Galt die ADHS vor einigen Jahren noch als eine Erkrankung des Kindes- und Jugendalters, so versteht man heute darunter ein Störungsbild über die Lebensspanne. Mit Beginn im Vorschulalter divergieren die Symptome in Abhängigkeit des Alters. Daraus lässt sich ein Modell ableiten, welches diese Veränderungen (Komorbiditäten, Alltagsbeeinträchtigungen) auf der Zeitachse dokumentiert und mit den jeweils gültigen diagnostischen Kriterien in Verbindung bringt. Ausgehend von einer genetischen Prädisposition und einer neurobiologischen Dysregulation sind Faktoren festzustellen, die über die Lebensspanne andauern, sich altersspezifisch verändern oder nur für einen bestimmten Entwicklungsabschnitt gültig sind. Es wird deutlich, dass das Vorschulalter und der Übergang in das Erwachsenenalter die Phasen darstellen, in denen sich die meisten qualitativen Veränderungen feststellen lassen. Dies hat Implikationen für die klinische Praxis, indem diese Veränderungen zu Zwecken der Prävention, des diagnostischen Prozesses und der Therapieplanung herangezogen werden können und somit sowohl dem Anwender eine Hilfestellung bieten, aber auch als Ausgangspunkt für weitere Studien herangezogen werden können.
In a cross-sectional study the influence of social inequality on resilience and psychological distress was investigated in a sample of N=4 142 adults. A social stratum was created, including education, financial income and job-status, as well as age (≥ 25 years). Multivariate analysis of variance (MANOVA) showed influences of gender, social status and age on resilience (RS-11) and psychological distress: depression (PHQ-2), anxiety (GAD-7), life satisfaction (FLZ(M)). In contrast to the most continuous influence of the social background in women across any age-group, older males (≥ 65 years) were not affected by their social background. In both sexes members of the social underclass had the lowest resilience. The results indicate the need for specific intervention as to prevention.
Zusammenfassung: Es wird der Forschungsstand zu sozialer Schichtzugehörigkeit und Gesundheit beleuchtet und hinsichtlich altersspezifischer Merkmale diskutiert. Daraus ergeben sich drei mögliche Entwicklungsverläufe, die sich in Divergenz, Kontinuität und Konvergenz ausdrücken. Um Diskrepanzen im psychischen und körperlichen Gesundheitsstatus schichtabhängig und altersspezifisch darzustellen, wurde eine bevölkerungsrepräsentative Stichprobe (N = 2222) mittels zweifaktorieller multivariater Kovarianzanalyse untersucht (Auflösung von Interaktionen in Einfacheffekte). Schichtunabhängig ließ sich eine Abnahme des psychischen und körperlichen Funktionsniveaus über die Altersspanne feststellen. Es ergab sich eine erhebliche Diskrepanz psychischer und körperlicher Belastungen zwischen den verschiedenen sozialen Schichten, indem mit Abnahme des sozialen Status konstant höhere Belastungen auftraten. Lediglich im frühen und späten Erwachsenenalter verpassten diese in der Mehrzahl der Fälle die Signifikanz, was für konvergierende Verläufe spricht. Die Ergebnisse unterstreichen die Bedeutsamkeit früher präventiver Maßnahmen, gerade bei Angehörigen niedriger sozialen Schichten. Im höheren Lebensalter sollten weitere Mechanismen, wie die Aktivierung sozialer Netzwerke, erfolgen um einen positiven Einfluss auf das psychische und körperliche Wohlbefinden auszuüben.
