\ criptococose é doença causada pelo Cryptococcus neoformans que ao atingir os pulmões, pela via inalatória, poderá disseminar-se ou não para diversos órgãos, tais como: S.R.E., adrenal, ossos e principalmente para o sistema nervoso central (SNC)2 4 6 7 8 9 14 15 2t _ A neurocriptococose é consequência habitual dessa doença fúngica e, para explicá-la, algumas especulações teóricas foram aventadas. A ocorrência da neurocriptococose em alcoólatras, diabéticos, no uso de corticóides e de neoplasias, realçou a im portância desses fatores como predisponentes dessa micose profunda3 4 5 6 7 9 1013 17 18 20>A depleção de tiamina no alcoólatra, assim como a deficiência do triptofano poderiam facilitar o desenvolvim ento da form a nervosa na criptococose7 12 16 17 18 20 22. Por outro lado, a ocorrência de neurocriptococose é frequente no doente aidético. Tal fato permitiu considerar a criptococose no SNC como doença oportunista nos aidéticos2 6 7 9 13 20. Aidéticos e não. Terapia.A terapia da criptococose apresenta controvérsia em relação à associação de antifúngicos e a monoterapia pela anfotericina B (AMB)6 7 9 13 20 21.A monoterapia pela AMB é usualmente utilizada por algumas semanas e, a seguir, a terapia supressiva ou de manutenção é aplicada com o objetivo de se evitar a recaída da neurocriptococose.A superioridade da associação AMB e 5 fluorcitosina (5FC) sobre a monoterapia (AMB) na esterilização liquórica, na menor nefrotoxicidade e na remissão clínica durante um período de 8 semanas foi verificado por Dismukes e col9. No entanto, em estudo similar Chuck e col6 não observaram diferença siginificativa na sobrevida de pacientes tratados pela associação de drogas (AMB e 5FC) e nos pacientes que utilizaram monoterapia (AMB).Logo, a associação não propicia melhor resultado quando comparada à monoterapia pela AMB na neurocriptococose.Pelo exposto, resolveu-se comparar os achados clínico-laboratoriais obtidos durante o uso da monoterapia com a associação antifungica (AMB e 5FC), em estudo retrospectivo realizado em 112 pacientes com neurocriptococose, aidéticos ou não, internados no Hospital Emílio Ribas -São Paulo, com a finalidade de se verificar qual a conduta terapêutica mais propícia nesses doentes. 241
A meningite neutrofílica persistente é raramente diagnosticada e é caracterizada pelo predomínio neutrofílico na contagem diferencial do número de leucócitos nas amostras de líquido cefalorraquidiano retiradas após sete dias de tratamento adequado. O paciente aqui descrito é soropositivo para o HIV, apresentou febre e confusão mental durante 4 meses e pleocitose neutrofílica na análise liquórica por mais 5 meses. Foi tratado desde o início com tuberculostáticos. Durante três meses as reações imunológicas, as culturas e as pesquisas diretas foram negativas. No sexagésimo dia de internação, a pesquisa de bacilo álcool-ácido resistente (BAAR) no líquor foi positiva e a cultura confirmou a presença de Mycobacterium tuberculosis resistente à isoniazida. Vários fatores podem provocar esta evolução incomum. O comprometimento da imunidade celular, principalmente na liberação de citocinas pró-inflamatórias como a IL 8 e o FNT. O uso concomitante de medicações que poderiam alterar a concentração liquórica dos tuberculostáticos e o aparecimento crescente de cepas multirresistentes foram discutidos.
Persistent neutrophilic meningitis is rarely found and it is characterized by predominance of the number of neutrophilus in samples of C SF (cerebrospinal fluid) from the patient after seven days of treatment. The above patient in HIV positive; he has developed fever and mental disorder for 4 months and has presented neutrophilic pleocytosis in analysis of CSF for more than 5 months. Since the beginning or the treatment he has taken antituberculous drugs and corticosteroids. For 3 months, the serologic avaliation, smears and cultures were negative. On the 60th. day in hospital, the investigation of acid-fast bacilli in CSF was positive and culture confirmed the presence of Mycobacterium tuberculosis resistant to isoniazid. Several factors that may have caused this uncommon development were discussed: the disturbance of cell-mediated immunity, mainly in realse of IL 8 and TNF, the simultaneous use of medicines that could alter the CSF concentration of antituberculous drugs, and the increasing number of multiresistant strains
