Background:The aim of this study was to investigate the effectiveness of intravenous isoniazid (H) and ethambutol (E) administered in patients with new sputum positive drug-susceptible pulmonary tuberculosis (TB) with tuberculous meningoencephalitis (TM) and human immunodeficiency virus (HIV) co-infection in the intensive phase of treatment. Methods: Fifty-four patients with TB/TM and HIV co-infection were enrolled for this study. Group 1 comprised of 23 patients treated with E and H intravenously, while rifampicin and pyrazinamide were prescribed orally. Group 2 consisted of 31 patients treated with the first-line anti-TB drugs orally. The concentrations of H and E in blood serum were detected using a chromatographic method. Results: A significant improvement in the clinical symptoms and X-ray signs in patients treated intravenously with H and E was observed and compared to group 2. The sputum Mycobacterium tuberculosis positivity was observed during the second month of the treatment in 25.0% of patients from group 1 and 76.1% of the patients from the control group (p=0.003). In addition, nine patients (39.1%) died up to 6 months when H and E were prescribed intravenously compared with 22 (70.9%) in group 2 (p=0.023). Conclusion:In TB/TM with HIV, the intravenous H and E treatment was more effective than oral H and E treatment at 2 months of intensive treatment in sputum conversion as well as in clinical improvement, accompanied by significantly higher mean serum concentrations. In addition, the mortality rate was lower in intravenous H and E treatment compared to oral treatment.
PURPOSE. To evaluate the stability of mixtures of anti-tuberculosis drugs for intravenous administration used for treatment of sensitive and resistant tuberculosis in time and in conditions close to the conditions of the use. MATERIALS AND METHODS. Studied combinations: ethambutol + isoniazid in various concentrations, ethambutol + levofloxacin, ethambutol + moxifloxacin. In the stability study, samples were taken for analysis of the appearance, pH and concentration of drugs at specified times (immediately after mixing, after 16, and 24 hours). RESULTS. The results of the chromatographic study indicate the absence of chemical interaction between the active pharmaceutical ingredients, high physical and chemical stability of the mixtures for up to 24 hours of storage under conditions close to the conditions of use, except the mixture of isoniazid with ethambutol, in which it is recommended to reduce the storage term to 16 hours due to the complexity of interpretation and the nature of potential impurities. CONCLUSIONS. Study results indicate the possibility of simultaneous administration of a mixture of isoniazid and ethambutol when the preparation of the mixture was not more than 16 hours prior to administration and mixtures of ethambutol + levofloxacin and ethambutol + moxifloxacin with the preparation of the mixture no more than 24 hours before administration.

НАМН Украины С.М. Кузнецова (г. Киев).Профессор напомнила о проблеме цереброваску-лярных заболеваний, сообщила о том, что еще в 2011 году в Украине официально было зарегистрировано 3 185 686 человек с разными формами такой патоло-гии, что составляет 7 % населения страны. При этом наиболее частый диагноз при нарушениях мозгового кровообращения -дисциркуляторная энцефалопатия (ДЭП), также известная под названиями «хроническая ишемия головного мозга», «хроническая церебровас-кулярная недостаточность».Докладчик раскрыла актуальность проблемы дис-циркуляторной (сосудистой) энцефалопатии -хрони-ческой прогрессирующей формы недостаточности церебрального кровообращения с развитием много-очагового и/или диффузного поражения головного мозга, проявляющейся неврологической, когнитивной и психопатологической симптоматикой. Ее главные этиологические факторы -артериальная гипертензия, атеросклероз, кардиальная патология, сахарный диа-бет и сочетание этих патологий. Прогрессирование не-врологических и нейропсихологических расстройств может быть вызвано устойчивой и длительной недо-статочностью мозгового кровообращения и/или по-вторными эпизодами дисциркуляции, протекающими с явной клиникой (ОНМК) или субклинически.Проф. С.М. Кузнецова сообщила о том, что, к со-жалению, очень «омолаживается» атеросклероз, и рас-сказала, что факторы, способствующие такому «омо-ложению», это: климатические катаклизмы (цунами, землетрясения); широкий диапазон новых технологий (мобильные телефоны, wi-fi, СВЧ и др.); рост «фарма-кологического груза» с детства; психоэмоциональные перегрузки (экономический и политический кризис); питание (в 5 раз увеличилось потребление пищевых добавок); пандемия и эпидемия вирусных заболеваний (грипп, герпес и др.).
