Op 17 april 2008 werd in Den Haag, ministerie van VWS, de toolkit regionale Volksgezondheid Toekomst Verkenning (rVTV) gepresenteerd. Onderstaand de geschiedenis van GGD-epidemiologie en de ontwikkelingen die onder andere hebben geleid tot die toolkit rVTV. Tevens een antwoord op de vraag waarom het onderwerp rVTV meestal uitmondt in een dispuut over de epidemiologische taken van GGD'en. En welke bijdrage die lokale epidemiologie zou moeten leveren aan inzicht in de landelijke gezondheidssituatie. Waar staan we over pakweg 10 jaar? De afgelopen 10 jaarDe Wet Collectieve Preventie Volksgezondheid (Wcpv) geeft de functie epidemiologie ten behoeve van publieke gezondheid een wettelijke status. Het college van B&W heeft een bevorderingstaak en draagt zorg voor ''het verwerven van, op epidemiologische analyse gebaseerd, inzicht in de gezondheidssituatie van de bevolking''. In de Wcpv wordt echter niet concreet aangegeven op welke wijze de gemeente die zorgtaak dient uit te voeren. Dit resulteerde in grote lokale diversiteit in beleid en aanpak van publieke gezondheid door gemeenten. Er was een lappendeken van meer dan 50 GGD'en in Nederland die (in samenspraak met hun gemeenten) allemaal zelf bepaalden hoe epidemiologie ingevuld werd. Dus bijvoorbeeld welke gezondheidsdata verzameld werden en de wijze waarop. De vergelijkbaarheid van onderzoeksresultaten was ver te zoeken.GGD'en gingen onderling op zoek naar afstemming bij epidemiologisch onderzoek en samenwerking. Dit leverde immers kwaliteitsverbetering en efficiëntiewinst op. In Limburg bijvoorbeeld werkten al in 1996 de vijf GGD'en samen aan een provinciaal onderzoek bij jongeren in het voortgezet onderwijs.De vakgroep Epidemiologie, onder vlag van de koepelorganisatie GGD-Nederland, was een goed platform om onderzoeken af te stemmen, kennis te delen en thema's op te pakken. Zo formeerde de vakgroep in 1997 een werkgroep om vraagstel-lingen in gezondheidsenquêtes te standaardiseren. Dit initiatief werd in 2000 overgenomen door VWS en resulteerde in een gesubsidieerd project Nationale en Lokale Monitor Volksgezondheid. Ook het RGI-project (Regionale GezondheidsInformatie) is een voorbeeld van landelijke samenwerking. RGI voorziet in landelijke aankoop van sterfte-en ziektecijfers, die na verwerking op een gestandaardiseerde wijze aan alle GGD'en geleverd worden voor lokaal gebruik.In de gewijzigde Wcpv 1 is behalve de verplichting voor gemeenten om elke vier jaar gemeentelijk gezondheidsbeleid te formuleren, uit te voeren en te evalueren, óók de relatie met de epidemiologische functie opgenomen: ''voorafgaand aan de opstelling van de nota gemeentelijk gezondheidsbeleid (...) op landelijk gelijkvormige wijze verzamelen en analyseren van gegevens omtrent deze gezondheidssituatie''.De landelijke sturing in lokale GGD-epidemiologie werd daarmee geleidelijk groter. Onder andere door druk uit te oefenen via de Inspectie Volksgezondheid en eisen te stellen aan het leveren van vergelijkbare indicatoren. Ook stimuleerde VWS kwaliteitsverbetering.
