In einer frfiheren Publikatinn warde darauf hingewiesen, dab wahrend operativer Eingriffe eine Steigerung der freien und Gesamt-Aeiditat des Magensaftes eintreten kann, die nach H/iufigkeit und Grad in verschiedenen Operationsgruppen differiert. Nach Art des anaesthesiologischen und operativen Traumas warden verschiedene Operationsgruppen (s.u.) gebfldet.Es ergab sich zum Beispiel, daf~ in Gruppe I in 35 %, in Gruppe II in 49%, in Gruppe III in 14%, in Gruppe IV in 11% und in Gruppe V in 9 % eine freie Aciditi~t yon fiber 70 Titrationseinheiten gefunden warden (Tabelle 1). ~_hnliche Verh~ltnisse zeigten sich bei Bestimmung der Gesamt-Acidit/~t (Ko~nAD u. Sc~rrz).
Naeh ausgedehnten ahirurgischen Eingriffen, besonders naeh Operationen am Herzen und den grogen Gef/tgen werden mituntar akute Erosionen und Uleera des Magens und Duodenums beobachtet, die, ude I%OTTgOgF, KO~AD u. WILLS~A~N (1957), KO~t~AD U. t~OTT~O~T (1960) und DE~aA u. BI~CKS (1961 zeigten, eine unerwiinsehte und den Patienten belastende postoperative Komplikation darstellen.-(~ber die Entstehung akuter postoperativer Uleera des Magens und Duodenums ist Eindeutiges noch nieht bakannt. Von amerikaniseher Seite wird besonders der ,,operative Stress" zur Erkl/irung herangezogen. Um einer Kls des Problems n~her zu kommen, haben wit zun/~ehst das Verhalten der Magensaftaaidit/~t w/ihrend varsebiedener ehirurgiseher Eingrilfe untersueht.Naehdem einige Tage vor der Operatien die Bestimmung der N/ieh-ternsaftaeidit&t erfolgte, wurde unmittelbar naeh Narkoseeinleitung eine diinne Duodenalsonde transnasal in den Magen vorgesehoben und die erste Magensaftprobe vor 0perationsbeginn aspiriert. Im Verlaufe der Operation wurden mehrere Proben entnommen. Um das effektive intraoperative Magensaftmilieu, wie as sieh aus Naehsekretion, Vermisehung mit t%estsaft, Speiehel, eventuell Galle und Blut sowie AbflieBen in das Duodenum ergibt, zu erfassen, wurden jeweils nut Proben yon je 5--10ml Saft entnommen. Aus diesem Grunde wurde die gesamte produzierte Magensaftmenge nieht bastimmt. Etwa bei der H/~lfte der Patienten wurde 4, 24 und 48 Std postoperativ neohmals Magensaft untersueht. Die Magensaftgewinmmg stieB w/ihrend dar Operation manehmal auf Sehwierigkeiten, die zum Teil teahnisaher Natur waren, andererseits auf mangelnder Saftproduktion beruhten. Um einen gleiahen Ausgangswart zu gew~hrleisten, wurde regelm~tgig mit 1 ml Magensaft, dem etwas Aqua dest. beigeiiigt war, titriert und auf 10 ml umgereehnet. Vergleiehsmessungen mit 1 und 5 ml ergaben eine l~'ehlerbreite yon h6ehstens 38*
