To measure insensible fluid loss from silicone membrane oxygenators during extracorporeal membrane oxygenation (ECMO), an in vitro system was used. A standard neonatal ECMO circuit (Avecor) was connected to a noncompliant reservoir, which was then primed with normal saline. The experiment was conducted by using two silicone oxygenators (Avecor 0.4 and 0.8 m2), three gas flow rates (0.5, 1.0, and 2.0 L/min) (sweep), and two fluid flow rates (200 and 400 ml/min). Two methods were used to measure the water loss. One method was to replace the water to the noncompliant circuit by using a calibrated burette, and the other method was to collect condensed water after cooling the postmembrane sweep gas to 0 degrees C. The influence of the amount of sweep, fluid flow rate, size of membrane, and inlet and outlet sweep gas temperatures on measured water loss was statistically determined. The amount of water loss correlated with sweep (r2 = 0.81; p<0.00001) but was not related to the fluid flow rate, membrane size, or inlet and outlet sweep gas temperature. The average daily fluid loss measured with replacement and collection methods for each liter of sweep per minute were 72.0+/-12.6 and 62.3+/-10.0 ml, respectively. This information may be applied to clinical practice to accurately manage fluid balance in the sick neonate on ECMO.
Introducción: los adultos mayores constituyen un importante segmento de la población, con su alto crecimiento en los últimos años. Son los más propensos a sufrir situaciones de aislamiento social y soledad, y los que menos utilizan las tecnologías para mejorar la comunicación y aumentar la calidad de vida. Objetivo: determinar las distintas técnicas de comunicación para reducir el aislamiento social y/o la soledad en los adultos mayores. Metodología: revisión bibliográfica entre enero y febrero de 2019 en las bases de datos CINAHL, Medline y Scopus. Se obtuvieron un total de 347 artículos en inglés, de los cuales fueron seleccionados finalmente 15. Resultados y discusión: en la mayoría de los estudios revisados se afirma la necesidad de la comunicación con el paciente adulto mayor, para reducir el aislamiento social y la soledad. Los pacientes deberán de aprender las distintas tecnologías que existen y su funcionamiento para una mejor calidad de vida. Conclusiones: una adecuada información e intervención con a los pacientes adultos mayores con aislamiento social y/o soledad, mejora los resultados, disminuyendo así los síntomas que provocan éstas.

Introducción: los últimos avances sobre la prescripción enfermera suponen un importante reconocimiento para la profesión en España. Ante la falta de publicaciones sobre el enfoque del proceso por los medios de comunicación, surge esta revisión. Objetivo: conocer el abordaje periodístico del tema por los principales medios de comunicación españoles durante el año 2018. Metodología: estudio observacional, descriptivo. Se utilizó la base de datos MyNews, combinando una búsqueda general en todos los medios y otra limitada a los principales periódicos consultados en España. Resultados: destacan por número de artículos los medios específicos de salud, y los publicados en formato digital. El pico de publicaciones se dio en el mes de octubre. Las secciones principales fueron “salud” y “sociedad”, según se tratara de prensa especializada en salud o genérica. Las fuentes más utilizadas fueron las etiquetadas como “Instituciones” y “Políticos”. Conclusión: numerosos medios publicaron noticias relacionados con el Real Decreto 1302/2018 durante el año 2018, principalmente medios digitales, específicos en salud. Destaca también el enfoque político desde el que se trata el asunto a través de la prensa generalista. Se incide en la necesidad de crear mecanismos de control de calidad en las publicaciones de contenido en salud.
