Due to the lower than intended number of patients, the results of the primary and secondary end points were not or only barely significant. Dapsone appeared to have a moderately higher corticosteroid-sparing potential than azathioprine. The combination regimen of either drug with oral methylprednisolone is associated with a relatively low 1-year mortality in this vulnerable patient population.
Background: In allergic rhinitis, a relevant outcome providing information on the effectiveness of interventions is needed. In MASK-air (Mobile Airways Sentinel Network), a visual analogue scale (VAS) for work is used as a relevant outcome. This study aimed to assess the performance of the work VAS work by comparing VAS work with other VAS measurements and symptom-medication scores obtained concurrently. Methods: All consecutive MASK-air users in 23 countries from 1 June 2016 to 31 October 2018 were included (14 189 users; 205 904 days). Geolocalized users selfassessed daily symptom control using the touchscreen functionality on their smart phone to click on VAS scores (ranging from 0 to 100) for overall symptoms (global), nose, eyes, asthma and work. Two symptom-medication scores were used: the modified EAACI CSMS score and the MASK control score for rhinitis. To assess data quality, the intra-individual response variability (IRV) index was calculated. Results: A strong correlation was observed between VAS work and other VAS. The highest levels for correlation with VAS work and variance explained in VAS work were found with VAS global, followed by VAS nose, eye and asthma. In comparison with VAS global, the mCSMS and MASK control score showed a lower correlation with VAS work. Results are unlikely to be explained by a low quality of data arising from repeated VAS measures. Conclusions: VAS work correlates with other outcomes (VAS global, nose, eye and asthma) but less well with a symptom-medication score. VAS work should be considered as a potentially useful AR outcome in intervention studies.
S u m m a r y Tacalcitol is a vitamin D analogue which has been developed for the therapy of psoriasis vulgaris. treatment with a twice daily application of 2 ug/g ointment randomized left comparison ition of a 4 wg/g ointm ent male and female d desquamation was influenced significantly more by tacalcitol ointment than by placebo < 0-0001) at every control point, starting from week 2. With regard to the individual symptoms desquamation, infiltration and erythema, the treatment with tacalcitol was also superior to cebo treatment beginning at week 2. Qualitatively, the same results were obtained with the iference assessment of both treated body sides and also the global assessments of efficacy and lefit. Symptoms of local skin irritation which may be related to the active compound or the ointment base were reported by 12-3% of patients. In only one patient, irritation required discontinuation of tacalcitol treatment. Laboratory criteria, including serum calcium, serum phosphate and serum levels of calcitonin, parathormone, 1 a,24-dihydroxy vitamin 1) , and 25hydroxyvitamin D3, did not reveal Furthermore, the dob al assesvsment treatment. c \ r\ demonstrate that the once daily application of a 4 /tg/g an efficacious therapy for psoriasis vulgaris in Caucasian patients, and that its tolerance is good, wherever the lesion is located, including on the face.
Aim/background-The Mx proteins are known to be specifically and dose dependently induced in mononuclear cells (MNC) by type I interferons (IFN). The aim of this study was to establish a staining method for the human intracellular Mx proteins, MxA and MxB, in leucocytes and bone marrow and skin cells. Methods-Several monoclonal antibodies directed against the MxA and MxB proteins were generated. These antibodies were used to stain Mx proteins in both frozen and paraffin wax sections using the standard alkaline phosphatase antialkaline phosphatase (APAAP)
Although health effects of occupational exposures to livestock emissions are known, potential associations between living in the neighbourhood of intensive livestock facilities and the risk of developing respiratory or allergic diseases are still under discussion. During routine school entry examinations in selected rural districts in Lower Saxony we asked parents about respiratory and allergic symptoms of their 5-6 year old children. A questionnaire with standardised questions of the ISAAC-Studies for respiratory and allergic symptoms and corresponding risk factors was used. Complementarily the children were examined for visible signs of flexural dermatitis and in a subsample the SX1-test was used as an in vitro test for inhalative IgE mediated sensibilisations in capillary blood. Individual exposure to bioaerosols (endotoxin, fungi, bacteria and total dust) coming from livestock facilities was estimated using a Lagrange dispersion model based on the emission rates and locations of the lifestock facilities. A total of 7943 questionnaires (response rate over 85%) were analysed, of which 3867 could be used for the correlation analyses between exposition to endotoxin and asthmatic symptoms. Multivariate logistic regression models were analyzed, considering an atopic disease of the parents as a potential effect modifier and not as a confounder. The prevalence of allergic and asthmatic symptoms is similar to the results of other comparable studies, e. g. "wheezing in the last 12 months" 15.9% for boys and 12.9% for girls. An increase in the prevalence of asthmatic symptoms for higher endotoxin levels was observed only for children of atopic parents. The multivariate controlled odds ratio for one unit logarithm endotoxin concentrations is 1.15, p-value 0.016. Similar effects were observed for other asthma indicators, but not for subjective criteria of the exposition, e. g. the distance to the next large livestock facility guesstimated by parents. Among the overall analysed potential associations between exposition to bioaerosols and asthmatic and allergic symptoms only a few were statistically significant. The discussed association between endotoxin and asthmatic symptoms in predisposed children needs further investigation.
