The principles of beneficence and patient autonomy can clash in various ways; this is the case of bilateral risk-reducing (BRRM) and contralateral riskreducing mastectomy (CRRM). Mastectomy, however, is rarely treated as an ethical issue. The literature of BRRM and CRRM is dominated by the topic of riskassessment. In this chapter, we are going to analyze BRRM and CRRM from a moral point of view. We are going to argue that patients' autonomy and women's self-determination over their own bodies can be considered as an indication of riskreducing mastectomies.
Neuroscience, with the wealth of information it has gathered about the human brain, might easily appear to be the scientific support education has always been looking for, over its centuries of mostly philosophically fueled experimentation. However, neuroscience is far from being some kind of panacea, which, once found, solves all problems of education in the blink of an eye, as some would like to look at it. The authors of this brief summary believe that neuroscience can become a powerful tool in the hands of education only if professionals of both fields speak both languages and understand each other's concepts. This summary is meant to be a kind of first sentence in that conversation, an introduction to the basic concepts of how the social brain works, and also a starting point for those who wish to further their knowledge in this field.
Abstract. Patients with periorbital defects experience social rejection and psychological impairment as a result of their defect. We have developed a procedure to repair the defect which gives aesthetically and functionally satisfactory results. Infraorbital bony reconstruction and reconstruction of the intraorbital anatomical units are performed as primary surgery; supraorbital reconstruction is done secondarily. We make use of implants made of heath-polymerizing prefabricated polymethyl-methacrylate, and have had satisfactory results.
A dohányzó nők életük során a legnagyobb valószínűséggel a várandósság során szoknak le a dohánytermékek használatáról. Ez az időszak az egészségügy számára kiváló esély, amely során kettős – anyai és magzati – egészségnyereség érhető el a leszokási kísérlet szakszerű támogatásával. A dohányzás felfüggesztése kiemelt jelentőségű a várandósság alatt a magzat és az anya érdekében egyaránt. Összefoglaló kéziratunk irodalmi áttekintés, melynek célja, hogy aktuális és praktikus kiegészítő információkkal szolgáljon a várandós dohányzók leszokását támogató szakemberek számára. A jelen áttekintés a PubMed adatbázisban elérhető publikációkon, valamint hazai és nemzetközi irányelveken és összefoglalókon alapul, melyek gyakorlati jelentőségük alapján kerültek beválogatásra. A várandósság alatti dohányzás szülészeti, neonatológiai és pediátriai szövődményeit tanácsos pozitívan átkeretezni, inkább a leszokás előnyeiről beszélni. A minimálintervenciót ajánlott kiegészíteni a gesztációs hétnek megfelelő tanácsadással, szakellátásra irányítással, illetve a várandós nő dohányzó partnerét célzó leszokástámogatással. Várandósok számára elsődlegesen a gyógyszermentes leszokás javasolható, ha azonban ez nem lehetséges vagy eredménytelen, indokolt lehet a nikotinpótló készítmények használata. Az általános nikotinpótló kezeléstől alapvetően három pontban tér el a várandósok nikotinpótló terápiája. Érdemes inkább az orális készítményeket használni a transdermalis nikotinbevitel helyett; óvatosabb titrálási időszak ajánlott; és a kezelés időtartamát tanácsos lerövidíteni. A várandós dohányzók leszokástámogatása során a magatartás-orvoslási módszerek intenzív alkalmazása javasolt. Nagy hangsúly helyeződik a stresszkezelésre, az érzelemszabályozásra, a viselkedésbeli és biológiai visszajelzésekre, az önjutalmazásra és a külső ösztönzők használatára. Ezek a módszerek a kognitív viselkedésterápiás modellen és a motivációs interjús technikákon alapulnak, amelyekre több gyakorlati példát is bemutatunk az összefoglaló közleményben. It is a necessary part of modern medical education that medical students should learn about the binary nature of human beings – biological and cultural – since both have an impact on our behavior. The subject of medical anthropology helps with understanding the mechanisms and lay concepts behind patients’ decisions which is particularly important in our globalized world. The major goal of this course is to help medical students with acquiring cultural competence through theoretical bases and empirical examples that may help them later in their work when they meet patients with different cultural backgrounds. In the present study, we introduce the course of Medical Anthropology as it happens at the Faculty of Medicine, University of Szeged: the so-called Szeged model – its aims, syllabus, strengths, opportunities and possible difficulties. During the development of the subject, we greatly focused on its links to other subjects of behavioral and medical sciences and on its practice-oriented nature. Thus, the course partly contains of lectures and seminars which display cultural variability in relation to biological reality through practical examples. As a result, the topics of medical anthropology prepare the students to use the knowledge as well as skills and attitudes in clinical practice. Orv Hetil. 2019; 160(39): 1527–1532.
