The majority of the available works have studied distinct hypoxic responses of respiratory and cardiovascular systems. This study examines how these systems interact while responding to hypoxia and whether baseline metrics moderate reactions to a hypoxic challenge. Central hemodynamic, aortic wave reflection, and gas exchange parameters were measured in 27 trained young men before and after 10-min normobaric isocapnic hypoxia (10 % O2). Associations were assessed by correlation and multiple regression analyses. Hypoxic changes in the parameters of pulse wave analysis such as augmentation index (-114 %, p=0.007), pulse pressure amplification (+6 %, p=0.020), time to aortic reflection wave (+21 %, p<0.001) report on the increase in arterial distensibility. Specifically, initially compliant arteries blunt the positive cardiac chronotropic response to hypoxia and facilitate the myocardial workload. The degree of blood oxygen desaturation is directly correlated with both baseline values and hypoxic responses of aortic and peripheral blood pressures. The hypoxia-induced gain in ventilation (VE), while controlling for basal VE and heart rate (HR), is inversely associated with HR and systolic blood pressure. The study suggests that cardiovascular and respiratory systems mutually supplement each other when responding to hypoxic challenge.
Проблема и цель. Физиологические механизмы взаимодействия системы внешнего дыха-ния и сердечно-сосудистой системы при выполнении функции газообмена организма человека с участием хеморецепторов остаются недостаточно изученными. При этом можно полагать, что регулярные занятия конкретными видами спорта формируют специфические хемореактив-ные свойства кардиореспираторной системы. Целью работы было изучение хемореактивных свойств кардиореспираторной системы, а также росто-весовых данных и газообмена у пловцов разной спортивной квалификации.Методология. Introduction:
The oxygen-dependence of myocardial contractility is known but it is not the case for ventricular distension in humans. It has been shown that acute ischemia causes incomplete and retarded relaxation through the decrease in intracellular pH and ATP content.
Purpose:
to verify whether hypoxemia changes the left ventricular relaxation time (LVRT).
Methods:
Overall, 37 physically active normotensive men (age 19-42 years, 20.4±2.3 yrs., M±SD) constituted the study sample. The LVRT was derived from aortic pulse wave analysis (SphygmoCor) and measured as difference between the end of systole (ejection duration point, ED, corresponding to the closure of aortic valve) and the time of peak pressure (T1), generated by LV in the aortic root and corresponding to the first incisura [LVRT,%=(ED-T1)/length of cardiac cycle*100]. Subjects inhaled hypoxic mixture with 10%O2 over 10 min for mean blood SpO2 of 80%. Associations were assessed by partial correlation (age, SBP=const.), multiple linear regression analyses (GLM option in SPSS), and t-test for dependent samples.
Results:
At baseline, the longer length of LVR was associated with lower end systolic blood pressure (r=-0.56, p<0.001). The regression model combining heart rate, augmentation pressure, and aortic-to-radial pulse pressure amplification explained 98.6% of total LVRT variation with significant effects of all covariates. Hypoxia increased LVRT from 19.0±2.8% to 23.1±4.2%, p<0.001. The response of LVRT to hypoxic exposure (range for Δ, -0.7%, +9.1%) was directly correlated with the extent of blood oxygen desaturation (r=0.53, p=0.016).
Conclusion:
The results show that cardiomyocytes retard their relaxation in hypoxic conditions. Cardiomyocytes happened to be quite sensitive to hypoxemia since even 10-min hypoxia was sufficient for elongation of the ventricular relaxation.
The noninvasive method of oscillovasometry, devised previously, was applied to the estimation of arterial pressure, effective radius of large arterial vessels, a number of indices showing elastic vessel properties and change in tone of the vessels on the right upper limb just before sublingual administration of nitroglycerin (0.5 mg) to the patients and 4-5 min after it. A group of 96 patients with different levels of arterial pressure were examined. It was found that all the patients showed a pressure decrease and tachycardia, but about a half of them had a passive response in large arterial vessels in the form of a decrease in the vessel volume, while others showed a decrease in arterial vessel tone (vasodilation), i.e. an active vascular response. From the data we drew the conclusion that there are two types of vascular response (active and passive) and they do not depend on the initial level of arterial pressure. Nitroglycerin probably acts more effectively on large arteries with a high initial tone. The possible mechanism of this phenomenon is discussed.
