INTRODUCTIONColorectal obstructive endometriosis is relatively rare in Japan and its differentiation from malignancy is often difficult. We report a case of rectal obstructive endometriosis.PRESENTATION OF CASEA 37-year-old woman was referred to our hospital with a suspected ileus. Her chief symptoms were left lower abdominal pain and vomiting. Colonoscopy showed an intraluminal mass of redness in the upper rectum. A proctectomy was performed because of the bowel obstruction. The rectum was filled with an intraluminal mass measuring 5 cm × 4 cm, and endometriosis was diagnosed pathologically.DISCUSSIONA preoperative diagnosis of colorectal obstructive endometriosis is often difficult because of the lack of definite diagnostic, clinical, sonographic, or radiological findings that are characteristic of this disease. Medical treatment is not always effective for colorectal obstructive endometriosis, and surgery is often performed.CONCLUSIONColorectal obstructive endometriosis should be considered as a differential diagnosis in cases of various gastrointestinal symptoms in women who are of reproductive age.
ObjectivesDespite evidence of breast cancer screening efficacy, the screening rate has remained less than 50% in Japan. This study aimed to evaluate the effect of an environmental approach offering on‐site mammography in workplaces.MethodsSupermarket stores were randomly assigned into two groups, the intervention group (leaflet and mammography) and the control group (leaflet). From May to July 2018, participants in the intervention group were given a leaflet informing them of the subsidies for breast cancer screening and offered the opportunity to have mammography in their workplaces. Participants in the control group were given the same leaflet, but had to arrange their own screening outside the workplace. The primary outcome was the breast cancer screening rate in 2018. The odds ratio (OR) and 95% confidence interval (CI) for having screening in the intervention group compared with the control group were estimated using multilevel logistic regression.ResultsWe analyzed data from 1624 participants (mean age 53 years) from 25 supermarket stores (intervention: 8 stores, control: 17 stores). Among participants who had not attended screening in the previous year, the screening rate was 7% in the control group and 53% in the intervention group, with an adjusted OR (95% CI) of 14.22 (8.97–22.54). The effect was greater in those who had never attended screening before.ConclusionIn a worksite‐based cluster randomized controlled trial in Japanese supermarket stores, an environmental approach offering mammography in workplaces substantially increased the breast cancer screening rate within 1 year (UMIN000030465).
[Purpose] To determine the motor Functional Independence Measure item and level that
contribute to improvement in Functional Independence Measure gain in the recovery
rehabilitation ward. [Participants and Methods] This study analyzed the data of 1,866
participants who were selected based on four criteria: age, number of days from onset to
admission, length of hospital stay, and motor Functional Independence Measure upon
admission. Moreover, all items examined were recorded. The participants were divided into
two groups, the non-improving and improving group, based on a motor Functional
Independence Measure gain of 22 points. The degree of contribution of each item was
analyzed based on the median motor Functional Independence Measure. Logistic regression
analysis was performed, with the two groups as dependent variables and the item with high
contribution as independent variable; receiver operating characteristic analysis was
performed. [Results] The items that highly contributed to motor Functional Independence
Measure gain were bathing (level 3), dressing (lower body) (level 4), bladder management
(level 5), and stair climbing (level 3). [Conclusion] The results of this study were
suggested that the items that contribute to the improvement in motor Functional
Independence Measure gain in stroke patients with a motor Functional Independence Measure
of less than 50 were related to self-care and at least over moderate assistance.
