The purpose was to study the clinical and epidemiological features of respiratory syncytial virus infection in hospitalized children the first year of life with lower respiratory tract diseases in different regions of the Russian Federation (Russia) during the 2015–2016 epidemic season.Materials and methods: in the original study data of a multicentre observational study conducted on the basis of children’s hospitals in St. Petersburg, Arkhangelsk, Kazan, and Saratov are presented. Etiology of acute respiratory viral infections were confirmed by examination of smears from the posterior pharyngeal wall by polymerase chain reaction. The study sample included 991 child’s first year of life with lesions of the lower respiratory tract.Results. In the etiological structure of the surveyed children in the season of 2015-2016, RSVI ranged from 14% to 46,2%, an average of 33%. RSVI dominated in the Centers of St. Petersburg (38,3%), Arkhangelsk (36.2%) and Kazan (42.5 per cent).RSVI significantly more often (p<0.01) leads to the development of bronchiolitis (29.4% against 16.3% and 10.0%, with rhinovirus infection and parainfluenza, respectively. Patients with RSUI often develop pneumonia (23,5%) vs 20.6% and 20,0% with rhinovirus infection, and parainfluenza. Patients with RSVI often suffer severe forms of the disease require oxygen support (13,8%) and treatment in the departmentof intensive care (15,9%).Seasonal peaks of hospitalization due to RSVI in all Centres were recorded in December-April 2015 and 2016. Regional differences in monthly intensity of hospitalization of children with RSVI were established.Thus, the high prevalence of RSVI among children in the first year of life, especially with heavy and complicated forms of lower respiratory tract disorders requiring intensive care benefits, represents a socially important issue, which requires monitoring for effective prevention in children at risk.
Ñòàòüÿ ïîñâÿùåíà îïðåäåëåíèþ äèàãíîñòè÷åñêîé çíà÷èìîñòè ãåìàòîëîãè÷åñêèõ ïîêàçàòåëåé èíòîêñèêàöèè, ñèñòåìíîé âîñïàëèòåëüíîé ðåàêöèè ïðè ãðèïïå ó äåòåé è ýôôåêòèâíîñòè âêëþ÷åíèÿ ïðåïàðàòà "Ðåàìáåðèí 1,5 % ðàñòâîð äëÿ èíôóçèé" â ïðî-ãðàììó èíôóçèîííîé òåðàïèè ãðèïïà ñ òÿae¸ëûì òå÷åíèåì.  ðàáîòå ïðåäñòàâëåíû ðå-çóëüòàòû êëèíèêî-ëàáîðàòîðíîãî íàáëþäåíèÿ çà 130 äåòüìè, áîëüíûìè ãðèïïîì, â âîç-ðàñòå îò 5 ìåñ äî 12 ëåò. Ëàáîðàòîðíûå ïîêàçàòåëè ñèñòåìíîé âîñïàëèòåëüíîé ðåàê-öèè: Ñ-ðåàêòèâíûé áåëîê, ëåéêîöèòàðíûé èíäåêñ èíòîêñèêàöèè, ãåìàòîëîãè÷åñêèé ïîêàçàòåëü èíòîêñèêàöèè îêàçàëèñü èíôîðìàòèâíûìè äëÿ ðàííåé äèàãíîñòèêè òÿaeå-ëîãî è îñëîaeíåííîãî òå÷åíèÿ ãðèïïà. Ñíèaeåíèå óðîâíÿ ÑÐÁ è ãåìàòîëîãè÷åñêèõ ïîêà-çàòåëåé èíòîêñèêàöèè ïðè âêëþ÷åíèè ðåàìáåðèíà â ïðîãðàììó èíôóçèîííîé òåðàïèè áîëüíûì ñ òÿae¸ëûì òå÷åíèåì ãðèïïà ñâèäåòåëüñòâóåò î âûðàaeåííîì äåòîêñèêàöèîí-íîì äåéñòâèè ïðåïàðàòà. Òàêèì îáðàçîì, âêëþ÷åíèå ðåàìáåðèíà â ïðîãðàììó èíôóçè-îííîé òåðàïèè áîëüíûõ ãðèïïîì ïîçâîëÿåò áûñòðåå êóïèðîâàòü êëèíè÷åñêèå ñèìïòî-ìû çàáîëåâàíèÿ, ÷òî ïðèâîäèò ê ñîêðàùåíèþ ïðîäîëaeèòåëüíîñòè ñòàöèîíàðíîãî ëå÷å-íèÿ.Êëþ÷åâûå ñëîâà: äåòè; ãðèïï; Ñ-ðåàêòèâíûé áåëîê; ãåìàòîëîãè÷åñêèå ïîêàçàòåëè èí-òîêñèêàöèè; ðåàìáåðèí. ÂÂÅÄÅÍÈÅÍàèáîëåå òÿaeåëîé èíôåêöèåé ñðåäè îñòðûõ ðåñïè-ðàòîðíûõ âèðóñíûõ çàáîëåâàíèé ïî êëèíè÷åñêèì ïðî-ÿâëåíèÿì, ÷àñòîòå îñëîaeíåíèé è íåáëàãîïðèÿòíûì èñ-õîäàì ÿâëÿåòñÿ ãðèïï. Ýïèäåìèè ãðèïïà ñîïðîâîaeäà-þòñÿ çíà÷èòåëüíûì ïîäúåìîì ñìåðòíîñòè, íàíîñÿò ñåðüåçíûé óùåðá çäîðîâüþ íàñåëåíèÿ è ýêîíîìèêå ñòðàí. Çàáîëåâàåìîñòü ãðèïïîì ó äåòåé â 4 -5 ðàç ïðå-âûøàåò òàêîâóþ ó âçðîñëûõ [3,4].Òÿaeåëîå òå÷åíèå ãðèïïà õàðàêòåðèçóåòñÿ ðàçâèòèåì ñèìïòîìîêîìïëåêñà èíòîêñèêàöèè, õàðàêòåðèçóþùå-ãîñÿ îáùåé ñëàáîñòüþ, âÿëîñòüþ, îòñóòñòâèåì àïïåòè-òà, íàðóøåíèåì ñíà, ãîëîâíûìè áîëÿìè, òåìïåðàòóð-íîé ðåàêöèåé, òàõèêàðäèåé, âûñîêîé ÷àñòîòîé ðàçâè-òèÿ íåéðîòîêñèêîçà è áàêòåðèàëüíûõ îñëîaeíåíèé â ðàííèå ñðîêè çàáîëåâàíèÿ [3 -5, 8].Ëàáîðàòîðíûìè ïîêàçàòåëÿìè ñèíäðîìà èíòîêñèêà-öèè è ñèñòåìíîé âîñïàëèòåëüíîé ðåàêöèè ÿâëÿåòñÿ èç-ìåíåíèå ñîäåðaeàíèÿ ëåéêîöèòîâ â ïåðèôåðè÷åñêîé êðîâè â âèäå ëåéêîöèòîçà èëè ëåéêîïåíèè, ïîÿâëåíèÿ íåçðåëûõ ôîðì ëåéêîöèòîâ, ïîâûøåíèÿ ëåéêîöèòàð-íûõ è ãåìàòîëîãè÷åñêèõ èíäåêñîâ èíòîêñèêàöèè, ðàç-âèòèå ãèïîêñåìèè, ìåòàáîëè÷åñêîãî àöèäîçà, ïîâûøå-íèå óðîâíÿ îñòðî ôàçíûõ áåëêîâ, â òîì ÷èñëå Ñ-ðåàê-òèâíîãî áåëêà [7].  äîñòóïíîé ëèòåðàòóðå íåäîñòà-òî÷íî îñâåùåíà èíôîðìàòèâíîñòü ãåìàòîëîãè÷åñêèõ ïîêàçàòåëåé èíòîêñèêàöèè è ñèñòåìíîé âîñïàëèòåëü-íîé ðåàêöèè ïðè äèàãíîñòèêå ãðèïïà ó äåòåé.Ïðîáëåìà èíôóçèîííîé òåðàïèè èíôåêöèîííîãî òîêñèêîçà ïðè ãðèïïå ó äåòåé îñòàåòñÿ àêòóàëüíîé. Ïðîäîëaeàåòñÿ ïîèñê ïðåïàðàòîâ äëÿ ïðîâåäåíèÿ èí-ôóçèîííîé òåðàïèè, îáëàäàþùèõ êîìïëåêñíûì äåéñò-âèåì äëÿ êîððåêöèè òîêñè÷åñêîãî ñèíäðîìà, ãèïîêñèè è ÷àñòî âîçíèêàþùèõ ïðè ýòîì ìåòàáîëè÷åñêèõ íàðó-øåíèé.Ñðåäè ðàñòâîðîâ äëÿ ïðîâåäåíèÿ èíôóçèîííîé òå-ðàïèè ïðåïàðàò "Ðåàìáåðèí 1,5 % ðàñòâîð äëÿ èíôó-çèé" (Reamberinum) ïðîèçâîäèòåëü ÎÎÎ "ÍÒÔÔ ÏÎ-ËÈÑÀÍ" (Ñàíêò-Ïåòåðáóðã) çàíèìàåò îñîáîå ìåñòî, îáóñëîâëåííîå âêëþ÷åíèåì â ñáàëàíñèðîâàííûé ïî-ëèèîííûé ðàñòâîð ÿíòàðíîé ...
Проблема выбора эффективной этиотропной терапии острых респираторных вирусных инфекций у детей раннего возраста является одной из актуальных в педиатрии. Материалы и методы: приведены данные о механизме комбинированного противовирусного и иммуномодулирующего действия препарата рекомбинантного интерферона альфа-2b с высокоактивными антиоксидантами витаминами Е и С (ВИФЕРОН ®). Представлены результаты наблюдения за 80 детьми в возрасте от 3 до 12 месяцев с острыми респираторными вирусными инфекциями различной этиологии. Результаты: Выявлены клинико-лабораторные показатели интоксикации, возникающие при острых респираторных вирусных инфекциях у детей. Показано, что применение ректальных свечей интерферона альфа-2b (ВИФЕРОН ®) в терапии детей раннего возраста с острыми респираторными вирусными инфекциями вызывает быстрое купирование клинических симптомов болезни и способствует сокращению продолжительности стационарного лечения больных.
