Objective: To determine risk factors associated with the presence of thyroid nodules in patients with type 2 diabetes Material and methods: A case control prospective study, matched 1:1, according to age, sex and smoking habit was conducted. Patients with type 2 diabetes were recruited from the Endocrinology Service of Cayetano Heredia Hospital, Lima-Perú. Clinical evaluation, laboratory tests and thyroid ultrasound were performed to indentify patients with type 2 diabetes with and without thyroid nodules. The association was evaluated by calculating Odds ratios (OR) by conditional multivariate logistic regression techniques. Results: 83 patients with type 2 diabetes and thyroid nodule and 83 patients with type 2 diabetes without thyroid nodule; were obtained. The risk factors associated with the presence of thyroid nodules in patients with type 2 diabetes were HbA1c (OR= 4.12, P= 0.002), BMI (OR= 1.13, P 0.030) and TSH (OR= 3.27, P= 0.0001), with cut-off points according to ROC curve of HbA1c ≥ 8%, TSH in the normal upper limit ≥ 2.3 μIU / mL and BMI ≥ 27 kg / m2. Patients with type 2 diabetes and thyroid nodule had higher values of blood glucose, thyroid volume, abdominal and neck circumference; greater frequency of acantosis nigricans and acrochordons compared to controls. Conclusions: The risk factors associated with the presence of thyroid nodules in patients with type 2 diabetes were HbA1c, BMI and TSH, with cut-off points according to ROC curve of HbA1c ≥ 8%, TSH in the normal upper limit ≥ 2.3 μIU / mL and BMI ≥ 27 kg / m2.
La diabetes mellitus tipo 1 (DM1) es una de las enfermedades crónicas más comunes de la infancia; sin embargo, un gran número de pacientes adultos viven con esta condición. La terapia con insulina es el pilar del tratamiento. Otros aspectos importantes son fomentar un estilo de vida saludable y lograr la adherencia a la medicación. Se presenta el caso de una mujer de 29 años con diagnóstico de diabetes mellitus tipo 1 hace 15 años. Describimos el manejo y evolución de los últimos 5 años y destacamos la importancia de las nuevas tecnologías, como el monitoreo continuo de glucosa, además de factores como la adherencia al tratamiento y el soporte familiar, los cuales fueron particularmente importantes para el control de la enfermedad de la paciente.Palabras clave: Diabetes mellitus tipo 1; Cumplimiento y adherencia al tratamiento; Automonitorización de la glucosa sanguínea (Fuente: DeCS BIREME).
La presencia de diabetes en el embarazo afecta a un número significativo de mujeres y es considerada como un factor con importante papel en la morbilidad y mortalidad perinatal. Con el objetivo de determinar los efectos de esta enfermedad sobre el recién nacido, se realizó un estudio retrospectivo en 35 casos de mujeres con diabetes mellitus o diabetes gestacional que habían tenido un parto previamente. Se escogieron además al azar, 26 controles de partos de mujeres normales para establecer las comparaciones respectivas. Se observó un efecto de la enfermedad en variables como el Apgar, la talla y los perímetros cefálico y torácico de los recién nacidos. No se encontraron efectos sobre el peso como tampoco diferencias sobre la presencia de malformaciones. Por la importancia de esta situación tanto para la madre como para el recién nacido, se recomiendan medidas de control estricto durante el embarazo de estas mujeres, así como en la atención del recién nacido buscando minimizar los riesgos.
