Perícias em corpos carbonizados (CC) apresentam grandes dificuldades técnicas, pois a destruição de suas partes prejudica as análises morfológica e toxicológica, bem como dificulta a identificação. Neste artigo é apresentada uma autopsia forense de CC, com ênfase na integração entre os conhecimentos médico legais, toxicológicos e datiloscópicos, e são discutidas as limitações na interpretação dos achados periciais. O grau de destruição tecidual produzido pela ação térmica do presente caso era compatível com a classificação 2, pela escala de Crow-Glassman. A causa médica da morte permaneceu indeterminada pela ausência de elementos periciais que comprovassem exposição à ação térmica em vida. Foi detectada alcoolemia na concentração de 8,81 dg/L. Análises toxicológicas constataram cocaína no humor vítreo e na urina, éster metilecgonina no sangue e na urina e cocaetileno no humor vítreo e vísceras, indicando uso da cocaína associado ao etanol previamente à morte. A mão direita apresentava os dedos fletidos, o que preservou parcialmente uma das polpas digitais. As técnicas utilizadas no preparo de um pequeno fragmento epidérmico permitiram a obtenção adequada de um registro dactiloscópico viável para análise que identificou o corpo. O caso destaca a importância do trabalho integrado entre diversas áreas de conhecimento para a análise forense de CC, das circunstâncias de sua morte, e de sua identificação.
A imagem da esquerda representa uma vista panorâmica da região anterior do coração de uma periciada de 52 anos, vítima de homicídio, ainda em sua localização anatômica (in situ). Na imagem ao centro, observa-se uma vista panorâmica após a abertura do átrio e do ventrículo direitos. A seta vermelha na imagem da direita indica um forame oval patente (FOP), a malformação cardíaca congênita mais frequente, presente em cerca de 25% dos adultos, sem predominância entre os sexos. A forma observada, do tipo ostium secundum, que se localiza na região da fossa oval do átrio direito é a apresentação de comunicação interatrial mais comum. No caso em tela, constituiu um achado necroscópico ocasional, não tendo desempenhado nenhum papel nos mecanismos que produziram a morte da periciada. Apesar de ser assintomático na maioria dos portadores, é um potencial local de formação de trombos que permite, também, a passagem desses de uma câmara atrial para outra, sendo um fator de risco para a ocorrência de um embolismo paradoxal (EP). No EP, um trombo formado em veias profundas dos membros inferiores (ou mesmo um êmbolo gorduroso formado por fraturas de ossos longos), após se deslocar para o átrio direito, passa para o átrio esquerdo e alcança a circulação arterial sistêmica. Pode, assim, obstruir qualquer ramo direto ou indireto da artéria aorta, das coronárias às artérias cerebrais. Portanto, em casos de morte súbita e inesperada com interesse médico legal, este achado deve ser pesquisado como um potencial fator de risco para eventos isquêmicos de origem embólica.
A imagem da esquerda representa visão panorâmica da região anterior das coxas de um periciado (57 anos), morto em confronto com autoridades policiais. Realizou-se uso progressivo da força, sendo utilizada, inicialmente, munição de impacto controlado (projéteis de impacto cinético - PIC), calibre 12 Precision (AM-403/P). Dois dos ferimentos produzidos pelos PIC são destacados nas imagens da direita. Os PIC Precision são constituídos por elastômero macio (polímero de borracha), pesam 19 gramas e medem 6,4 centímetros de comprimento. Tendem a produzir ferimentos superficiais e dolorosos, incapacitando temporariamente o agressor. Como em geral não penetram na pele, sua ação é predominantemente contundente, resultando em escoriações, equimoses, feridas contusas e/ou esmagamentos nos tecidos moles. Dada a deformação tanto da superfície corporal quanto do próprio PIC contra a pele durante o impacto, o diâmetro médio das lesões cutâneas (cerca de 2,5 centímetros) é superior ao diâmetro do próprio projétil (cerca de 1,8 centímetro). O formato anatômico dos PIC confere maior precisão ao tiro, devendo o disparo ser realizado, preferencialmente, a uma distância mínima de 20 metros e contra os membros inferiores, como observado neste caso, evitando-se a cabeça e tronco. O uso em geral desses projéteis previne a necessidade da utilização de munição metálica2,3. Fatalidades envolvendo este tipo de munição são raras quando utilizadas da forma correta. Apesar do potencial poder de contenção dos PIC, no presente caso foi necessário o uso sequencial de projéteis de arma de fogo, o que resultou no óbito do periciado.
Formigas pertencem à ordem Hymenoptera e, com a Diptera e Coleoptera, compõem as ordens de insetos com maior importância forense. Ubíquas em nosso meio, são onívoras oportunistas e podem colonizar rapidamente o cadáver produzindo extensas lesões cutâneas e mucosas. A imagem da esquerda mostra uma vista panorâmica do dorso de uma vítima de homicídio encontrada em área de mata. Na imagem do centro, observa-se detalhe das lesões cutâneas encontradas por todo o corpo, mas concentradas no dorso. Estas lesões apresentavam características típicas de terem sido produzidas por formigas após a morte: áreas irregulares de erosão cutânea, com profundidade variável, mas sem destruição completa da derme, nem sinais de reação vital (tais como infiltrados hemorrágicos, hiperemia ativa ou edema), mostrando-se intercaladas com regiões de pele preservada. A exposição da derme produz seu rápido ressecamento, levando a uma variação cromática das lesões que vão do branco ao marrom escuro, passando pelo alaranjado. Estas lesões são geralmente observadas nas áreas expostas do corpo, como a face e as extremidades dos membros, mas as vestes também podem ser acometidas. Não devem ser confundidas com escoriações ou queimaduras. Particularmente, quando presentes no pescoço, podem simular sinais de constrição cervical. Também são capazes de produzir alterações ou, mesmo, a destruição de lesões produzidas em vida ou de sinais particulares, como tatuagens, o que está indicado pelas setas amarelas na imagem do centro. Quando presentes na região genital ou perineal, como destacado na imagem da direita, não devem ser confundidas com sinais indicativos de traumas relacionados a atos sexuais.
