Gastrointestinal metastases from breast cancer are rare and generally occur several years after the diagnosis of the primary lesion. The diagnosis of gastric metastasis as the initial presentation of breast cancer is even rarer and can potentially mimic gastric carcinoma. We report the case of a 66-year-old female patient submitted to a total gastrectomy because of the histological diagnosis of undifferentiated gastric carcinoma. During the surgical procedure, biopsies of the peritoneum and the liver were performed, which were consistent with metastatic breast invasive lobular carcinoma (ILC). The primary lesion of the breast was detected during the post-operative period, when a 4-cm-long lesion was detected on physical examination and mammography. The revision of the gastric biopsy confirmed the diagnosis of ILC. The authors call attention to the rarity of gastrointestinal metastases as the initial presentation of breast ILC.

INTRODUCTIONOvarian cancer has a high mortality and delayed diagnosis. Inflammation is a risk factor for ovarian cancer, and the inflammatory response is involved in almost all stages of tumor development. Immunohistochemical staining in stroma and epithelium of a panel of cytokines in benign and malignant ovarian neoplasm was evaluated. In addition, immunostaining was related to prognostic factors in malignant tumors.METHODThe study group comprised 28 ovarian benign neoplasias and 28 ovarian malignant neoplasms. A panel of cytokines was evaluated by immunohistochemistry (Th1: IL-2 and IL-8; Th2: IL-5, IL-6, and IL-10; and TNFR1). Chi-square test with Yates’ correction was used, which was considered significant if less than 0.05.RESULTSTNFR1, IL-5, and IL-10 had more frequent immunostaining 2/3 in benign neoplasms compared with malignant tumors. Malignant tumors had more frequent immunostaining 2/3 for IL-2 in relation to benign tumors. The immunostaining 0/1 of IL 8 was more frequent in the stroma of benign neoplasms compared with malignant neoplasms. Evaluation of the ovarian cancer stroma showed that histological grade 3 was significantly correlated with staining 2/3 for IL-2 (P = 0.004). Women whose disease-free survival was less than 2.5 years had TNFR1 stromal staining 2/3 (P = 0.03) more frequently.CONCLUSIONIL-2 and TNFR1 stromal immunostaining are related prognostic factors in ovarian cancer and can be the target of new therapeutic strategies.

To compare parasitism and inflammatory process in esophagus and colon from chronic chagasic patients, immunohistochemistry was carried out to research for T. cruzi and to evaluate the inflammatory infiltrate in the muscular and myenteric plexus in 39 esophagi (20 with and 19 without megaesophagus) and 50 colons (25 with and 25 without megacolon). The frequency of T. cruzi in megaesophagus was 20%, and in megacolon it was 4%. No amastigotes were found in organs without mega; considering the total of esophagi (with and without mega), the frequency of T. cruzi would be 10% and 2% in the colon. Myositis and ganglionitis were more frequent and intense in organs with mega compared to those without mega, and in esophagus compared to colon. Qualitatively, inflammatory infiltration in esophagus and colon, with or without mega, was similar, consisting predominantly of T lymphocytes (CD3+), scarce macrophages (CD68+), and rare B lymphocytes (CD20+).

ABSTRACT. Epithelioid sarcoma is a rare, aggressive soft tissue tumor of unknown histogenesis showing predominantly epithelioid cytomorphology. We conducted a conventional and molecular cytogenetic study of a 27-year-old male with epithelioid sarcoma with angiomatoid features. Cytogenetic analysis of epithelioid sarcoma metaphase spreads by GTG-banding revealed a diploid chromosome complement with structural and numerical aberrations. Comparative genomic hybridization analysis demonstrated the amplification of 3p24~pter, 4p15.2-p16 and 18q23, while chromosome losses involved 3p13-p14, 3q24-q26.1, 9q21, and 11q21. Fluorescence in situ hybridization assessment showed normal hybridization patterns for the C-MYC and CCND1 loci; CCND1 RNA overexpression was detected by real-time polymerase chain reaction analysis. Genetic evaluation of this rare condition may be useful in determining if epithelioid sarcoma is associated with a distinct genetic background.

The well known constant rank constraint qualification [Math. Program. Study 21:110-126, 1984] introduced by Janin for nonlinear programming has been recently extended to a conic context by exploiting the eigenvector structure of the problem. In this paper we propose a more general and geometric approach for defining a new extension of this condition to the conic context. The main advantage of our approach is that we are able to recast the strong second-order properties of the constant rank condition in a conic context. In particular, we obtain a second-order necessary optimality condition that is stronger than the classical one obtained under Robinson's constraint qualification, in the sense that it holds for every Lagrange multiplier, even though our condition is independent of Robinson's condition.

RESUMOAvaliaram-se os efeitos da suplementação de dois tipos de probióticos para cães filhotes da raça Beagle, que receberam dois tipos de dieta de alta e baixa qualidade, sobre os achados em exames de diagnóstico por imagem. Foram utilizados 18 cães filhotes da raça Beagle, distribuídos em três tratamentos inteiramente ao acaso: tratamento 1, controle (ração); tratamento 2, ração com probiótico 1 (Bifidobacterium); e tratamento 3, ração com probiótico 2 (Lactobacillus). O experimento foi dividido em duas fases, cada uma caracterizada pela mudança da dieta de qualidades diferentes. Ao exame ultrassonográfico, foi avaliada a espessura da parede gastrintestinal, e ao radiográfico, a presença de gás em alças intestinais e o espessamento da parede. Observou-se aumento da espessura da parede das alças intestinais (P<0,05), nos animais dos grupos tratados com probiótico 1 e 2 (5,74mm e 6,01mm, respectivamente) em relação aos do grupo-controle (5,03mm). O uso de probióticos não influenciou a produção de gás intestinal. Não houve diferença (P>0,05) entre os tratamentos quanto ao escore de gás intestinal. A avaliação quantitativa permitiu a determinação de um escore de gás intestinal, de forma não invasiva, por meio do software Image J ® . Concluiu-se que o diagnóstico por imagem sustenta a recomendação dessa metodologia como ferramenta auxiliar na avaliação nutricional de dietas para cães.Palavras-chave: cão, ultrassonografia, radiografia, probióticos (P <0.05) in animals treated with probiotic, groups 1 and 2, ((5,74mm and 6,01mm, respectively) ABSTRACT The effects of supplementation with types of probiotics for beagle puppies that received high and low quality diets on the findings of imaging techniques were evaluaded. Eighteen animals were distributed in three randomized treatments: 1, control (diet), 2, diet with a probiotic (Bifidobacterium), and 3, diet with another probiotic (Lactobacillus). The experiment was divided into two phases each characterized by a change of diet. By ultrasonography, the thickness of the gastrointestinal wall was evaluated as well the presence of gas in the bowel and thickening of the wall by radiography. There was an increase in the thickness of the intestinal wall
