BackgroundMonogenic diseases provide favorable opportunities to elucidate the molecular mechanisms of disease progression and improve medical diagnostics. However, the complex interplay between genetic and environmental factors in disease etiologies makes it difficult to discern the mechanistic links between different alleles of a single locus and their associated pathophysiologies. Inverted formin 2 (INF2), an actin regulator, mediates a stress response—calcium mediated actin reset, or CaAR—that reorganizes the actin cytoskeleton of mammalian cells in response to calcium influx. It has been linked to the podocytic kidney disease focal segemental glomerulosclerosis (FSGS), as well as to cases of the neurologic disorder Charcot–Marie–Tooth disease that are accompanied by nephropathy, mostly FSGS.MethodsWe used a combination of quantitative live cell imaging and validation in primary patient cells and Drosophila nephrocytes to systematically characterize a large panel of >50 autosomal dominant INF2 mutants that have been reported to cause either FSGS alone or with Charcot–Marie–Tooth disease.ResultsWe found that INF2 mutations lead to deregulated activation of formin and a constitutive stress response in cultured cells, primary patient cells, and Drosophila nephrocytes. We were able to clearly distinguish between INF2 mutations that were linked exclusively to FSGS from those that caused a combination of FSGS and Charcot–Marie–Tooth disease. Furthermore, we were able to identify distinct subsets of INF2 variants that exhibit varying degrees of activation.ConclusionsOur results suggest that CaAR can be used as a sensitive assay for INF2 function and for robust evaluation of diseased-linked variants of formin. More broadly, these findings indicate that cellular profiling of disease-associated mutations has potential to contribute substantially to sequence-based phenotype predictions.

Київ, Україна РезюмеМета роботи -з'ясувати основні особливості клінічної характеристики пацієнток, хворих на зовнішній генітальний ендометріоз в періопераційному періоді. Для досягнення поставленої мети були визначені наступні задачі: 1) вивчити локалізацію розподілу ендометріоїдних гетеротопій у пацієнток, хворих на зовнішній генітальний ендометріоз на тлі гіпотиреозу; 2) вивчити структуру хірургічних втручаннь у пацієнток з поєднаною патологією (зовнішній генітальний ендометріоз на тлі гіпотиреозу); 3) вивчити частоту рецидивування ендометріозу у пацієнток з поєднаною патологією після хірургічного лікування. Матеріали і методи. Нами було обстежено 100 пацієнток: 40 -з поєднаною патологією (І основна група); 60 з ендометріозом, без патології щитовидної залози (ІІ група порівняння). Загально-клінічні, лабораторно-інструментальні та променеві методи дослідження (УЗД, КТ, Ro-графія). Діагноз у всіх пацієнтів підтверджений результатами гістопатологічного дослідження. Всі оперативні втручання виконані ендовідео-хірургічним методом. Результати дослідження та їх обговорення. За результатами дослідження локалізації та ступеня поширення ендометріоїдних гетеротопій у пацієнток з поєднаною патологією були отримали наступні результати: I ступінь -38,0% жінок з ендометріоїдними кістами яєчників та аденоміозом; II ступінь -27,0% пацієнток з ендометріоїдним ураженням очеревини і аденоміозом; III ступінь -23,0% хворих з ендометріоїдними кістами яєчників, аденоміозом, з ендометріоїдним ураженням очеревини і сечовивідних органів; IV ступінь -12,0% пацієнток з ендометріоїдними кістами яєчників, аденоміозом, з ендометріоїдним ураженням очеревини і ретроцервікальної області. Структура ендохірургічних втручань у обстеженного контингенту представлена наступним чином: у 80,0% клінічних випадків були проведені органозберігаючі операції; у 20,0% -радикальна операція; у 56,0% -комбінація лапароскопії та гістероскопії; у 42,0% -лапароскопічнийвагінальний доступ і тільки в 2,0% була здійснена конверсія на лапаротомію. Частота випадків рецидиву ендометріозу після хірургічного лікування, у пацієнток, хворих на ендометріоз на тлі гіпотиреозу, склала: через 6 місяців -4,0%; через 9 місяців -6,0%, через 12 місяців -10,0% відповідно. Повне полегшення симптомів досліджуємого захворювання після операції спостерігалося у 80,0% жінок, а у 24,0% жінок відновлена репродуктивна функція. Висновки. Хірургічне лікування пацієнтів з ендометріозом, асоційованим з гіпотиреозом, має базуватися на принципі радикального видалення ендометріоїдних вогнищ, особливо за допомогою комбінованих симультанних операцій у разі тяжкого екстрагенітального ендометріозу. Introduction Menopause is associated with disturbed cardiovascular health. The objective of the study was to compare the effect of hormonal replacement therapy (HRT) and its combination with diet, L-arginine, and xylitol solutions on metabolic processes and cardiovascular health in perimenopausal women. Material and methods In total 106 women were enrolled in the cross-sectional study. The 36 patients of Group II received HRT. 35 women who had been prescribed additionally to HRT a diet, L-arginine, and also xylitol were included in Group III. 35 healthy reproductive-aged women were included in Group I (control). The variables body mass index (BMI), heart rate (HR), blood pressure (BP), lipid and carbohydrate metabolism and C-reactive protein, and menopausal Cooperman’s score were determined before and after the 3-month program. Results The obtained results showed the homogeneity of average age, BMI, HR, BP, and Cooperman’s score in Group II and Group III before inclusion in the study. But several variables changed significantly after a 3-month period. The study showed the effect of the 3-month program on BMI and Cooperman’s score. We also found the restoration of the lipid profiles in Group III. The patients of perimenopausal age featured elevated levels of insulin and C-RP (C-reactive protein). The restoration of levels of insulin and CRP occurred in the process of the 3-month program. Conclusions The use of a 3-month program including diet, xylitol, and L-arginine solutions contributed to the reduction of Cooperman’s score, chronic inflammation, and restoration of lipid and carbohydrate metabolism.
