Introdução: O surgimento do Vírus da Imunodeficiência Humana - HIV modificou o perfil clínico-epidemiológico da tuberculose - TB, aumentando sua morbimortalidade. Cerca de 5,6 milhões de pessoas no mundo estão coinfectadas, 350 mil só no Brasil. Objetivo: Identificar o coeficiente de incidência da co-infecção TB/HIV, buscando relacioná-lo com indicadores sociais. Metodologia: Estudo quantitativo, observacional, retrospectivo, socioeconômico, com amostra de pacientes co-infectados no Distrito Federal em 2007, segundo dados do Ministério da Saúde. Resultados: A incidência de TB-HIV foi de 46 casos e os maiores coeficientes ocorreram no Varjão (14,49) e no Paranoá (8,49), que possuem Índice de Desenvolvimento Humano (IDH) menor que o do Distrito Federal (DF). Asa Norte e Cruzeiro, que possuem alto IDH, também apresentaram um alto coeficiente de incidência de TB/HIV. As maiores taxas estiveram entre aqueles com baixa escolaridade. Há maior acometimento da população masculina (78%), especialmente entre 20-39 anos. Conclusão: Áreas de elevado IDH apresentaram um coeficiente de coinfecção maior que o do DF, o que indica a possível disseminação dessas doenças para segmentos sociais mais elevados. A AIDS trouxe consigo outro patamar para a tuberculose, levando à necessidade de estudos e intervenções que abranjam todos os segmentos sociais.
Objetivo: Estabelecer o perfil clínico e epidemiológico dos pacientes atendidos como urgência no setor de Endoscopia Digestiva do Hospital Regional de Sobradinho com protocole de rastreamento de hemorragia digestiva alta no período janeiro de 2019 a dezembro de 2019.Métodos: Trata-se de um estudo descritivo e exploratório, do tipo transversal. Para tanto, será utilizada uma abordagem quantitativa. Assim, foram analisados os prontuários dos pacientes contidos no Track Care que realizaram endoscopia digestiva alta solicitadas com a hipótese diagnóstica de hemorragia digestiva alta no pedido, no Hospital Regional de Sobradinho entre janeiro e dezembro de 2019.Resultados: Foram registrados 137 casos, a idade média foi de 61,7 anos. Registrada maior ocorrência no sexo masculino (61%). A grande maioria dos pacientes (88%) possuía uma ou mais comorbidades prévias. A mortalidade global foi de 27%.Conclusão: A hemorragia pode trazer um risco à vida e a terapêutica endoscópica é um recurso importante para diagnóstico e tratamento das patologias causadoras do sangramento. São necessários mais estudados para que se tenha mais conhecimento sobre o assunto.
RESUMO Introdução: Síndrome neuroléptica maligna é uma doença rara, observada principalmente em homens de meia idade antes da 4ª década de vida, caracterizada por flutuação do nível de consciência, rigidez muscular intensa, disâutonomia (variação de pulso, pressão arterial (PA) e frequência respiratória) e hipertermia. O diagnóstico é clinico e laboratorial, devendo ser excluídas outras causas. Descrição: D.G.S., 43 anos, masculino, casado. Apresentou quadro de surto psicótico sendo medicado com haloperidol e prometazina, evoluindo posteriormente com quadro de rigidez muscular, sudorese, hipertermia, alteração neurológica e elevação de creatinina fosfoquinase (CPK), compatível com o diagnóstico de Síndrome Neuroléptica Maligna, provavelmente secundário a infusão de haloperidol. Conclusão: A Síndrome Neuroléptica Maligna se caracteriza por uma reação ao uso de neurolépticos e leva o indivíduo a apresentar múltiplas alterações orgânicas, possivelmente fatais. O diagnóstico é clínico e laboratorial. O tratamento primordial é a suspensão da droga causadora, não sendo ainda estabelecido um tratamento específico.
Paciente A.S.C, 66 anos, sexo feminino, internada com quadro de icterícia progressiva há 1 semana, associada a prurido, desconforto abdominal, astenia, inapetência, colúria e acolia fecal, além de episódio de febre não aferida. Estava realizando tratamento para Toxoplasmose ocular com Sulfametoxazol-Trimetropim oral há 45 dias. Nega etilismo, tabagismo ou outras comorbidades. Ao exame, hipocorada +/4, ictérica 3+/4, hidratada, acianótica, afebril, com abdome globoso, flácido, indolor, sem visceromegalias palpáveis ou sinais de ascite. Apresentava hiperbilirrubinemia às custas de bilirrubina direta (Bilirrubina Direta:4,2), além de aumento das enzimas canaliculares (Fosfatase Alcalina: 637 U/L e Gama GT: 880) e elevação das transaminases 2 x acima do limite superior da normalidade. Sorologias negativas para hepatites virais. Durante a internação foi realizada colangiorressonância não sendo evidenciada dilatação das vias biliares intra e extra-hepáticas. Diante da hipótese de hepatite medicamentosa, foi suspenso o Sulfametoxazol-Trimetropim e a paciente evoluiu com melhora do quadro clínico e queda progressiva das transaminases, das bilirrubinas e das enzimas canaliculares. A lesão hepática induzida por drogas é uma das causas mais comuns de hepatite no mundo. A maioria dos casos são de natureza idiossincrática, podendo ocorrer lesão hepática com padrão hepatocelular, colestático ou acometimento sobreposto. Os antibióticos são a classe de medicamentos mais implicados na Lesão Hepática Induzida por Drogas, sendo imperativo, portanto, a suspensão do medicamento. Geralmente a variabilidade genética das enzimas do citocromo P450 influenciam na resposta do indivíduo à medicação, levando ao desenvolvimento de injúria hepática aguda em apenas alguns indivíduos. A paciente em questão desenvolveu hepatite aguda medicamentosa secundária ao uso do Sulfametoxazol-Trimetropim, haja vista a exclusão de todas as demais causas de hepatite aguda e recuperação do quadro após duas semanas da suspensão do medicamento.
