Atualmente através do desenvolvimento tecnológico com melhora na qualidade das imagens, nos procedimentos diagnósti-cos e terapêuticos como biópsias, polipectomias, mucosectomias, tatuagem de lesões suspeitas para posterior ressecção ou revisão endoscópica, coloração de mucosa a fim de identificar limites de lesão, magnificação de superfícies para melhor caracterização 1,2,3 , este se tornou o método principal na busca do diagnóstico das doenças colorretais, principalmente tumores e suas lesões precursoras, os pólipos 1,3,4 . Considerando isso, o uso da colonoscopia tem aumentado no decorrer dos anos acompanhado pelo aumento da incidência de complicações 5 , apesar de ser exame invasivo seguro 1 . Mesmo com este aumento, a literatura considera que o risco é menor que o ganho que o exame traz na investigação de tumores colorretais e lesões precursoras 5,6 . PURPOSE:To investigate the differences in Langerhans cells (LCs) populations between HIV-positive and negative anal squamous cell carcinomas patients. METHODS:Twenty five patients (14 HIV-positive and 11 HIV-negative) were evaluated. Paraffin-block transversal thin sections from biopsies of anal squamous cell carcinomas (ASCC) were stained using the anti-CD1A antibody that identifies activated LCs. LCs counts were performed using histometry at 20 different sites, at baseline in the ASCC cases. These were then compared with LCs counts in anal canal specimens from HIV-negative and positive patients without ASCC (controls groups). RESULTS:In patients with ASCC, the LC count was greater among HIV-negative individuals than among HIV-positive individuals (p<0.05). The LC count was greater in the control HIV-negative group than in HIV-positive patients with ASCC (p<0.05). CONCLUSION:There was a lower amount of activated LCs in HIV-positive patients with anal squamous cell carcinomas than in HIVnegative patients, thereby suggesting worsening of the immune response. MÉTODOS:Avaliamos 25 pacientes, sendo 11 HIV-negativo e 14 HIV-positivo portadores do CEC do canal anal. Realizamos estudo com a coloração imunoistoquímica anti-CD1A para avaliar as CL ativadas. Utilizamos as lâminas coradas e pelo método da histometria contamos em 20 campos diferentes as células coradas na camada basal da lâmina própria, onde era evidente a disseminação tumoral.Realizamos dois grupos controles compostos por pacientes submetidos à biopsia anal sem neoplasia (sete pacientes HIV-negativo e quatro HIV-positivo). Comparamos as contagens de CL. RESULTADOS:A quantidade de CL foi superior nos pacientes portadores do CEC do canal anal soronegativo para o HIV, em relação aos soropositivos (p<0,05). A quantidade de CL foi superior no grupo controle HIV-negativo em relação ao grupo composto por pacientes soropositivos portadores do CEC do canal anal (p<0,05). CONCLUSÃO:Houve aumento das células de Langerhans ativadas na área peritumoral dos pacientes soropositivos para o HIV, o que sugere diminuição da resposta imune local.
RESUMO: A incidência do carcinoma espinocelular (CEC) anal e das neoplasias intra-epiteliais anais (NIA) é maior nos pacientes infectados pelo papilomavírus humano (HPV), e está relacionada à imunidade e à infecção pelo vírus da imunodeficiência humana (HIV). A associação com viroses de transmissão sexual indica que seja tumor sexualmente transmitido. A infecção pelo HIV mudou o perfil dos portadores do CEC anal. A doença que anteriormente acometia mulheres na 6 a década de vida, hoje atinge preferencialmente homens na 3ª e 4 a décadas. Nos Estados Unidos, a expectativa de diagnóstico desse tumor aumentou de 19/ 100.000, na época pré-HAART (1992-1995), para 48,3/100.000 no período pós-HAART imediato (1996-1999) e para 78,2/100.000 pessoas por ano, nos anos mais recentes (2000-2003). O tratamento do CEC anal, descrito por Nigro em 1974, combina radio e quimioterapia. Operações de resgate estão indicadas caso haja persistência ou recidiva da doença. Estudos comparando, respectivamente, doentes imunodeprimidos e imunocompetentes vêm mostrado envolvimento linfonodal em 60% e 17%, recidivas em 75% e 6%, boa resposta à radio e quimioterapia em 62% e 85%, toxicidade a esse tratamento em 80% e 30%, e sobrevivên-cia global de 1,4 e 5,3 anos. A contagem sérica baixa de linfócitos T CD4 prediz mau prognóstico. Quando acima de 200/mm 3 , os resultados são comparáveis aos observados entre os imunocompetentes.
Objective The aim of our study was to analyze whether the presence of content in the rectum influences the anorectal manometry examination results in chronically constipated patients. Methods We evaluated 38 chronically constipated patients, 36 women and 2 men, with an average age of 53.55 years of age, all with a score above 10 on the Agachan Constipation Scoring System. All the patients underwent rectal preparation and then had the anorectal manometry examination without rectal content and after 5 min had it with a rectal balloon inflated with 200 ml of air in the rectum. Statistical analysis The statistical parametric paired-t test was applied in order to verify the difference in response between the groups after an intervention, adopting a significant level of 5% (p < 0.05). Results The anal pressure was analyzed from the standard anal manometry examination and we found a statistically significant influence on the manometric results for the patients’ average resting pressures, absolute average contractions, average contractions, evacuations, and sustained contractions in the functional anal canals. Conclusion The presence of rectal content influences the manometric values of average resting pressure, average absolute contraction, average contraction, evacuation, and average sustained contractions in the functional anal canal in the group of chronically constipated patients.
