Mục tiêu: Nhận xét kết quả chọc ối ở những trường hợp thai nhi có tăng khoảng sáng sau gáy(KSSG) tại Bệnh viện Sản Nhi Nghệ An.Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả tiến cứu các trường hợp thai phụđược siêu âm trong khoảng tuổi thai 11 tuần đến 13 tuần 6 ngày được đo KSSG và những thai nhicó tăng KSSG có kết quả xét nghiệm di truyền từ dịch ối tại BV Sản Nhi Nghệ An từ tháng 8/2021đến tháng 8/2022.Kết quả: Đặc điểm chung: Trung bình tuổi của thai phụ là 30,1 ± 5,5. Nhóm từ trên 35 tuổichiếm tỷ lệ 20,3%. Tỷ lệ tiền sử sinh con dị tật chiếm 2,0%. Nhận xét kết quả chọc ối ở nhữngtrường hợp thai nhi có tăng khoảng sáng sau gáy: có 61,2% trường hợp được chỉ định chọc hútdịch ối làm nhiễm NST đồ thai nhi do tăng khoảng sáng sau gáy đơn thuần. Còn lại 38,8% là cócác bất thường khác kèm theo. Chọc hút dịch ối vào thời điểm lý tưởng từ 16 đến 20 tuần chiếm tỷlệ 100%. Tỷ lệ bất thường NST là 2,3%.Kết luận: Tỷ lệ bất thường NST là 2,3% trong đó bất thường NST hay gặp nhất là trisomy 21và bất thường NST giới tính.
Mục tiêu: Đánh giá hiệu quả giảm đau sau phẫu thuật chi trên ở trẻ em bằng gây tê đám rốithần kinh cánh tay đường trên đòn dưới hướng dẫn siêu âm.Đối tượng - Phương pháp: 54 bệnh nhi phẫu thuật chi trên, đủ tiêu chuẩn lựa chọn được phânbố ngẫu nhiên vào 2 nhóm: Gây mê có phối hợp gây tê đám rối thần kinh cánh tay đường trên đòndưới hướng dẫn siêu âm (nhóm I) hoặc gây mê đơn thuần (nhóm II).Kết quả: Điểm an thần Ramsay sau mổ: Nhóm I 100% bệnh nhân đạt ≤ 3, nhóm II có 85,18%đạt ≤ 3 và 14,82% bệnh nhân đạt 4 điểm (p<0,05); thời gian giảm đau trung bình ở nhóm I 7,13 ±0,64 giờ; thời gian tỉnh trung bình sau mổ nhóm I 7,85 ± 2,35, nhóm II 13,70 ± 2,65, (p<0,05); sốlần dùng thêm giảm đau sau mổ nhóm I là 1,30 ± 0,47, nhóm II 3,26 ± 0,59, (p<0,05).Kết luận: Hiệu quả giảm đau sau mổ tốt hơn gây mê đơn thuần với: điểm an thần Ramsay saumổ tốt hơn, thời gian tỉnh mổ ngắm hơn, thời gian giảm đau sau mổ kéo dài và số lần dùng thêmgiảm đau sau mổ ít hơn đáng kể so với các bệnh nhi chỉ được gây mê đơn thuần.
