The orally administered topical active steroid budesonide is highly effective in the therapy of loperamide-refractory chemotherapy (CPT-11 or 5-FU)-induced diarrhea.
The oxazaphosphorine analog ifosfamide (IFO) has demonstrated an increased therapeutic index in a variety of solid tumors and hematologic malignancies compared with its parent compound cyclophosphamide. A fractionated dose schedule over 5 days as continuous infusion in combination with the uroprotective agent sodium-2-mercapto-ethane-sulfonate (mesna) is considered to provide an improved therapeutic/toxic ratio. Stability data of IFO demonstrate long-term stability for use in disposable infusion pumps as outpatient treatment. In all, 52 patients with various malignancies were entered in a feasibility study to receive outpatient continuous infusion of IFO. All patients were required to have a subcutaneous venous port system implanted. The following drug combinations were used: IFO as single agent, IFO/mitoxantrone, IFO/carboplatinum/etoposide, IFO/etoposide/MTX, IFO/epirubicin. Mitoxantrone and epirubicin were given as continuous infusion together with IFO. Starting dose of IFO was between 1.6-2.0 g/m2/day x 5 and was increased in absence of major hematologic or peripheral toxicity. Mesna was given in combination with IFO as continuous infusion at a dose of 50% of that calculated for IFO. No renal, bladder or central nervous system toxicity was observed. In 247 courses of outpatient continuous ifosfamide infusion only few technical complications due to improper handling were documented.
Während tuberkulostatischer Kombinationstherapie mit Isoniazid, Ethambutol und Rifampicin sowie bei Monotherapie mit Rifampicin wurde ein Abfall des radioimmunologisch gemessenen Serum-Digitoxinspiegels beobachtet. Nach mitteischneller Aufsättigung und einer oralen Digitoxin-Erhaltungsdosis von 0,1 mg/d hatten 19 gesunde Probanden einen Serum-Digitoxinspiegel ( ± s) von 39,1 ± 7,5 ng/ml. Die Serum-Digitoxinkonzentrationen von 21 Tuberkulosekranken unter der Kombinationstherapie lagen mit 18,4 ± 3,9 ng/ml signifikant niedriger (P < 0,0005). Bei digitalisierten Tuberkulosekranken betrug der Abfall des Serum-Digitoxinspiegels nach Beginn der Kombinationstherapie trotz Fortführung der Digitoxintherapie (Erhaltungsdosis 0,1 mg/d oral) 53,0 ± 14,90/o innerhalb von 14 Tagen, unter Monotherapie mit Rifampicin 54,0 ± 6,9°/o. Eine Absorptionsstörung oder Verminderung der Eiweißbindung des Digitoxins und seiner Metaboliten kommt als Ursache dieses Effektes nicht in Betracht. Dagegen fand sich unter der Kombinationstherapie eine mit 2,2 ± 0,4 mi/mm gesteigerte »Digitoxin-Clearance«, die bei den unbehandelten Probanden bei 1,2 ± 0,1 mI/mm lag (P <0,0005). Auch die Serum-Halbwertszeit des Digitoxins und seiner Metaboliten war mit 4,5 ± 1,5 Tagen gegenüber 8,2 ± 1,4 Tagen bei der unbehandelten Kontrollgruppe verkürzt (P < 0,0005). Diese Befunde könnten mit einer vermehrten 12-Hydroxylierung des Digitoxins unter dem Einfluß der Kombinationstherapie oder Monotherapie mit Rifampicin über eine Induktion des mikrosomalen NADPH-abhängigen Enzymsystems erklärt werden, das für die Hydroxylierung von Arzneimitteln notwendig ist. Ein direkter Angriff an den Nierentubuli mit Blockierung der Reabsorption des Digitoxins ist jedoch nicht auszuschließen. Digitoxin concentration, measured by radio-immunoassay, was significantly lower in 51 patients in chronic renal failure (23.2 +/- 7.8 mug/l) than in 29 patients in heart failure (26.5 +/- 7.3 mug/l), although both groups were on the same maintenance dose of 0.1 mg daily. Despite a normal serum albumin concentration, digitoxin protein binding was less in uraemic patients than in those with normal renal function. Renal failure did not affect intestinal digitoxin absorption. In patients in chronic renal failure elimination half-time was significantly shorter (5.7 +/- 0.9 days) than in healthy controls (7.6 +/- 1.6 days). There was no significant difference in the excretion of water-soluble ("cardioinactive") digitoxin metabolites in urine between patients in chronic renal and those in heart failure. In patients with normal renal function, of dichloromethane-soluble (cardioactive) metabolites only digitoxin could be demonstrated by thin-layer chromatography. The results indicate that patients in chronic renal failure can safely be given the same dose as those with normal renal function, without danger of over- or underdosage.
Die radioimmunologische Digitalisb estimmung im Blutserum kann durch das Angebot gebrauchsfertiger Testpackungen breitere Anwendung finden. Vor Auswertung der Bestirnmungsergebnisse müssen jedoch Untersuchungen über ihre Genauigkeit und Reproduzierbarkeit sowie über die Eigenschaften des verwendeten Antiserums durchgeführt werden. Während sich in einer Gruppe von 21 herzgesunden Probanden eine lineare Beziehung (r = 0,9076) zwischen der Digitoxinerhaltungsdosis pro kg Körpergewicht und der Serum-Digitoxinkonzentration fand, ließ sich eine solche Korrelation bei 34 Patienten mit Myokardinsuffizienz nicht nachweisen. Dagegen bestand bei den herzinsuffizienten Patienten eine Korrelation zwischen dem Serum-Digitoxinspiegel und der Serum-Albuminkonzentration (r = 0,5629; P < 0,025). Bei der üblichen Therapie mit verschiedener Dosierung lagen die Digitoxinspiegel zwischen 15 und 40 ng/ml. Patienten mit Werten über 45 ng/ml wiesen regelmäßig Intoxikationszeichen auf (n = 12). Eine toxische Kumulation des Digitoxins war bei niereninsuffizienten Patienten (n = 31) im Gegensatz zu dem fast ausschließlich renal eliminierten Digoxin nicht feststellbar. Die Serum-Digitoxinspiegel lagen zum Teil sogar signifikant niedriger als die Werte, die bei gleicher Dosierung bei herzinsuffizienten Patienten ohne Niereninsuffizienz gemessen wurden. Die niedrigsten Serum-Digitoxinspiegel fanden sich in einer kleinen Gruppe von Patienten mit nephrotischem Syndrom (n = 4). Aufgrund der Ergebnisse wird empfohlen, die in der Phase der mittelschnellen. Auf sättigung bisher meist angewandte Vollwirkdosis von 1,8-2,0 mg auf 1,2 bis 1,5 mg Digitoxin zu reduzieren. Als orale Erhaltungsdosis dürfte im allgemeinen 0,7 mg/Woche ausreichend sein. Alle Manuskripte sind ditekt an die Schriftleitung zu richten.
