Background Dentin hypersensitivity (DH) is a sharp toothache that influences a patients' oral health-related quality of life. Oral dentifrices have been marketed for pain relief within a minute for DH. The permanent management of DH is being investigated with the remineralisation potential of bioactive agents in dentinal tubules. This study investigated the relief from pain in DH in one minute after applying over the counter (OTC) dentifrices with Pro-Argin™ and strontium acetate and directly compared them with fluoro-calcium phospho-silicate (FCPS)-based dentifrices for immediate and sustained inhibition of painful stimulus provoking DH. Methods A randomised, controlled, triple-blinded clinical trial was conducted with 140 participants clinically diagnosed with DH and equally randomized into four groups with parallel treatment assignment of FCPS, Pro-Argin™, 8% strontium acetate, and sodium fluoride-based OTC dentifrices, and tested for DH with air blast, mechanical, and water jet stimuli on SCHIFF cold air sensitivity scale (SCASS) and visual analogue scale (VAS) at interim efficacy intervals of one minute, three days, two, four, and six weeks, subsequently. Results A total of 128 participants completed the trial. All the treatment groups showed statistically significant improvement in DH with p < 0.001 relative to baseline at all time points. Pro-Argin™ showed a greater reduction in DH with mean scores of (1.34 ± 0.68) (4.20 ± 1.70) (3.05 ± 2.17) followed by strontium acetate (1.57 ± 0.81) (4.65 ± 1.87) (3.75 ± 1.97) on SCASS and VAS for mechanical and water jet stimuli, one minute after application. There was no statistically significant treatment difference between the two (p = 0.499). FCPS showed the highest reduction in DH on SCASS and VAS for waterjet stimuli with mean scores of (0.97 ± 0.68) (1.80 ± 1.73) and Pro-Argin™ on VAS for mechanical stimuli with mean scores of (2.15 ± 1.92) in six weeks. Conclusion OTC dentifrices with Pro-argin™ and strontium acetate are effective for immediate pain relief from DH, and FCPS could be the best possible treatment option for long term management of DH. Background Vitamin D is not only an essential part of a healthy diet but it is also known as the sunshine hormone. It helps to absorb calcium and phosphate within the body and is essential for the development of teeth and bones in growing children. Deficiency in vitamin D causes weak bones, osteoporosis in older individuals, and osteomalacia in children. It also causes loss of alveolar bone around the teeth, increased dental cavities, and other problems associated with gum disease. It can cause depression, fatigue, and appetite loss. This study aims to observe vitamin D deficiency, insufficiency, and sufficiency among children, adults, adolescents, and elders in the Pakistani population. Methods A cross-sectional survey was conducted with 27,880 individuals referred from the general out-patient-department (OPD) to Dow Diagnostic Research and Reference Laboratory (DDRRL) units at Dow University of Health Sciences (DUHS) Karachi, Pakistan, for a period of three months from January to March 2017. They were examined for laboratory findings of serum 25 hydroxyvitamin D levels to determine vitamin D deficiency, insufficiency, and sufficiency among all age groups of both male and female genders. Results A total of 26,750 individuals with a mean age of 38 ± 18 years were statistically examined for laboratory findings of serum vitamin D levels. Vitamin D deficiency was observed in 56% of individuals with a mean log of 1.01 ± 0.18 ng/ml serum vitamin D levels, vitamin D insufficiency in 20% of individuals with a mean log of 1.38 ± 0.05 ng/ml serum vitamin D levels and vitamin D sufficiency in 24% individuals with a mean log of 1.63 ± 0.12 ng/ml serum vitamin D levels. The highest percentage of individuals deficient in vitamin D were children and adolescents of age ranging between 6 to 18 years. Conclusion The findings of vitamin D deficiency in children and adolescents direct higher authorities in the public health sector to take immediate steps to screen, intervene and educate high-risk populations incorporating vitamin D supplements to establish preventive and therapeutic measures.
Objectives: Oral Submucous Fibrosis is a chronic, high risk, debilitating, pre-cancerous oral condition that is prevalent in Southeast Asian consumers of areca nut. This study was conducted to determine regarding gender specificity with relation to stages of OSF and medical & surgical treatment modalities. Setting: OSF patients treated at Oral Surgery clinics of Dow International Medical College, Dow University of Health Sciences. Period: From January 2016 to November 2017. Design of Study: Cross-sectional Hospital based study. Materials & Methods: Patients were divided into four groups based on their clinical presentations according to Khanna JN and Andrade NN classification. Two groups were divided based on treatment modalities. First group comprised of patients treated conservatively with medications and physiotherapy while the second group comprised of patients treated surgically. Results: Out of 225 patients treated with OSF, 88% of them were males while 12% were females with male to female ratio of 7:1. As per Khanna & Andrade classification, 9.7% belonged to group II, while 34.6% belonged to III and 55.5% of OSF patients belonged to IV-A. 56.8% of patients were treated conservatively while 43.1% were treated surgically. In the group of patients treated surgically, majority 84.6% of patients were treated with buccal fat pad while only 15.4% has nasolabial flap surgery performed. Conclusion: Statistical significant difference male preponderance in OSF disease severity was observed as compared to women in this sample of Pakistani population reporting to tertiary care hospital. A holistic approach should be undertaken with public health policies in place to discourage the use of areca nut products with or without tobacco and lower the burden of OSF in the general population.
Objectives: Tooth morphology plays an important role in dental anthropology& forensic odontology. This study was conducted to determine the frequencies of variationsin occlusal surface patterns in permanent mandibular second molars in a subset of Pakistanipopulation. Data Source: 500 medical and dental students of Dow University of Health Sciencesand healthy volunteers visiting the dental OPD with age range from 17 to 25 years. Design ofStudy: Cross-sectional analytical Study. Setting: Department of Oral Biology, Dr. IshratulIbadKhan Institute of Oral Health Sciences, Dow University of Health Sciences. Period: May 2017till August 2017. Materials & Methods: This analytical study was conducted in the departmentof Oral Biology at Dr Ishratul Ibad Khan Institute of Oral Health Science. Five hundred dentalcasts after interoral exam and impression taking were studied for the variations in the frequencyof of cusps and their relative groove and fissure pattern. Statistical analysis was performed byDescriptive analysis and Chi Square. Results: Out of the fivbe hundred casts that were analysed,51% were of males and 49% were of females. The four-cusped variety of mandibular molars withpresence of distal fovea and distal marginal ridge was the most frequent (95.4%) while the mostprevalent occlusal groove form was “+” shape (88.4%). Frequently present occlusal patternwas the “+4” form (85.4%). Conclusion: The occlusal configurations of mandibular molarsshowed high degree of variability. The most frequently occurring pattern of occlusal surface ofmandibular second molars in this subset of Pakistani population was determined to be “+4”form. These findings can be used for forensic, teaching & clinical purposes by providing aPakistani database for morphological characteristic of mandibular molars.
Objectives: To evaluate the individuality of palatal rugae patten and correlate them in males and females in different age groups by using Trobo’s classification. Study Design: Cross Sectional. Setting: Department of Prosthodontics, Dow Dental College, Dow University of Health Sciences. Period: September 2019 to January 2020. Material & Methods: This study comprised of ninety-four participants with equal number of males and females from 10 years to 70 years, visiting the Prosthodontic department. Study casts included were free of dental abnormality, trauma, malocclusion, orthodontic problems, and deformity in palatal rugae area. Sample size was calculated from software name openepi.com with 95% confidence interval. Data was tabulated and SPSS version 22.0 was used to identify the results. Results: Frequency distribution was performed among age groups in which second group (20 to 39 years) has highest frequency with 52 in number. The cross tab between age group and type of rugae was made which showed group II, having age between 20 to 39 years, had the highest number of palatal rugae configurations amongst all. Conclusion: Sinuous or type E was the predominant type of palatal rugae found in this study. The study did not show significant difference in gender, however number of rugae were found consistently more in young adults with propensity towards individuality. Their use can thus be justified for dental identification in forensic odontology.
