Objetivo: Determinar las conductas del cuidador que afectan el proceso de crianza de los niños en etapas preescolar y escolar basado en la revisión bibliográfica. Metodología: Esta revisión bibliográfica integrativa fue realizada a través de un estudio detallado, selectivo y crítico que sintetiza la información en una perspectiva unitaria y de conjunto; para mostrar resultados de investigaciones anteriores, y recoger las conclusiones de la literatura sobre las conductas del cuidador que afectan el proceso de crianza de los niños en etapas preescolar y escolar. Resultados: Las conductas más relevantes encontradas en la literatura, que afectan el proceso de crianza de los niños son: el lenguaje inapropiado que usan los cuidadores hacia ellos, la violencia, los malos tratos, la carencia de afecto y apoyo hacia ellos. Además de estas conductas hay un factor comportamental de rol por carencia de tiempo de los padres para compartir con los hijos, esta variable de la falta de tiempo para dedicar a sus hijos, ha generado cuidadores indirectos y cambios en la conducta de los niños por mala utilización del tiempo en ellos. Conclusiones: Esta revisión crítica de la literatura evidencio que conductas simples y cotidianas para algunas personas como el lenguaje inapropiado, la violencia, el tiempo o carencia de este, el poco afecto en el hogar afectan negativamente el proceso de crianza de los niños, y generan daños irremediables en los niños; modificando su estado de ánimo, su manera de ser, su estilo de vida, su salud y su comportamiento a largo plazo ya que serán adolescentes y adultos con dificultades sociales, e incapaces de enfrentar un mundo con adversidades diarias. La situación de riesgo descrita en el entorno familiar de los niños desde una construcción académica de revisión de literatura, requiere una intervención oportuna desde los entes gubernamentales, sociales y jurídicos que generen estrategia pedagógica de intervención y enseñanza para los padres o cuidadores en busca de mejores conductas de crianza a la hora de educar en pro de formar niños buenos, con capacidades extraordinarias, niños sanos, niños capaces de tomar decisiones adecuadas y adultos estructurados que puedan alcanzar una calidad de vida óptima.
Helicobacter pylori is a Gram-negative, microaerophilic bacterium with the ability to survive in extreme acid environments, with pH below 4.0. It is estimated that more than half of the world’s population is infected with this bacterium. Objective: To determine the prevalence, clinical and demographic characteristics of Helicobacter pylori infection in patients undergoing esophagogastroduodenoscopy in two specialized centers of the Coffee Growing Axis in Pereira and Manizales from February to October 2018. Method: Cross-sectional descriptive observational study in which the proportion of positivity to Helicobacter pylori in a period of time was determined. Results: The study included 94 patients; the prevalence of positivity to Helicobacter pylori was 86.1% (81/94), 56 (59.5%) of which were diagnosed by culture and 52 (55.3%) by histology. In the histological study, of the 24 (25.5%) subjects who presented precursor lesions of malignancy, 9 (9.5%) were positive for Helicobacter pylori, while in the 70 subjects (74.5%) without precursor lesions of malignancy, 43 (45.7%) were positive. The significant variables for Helicobacter pylori infection were: Histological response for precursor lesions of malignancy: OR of 0.3808 (95% CI: 0.1408-0.9907) p=0.04 and epigastric pain: OR 0.09489 (95% CI: 0.01928-0.3637) p=0.001. Conclusion: The frequency of Helicobacter pylori infection in the Departments of Risaralda and Caldas is high, and does not discriminate against gender, age, race or origin of the study subjects; Epigastric pain continues to be the most relevant symptom in the diagnosis of Helicobacter pylori infection and the combination of diagnostic tests improves the diagnosis of Helicobacter pylori infection.
Objetivo Caracterizar aspectos gestacionales y complicaciones presentadas en mujeres gestantes que asisten a su control prenatal en una institución de segundo nivel. Metodología Se realizó un estudio de carácter descriptivo, de corte y retrospectivo. Se seleccionaron y revisaron 162 historias clínicas, a partir de una base de datos institucional, de mujeres que ingresaron a su control prenatal en el segundo y tercer trimestre de su embarazo y que hubiesen concluido su etapa de gestación. Los datos se analizaron en el programa SPSS versión 19.0. Resultados El 38,3% de las gestantes no estaban aseguradas al Sistema General de Seguridad Social en Salud y 74,1% fueron clasificadas como de alto riesgo. Las complicaciones más frecuentes fueron: amenaza de parto prematuro (11,7%), infecciones de las vías urinarias (7,4%), hipertensión inducida por el embarazo (6,8%) e infecciones vaginales (5,6%). Se observaron relaciones estadísticamente significativas para: clasificación del riesgo en su primer control, con la presentación de al menos una complicación gestacional (p=0,037); infecciones de vías urinarias y vaginales con la amenaza de parto pretérmino: (p=0,006) y (p=0,012) respectivamente y antecedentes de hipertensión inducida por el embarazo con la presentación de la misma patología en la gestación actual (p=0,001). Conclusiones Las asociaciones encontradas permiten recomendar que los factores de riesgo como las infecciones urinarias y vaginales y el antecedente de hipertensión inducida por el embarazo deben ser intervenidos temp ntación de complicaciones severas en la madre y el feto.
Objetivo general: determinar la efectividad del examen histológico frente al cultivo, para el diagnóstico de Helicobacter pylori, en pacientes del Eje Cafetero, 2018. Materiales y métodos: estudio de pruebas diagnósticas, derivado de una cohorte. Se incluyeron 95 registros de pacientes que requirieron interconsulta por gastroenterología en las capitales de Risaralda y Caldas, se les realizó histopatología para el diagnóstico de infección por Helicobacter pylori (Hp). Para el diagnóstico de Hp se hizo un análisis de los resultados del examen histológico (1 biopsia/paciente) y cultivo (3 biopsias/paciente). Se calculó la sensibilidad, especificidad, valor predictivo positivo y valor predictivo negativo. Resultados: 77,8 % de los pacientes presentaron comorbilidades; el 61,7 % (58/94) de los pacientes presentaron cultivo positivo para H. pylori, mientras que la presencia de H. pylori en el examen histológico fue del 50 %. Se encontró relación estadísticamente significativa entre el resultado del examen histológico y el cultivo como Gold estándar (p=0,001) para el diagnóstico de infección por Hp. Para el examen histológico se dio una sensibilidad del 63,6 % y una especificidad de 71,4 %. Conclusiones: el cultivo resultó ser la mejor prueba para el diagnóstico definitivo de casos positivos de Helicobacter pylori, con niveles superiores de especificidad y sensibilidad; no obstante, el examen histológico resulta una muy buena opción para la detección de Hp, considerando las condiciones de acceso al sistema de salud de la población colombiana.
La infección por Helicobacter pylori afecta alrededor del 50% de la población mundial y es el principal agente etiológico de la gastritis crónica, la ulcera péptica, la duodenal y el cáncer gástrico. Cerca del 30% de los casos son resistentes al tratamiento con antibióticos y se requiere un estricto seguimiento durante el tratamiento. El objetivo de este estudio fue determinar la frecuencia, las características clínicas y sociodemográficas relacionadas con el abandono del tratamiento en un grupo de pacientes sometidos a esofagastroduodenoscopia y diagnosticados con infección por H. pylori en ciudades del eje cafetero 2018. El método utilizado es un estudio observacional, descriptivo y longitudinal. Entre los resultados se observó que, de la totalidad de pacientes (N: 94), el 74,5% debió tratarse (70/94) y, de estos, el 88,6% estuvo en tratamiento (62/70). Al finalizar el estudio, se observó que aproximadamente el 84% de los pacientes que recibieron tratamiento lograron terminarlo (52/62). Se concluye que la frecuencia de abandono fue del 16% y, al comparar las características tanto sociodemográficas como clínicas entre los que terminaron y no terminaron su tratamiento, no hubo ninguna relación significativa.
