The article shows problems of distance education at a medical university. It is proved that despite the specifics of the training of medical professionals, distance education should exist, although in a limited form. The pros and cons of distance education at a medical university were highlighted and analyzed. Particular attention is paid to the problems of introducting distance education in the studying process of disciplines at clinical departments. The selected advantages are aimed at forming a theoretical basis in the training of future doctors. The shortcomings of distance education at a medical university were reviewed in detail, including general (mostly technical and organizational aspects) and inherent only in medical university (lack of immersion in the professional environment, the impossibility of forming clinical thinking, etc.).
Целью работы был анализ истории развития педиатрии по нумизматическим материалам. Отрасль исторической науки нумизматика (от лат. numisma — «монета») зародилась в XIX веке и стала тесно связанной с экономикой, политикой, культурой и правом, включает в себя тематическое изучение монет, медалей и плакет. Лучше всего историю педиатрии иллюстрируют различные формы медальерного изобразительного искусства (экзонумия или паранумизматика), а медаль стала прообразом памятной (мемориальной) монеты. В данной работе представлен каталог 116 нумизматических материалов (97 медалей, 11 плакет, 8 монет), в том числе некоторых уникальных, впервые приведенных. Имеются изображения на монетах и медалях международных общепринятых символов педиатрии — Гор-па-херда, или Гарпократа (мальчика, держащего палец руки у рта), и более поздних (пеликана, кормящего кровью из разорванной груди своих птенцов, и флорентийского запеленатого младенца). В работе по материалам нумизматики отражены этапы изучения использования лечебных факторов, есть ссылки на значительные исторические события, упоминаются краткие биографии медиков (46 персон), внесших неоценимый вклад в формирование педиатрической науки. На медалях можно встретить портреты организаторов службы оказания медицинской помощи детям, видных педиатров и представителей узких специальностей. К большому сожалению, пока память о знаменитых врачах-педиатрах прошлого недостаточно отмечена выпуском нумизматической продукции, поэтому в будущем надеемся на планомерный подход к этому делу, на целенаправленную пропаганду достижений педиатрии средствами нумизматики, которая подает наглядный пример для изучения истории медицины, способствует повышению уровня образованности врачей. Авторы рассчитывают на появление новых интересных материалов таких малых форм изобразительного искусства.
В сообщении 1 были представлены дефиниция нумизматики как исторической науки, аспекты изучения ее составляющих (монеты, медали, плакеты), монеты и медали, на которых изображены международные общепринятые символы педиатрии (Гор-па-херд, или Гарпократ, пеликан, кормящий своих птенцов, и флорентийский младенец), подчеркнута роль видных ученых и врачей, которые внесли неоценимый вклад в развитие педиатрии, и они были представлены на нумизматических материалах, кратко освещены их биографии. Подчеркивалось недостаточное современное использование средств нумизматики в деле увековечивания памяти знаменитых педиатров. В сообщении 2 рассматриваются вопросы истории детской хирургии по нумизматическим материалам, приведены специализированные лечебные учреждения Аргентины, Венгрии, Германии, Латвии, Литвы, Польши, Португалии, России, США, Уругвая, Франции, где проводятся оперативные вмешательства больным детям, освещены научные форумы детских хирургов (конгрессы, съезды, конференции, симпозиумы).
Актуальність. Поширеність анкілозивного спондиліту (АС) серед населення сягає 0,3 %, він значно частіше розвивається у віці 20–30 років. При цьому виділяють дві форми АС — дитячу і дорослу, залежно від віку дебюту захворювання. Проблема діагностики ювенільного АС (ЮАС) належить до найбільш актуальних у педіатричній ревматології. Незалежно від віку хворих частим тяжким системним проявом АС є запалення судинної оболонки ока. При цьому еволюція ЮАС у дорослому віці залишається невизначеною. Мета дослідження: вивчити у хворих на АС частоту й характер перебігу увеїту та оцінити його особливості при захворюванні, що почалося в дитячому і дорослому віці. Матеріали та методи. Обстежено 217 хворих на АС (193 чоловіки і 24 жінки), середній вік становив 38 років. Швидко прогресуючий перебіг констатовано в 21 % спостережень, помірний та високий ступінь активності — в 79 %, ІІ–ІІІ стадія — у 82 %, поліартрит — в 65 %. ЮАС відзначений в 16 % випадків (усі хлопчики), у яких ІІІ стадія спостерігалась удвічі частіше, аніж в інших пацієнтів. Результати. Запалення судинної оболонки ока на етапах формування ЮАС розвивається в 42 % випадків, а у хворих із дебютом захворювання в дорослому віці — у 4,6 раза рідше, до того ж у 1-й групі периферійний, передній, тотальний і задній увеїти співвідносилися як 7 : 5 : 2 : 1, мали однобічний характер, у 4 рази частіше характеризувалися гострим перебігом, тоді як для 2-ї групи властивий передній варіант цієї офтальмопатії, у кожному другому спостереженні є двостороннім інтермітуючим, при цьому незалежно від віку на початку хвороби він тісно пов’язаний із високою активністю АС. У пацієнтів з ЮАС відмічається залежність від наявності гоніту та енцефалопатії, у 2-й групі — від інтегрального індексу активності артриту, наявності кокситу і поширеності спондилопатії. Зі свого боку, у випадках АС 1-ї й 2-ї груп спостерігаються різноспрямовані зв’язки в патогенетичних побудовах офтальмопатії з концентрацією сироваткового C-реактивного протеїну, а чинником ризику є, відповідно, високі показники в крові імуноглобуліну G й туморонекротичного фактора α. Висновок. Початок АС у дитячому віці є чинником ризику тяжкого перебігу такої офтальмопатії, як увеїт.
Актуальність. Анкілозуючий спондиліт є хронічним прогресуючим системним автоімунним ревматичним захворюванням хребта, периферичних суглобів, крижово-клубових зчленувань, ентезисів, зв’язкового апарату й внутрішніх органів, належить до факторів ризику тяжкої судинної патології. Мета дослідження: вивчити при анкілозуючому спондиліті клініко-інструментальний перебіг ангіопатії, з’ясувати патогенетичні зв’язки з імунними показниками. Матеріали та методи. Під наглядом перебували 79 хворих віком від 15 до 66 років (в середньому 38 років), серед яких чоловіки становили 95 %, жінки — 5 %. Тривалість хвороби від перших її проявів дорівнювала 11 рокам, І ступінь активності відмічався в 32 % спостережень, ІІ — у 39 % і ІІІ — у 29 %, а співвідношення повільнопрогресуючого та швидкопрогресуючого перебігу патологічного процесу становило 4 : 1. Периферичний моноолігоартрит констатовано в 47 % від числа хворих, поліартрит — у 53 %. Пацієнтам виконували ехокардіографію, ультразвукову допплерографію судин й біомікроскопію судин кон’юнктиви. Результати. Ангіопатія спостерігається у 85 % від числа хворих на анкілозуючий спондиліт, розвиток якої прямо пов’язаний зі ступенем активності патологічного процесу й обернено — із серопозитивністю за ревматоїдним фактором. Шкірний васкуліт залежить від віку пацієнтів, увеїт, склерит, синдром Рейно, ураження центральної та периферичної нервової системи — від особливостей перебігу суглобового синдрому. Інтегральні клінічні й інструментальні судинні показники вірогідно співвідносяться з імунними параметрами — рівнями ревматоїдного фактора, С-реактивного протеїну, циркулюючих імунних комплексів, значеннями імуноглобулінемії, а ураження судин перебігає з підвищенням значень легеневого артеріального тиску та судинного опору, у генезі яких бере участь C-реактивний протеїн, що має прогностичну значущість.
