizar a bancada, com 231 (70,6%). A etapa de preparo e diluição do medicamento obteve desempenho satisfatório em seis ações: separar bolas de algodão com álcool e seca, com 252 (77%); escolher seringa compatível com o volume a ser administrado, com 264 (80,7%); selecionar adequadamente a agulha para aspiração, com 233 (71,2%), e a agulha para administração, com 320 (97,8%); trocar agulha depois do preparo, com 266 (81,3%); e reconstituir medicação em água destilada, com 327 (100% RESUMEN: Se objetivó evaluar la actuación del equipo de Enfermería en la organización del ámbito, la preparación y dilución de medicamentos de administración intramuscular en pediatría. Estudio observacional, desarrollado sobre 327 observaciones de preparación de medicamentos intramusculares, entre diciembre de 2014 y mayo de 2015 en un hospital pediátrico de Fortaleza-Ceará. Se utilizó en las observaciones un check-list integrado por las etapas: organización del ámbito, con cuatro acciones; preparación y dilución de medicamentos, con 12 acciones. En la organización del ámbito se alcanzó desempeño satisfactorio en acción de organización de mesada, con 231 (70,6%). La etapa de preparación y dilución del medicamento obtuvo desempeño satisfactorio en seis acciones: separar trozos de algodón con alcohol y secos, con 252 (77%); elegir jeringa compatible con volumen a administrar, con 264 (80,7%); selección adecuada de aguja para aspiración, con 320 (97,8%); cambiar aguja luego de preparación, con 266 (81,3%); y reconstitución de medicación con agua destilada, con 327 (100%). Así, fueron identificadas debilidades, como observar fecha de vencimiento del medicamento e identificar la medida preparada, que interfieren con la seguridad de administración de medicamentos en pediatría.
Hygienic-sanitary quality of fresh Minas cheese: a review Qualidade higienicossanitária do queijo Minas frescal: uma revisão
Objetivo: Avaliar a prática da administração de medicamento endovenosa em pacientes adultos internados em hospital de doenças infectocontagiosas. Metodologia: Estudo descritivo, observacional, desenvolvido em hospital de referência em doenças infectocontagiosas de Fortaleza-CE. Foram realizadas 111 observações de profissionais de enfermagem procedendo à administração de medicamentos endovenosos em pacientes com idade > 18 anos. Resultados: As ações mais realizadas foram: lê a prescrição médica (100%), consegue entender a grafia médica (97,2%) e confere o medicamento relacionado à prescrição (90,6%). Entretanto, as menos executadas foram: acalmar o paciente (21,6%), realizar limpeza da bancada (13,5%), usar equipamento de proteção individual (18,9%), desinfetar ampola ou frasco ampola (11,7%). Conclusão: O estudo mostrou diagnóstico situacional da equipe de enfermagem na atuação do preparo e da administração de medicamentos, caracterizando segurança de pacientes no nível sofrível, segundo o índice de positividade. Methodology: Descriptive, observational study, developed at a referral hospital in infectious diseases of Fortaleza-CE. A total of 111 observations were made by nursing professionals on the administration of intravenous drugs in patients> 18 years of age. Results: The most accomplished actions were: read the medical prescription (100%), can understand the medical script (97.2%) and check the medication related to the prescription (90, 6%). However, the least performed were: calming the patient (21.6%), cleaning the workbench (13.5%), using personal protective equipment (18.9%), disinfect ampoule or ampoule vial (11.7%).
