Resumen El objetivo de esta revisión integrativa fue identificar y analizar artículos científicos sobre la participación de las personas usuarias en las políticas públicas de salud mental. Se realizó una búsqueda bibliográfica durante los meses de noviembre 2016-mayo 2017 en las bases de datos Pubmed, Scielo y Lilacs. La muestra de la revisión integrativa quedó constituida por siete artículos relevantes que se adecuaron a los criterios de inclusión. Los documentos obtenidos fueron analizados a partir del software de análisis hermenéutico Atlas Ti 6.1. Como resultado, se constató un creciente reconocimiento legislativo y gubernamental de la participación de las personas usuarias de servicios de salud mental en políticas públicas; la persistencia de cuestionamientos sobre su “capacidad” para participar; una diversidad de barreras que dificultan su participación exitosa; y algunas estrategias para fortalecer esta participación. Uno de los principales desafíos identificados se vincula a la necesidad de superar las creencias prejuiciosas y estigmatizantes que operan como barreras a su participación efectiva, y construir las adecuaciones necesarias que posibiliten que las personas con discapacidad psicosocial puedan participar en igualdad de condiciones que los demás.
El objetivo de este trabajo consistió en analizar las barreras para la participación ciudadana de personas usuarias de servicios de salud mental en un órgano consultivo de formulación de políticas públicas: el Consejo Consultivo Honorario en Salud Mental y Adicciones de Argentina (en adelante, CCH). Para ello, desplegamos una estrategia de investigación cualitativa anclada en el paradigma interpretativo-comprensivo y realizamos entrevistas semi-estructuradas a quince informantes clave, análisis documental de información referida al CCH y análisis documental de catorce instrumentos internacionales de derechos humanos en materia de salud mental y discapacidad. Para el análisis de datos, empleamos los principios de la Teoría Fundamentada y nos apoyamos en el software Atlas Ti 6.1. Como resultado, identificamos una sub-representación de las personas usuarias en el CCH durante el período 2014-2019, junto a una serie de barreras económicas, comunicacionales y simbólicas-actitudinales que obstaculizaron su participación igualitaria en este órgano colegiado. Además, registramos una escasez de medidas de accesibilidad implementadas desde el Estado, orientadas a remover las barreras identificadas. Estos resultados alertaron sobre el imprescindible cumplimiento de los compromisos estatales en esta materia, para que estos noveles mecanismos de participación ciudadana no acaben reproduciendo patrones pre-existentes de exclusión social.
La Organización de Naciones Unidas y la Organización de los Estados Americanos han desarrollado sistemas de indicadores de derechos humanos para monitorear el cumplimiento de los compromisos asumidos por los Estados a partir de la ratificación de los pactos y protocolos internacionales. Además de los clásicos indicadores cuantitativos, estos sistemas proponen incorporar “indicadores cualitativos” y “señales de progreso cualitativas”, aunque se observan diferencias en sus formas de construcción y conceptualización. El propósito de este trabajo fue desarrollar un análisis crítico de las propuestas de inclusión de metodologías cualitativas en los sistemas de monitoreo de derechos humanos. Para ello, se llevó a cabo una revisión de los aspectos definitorios de la investigación y evaluación cualitativas, así como de los presupuestos ontológicos, epistemológicos y axiológicos que las vinculan al paradigma interpretativo comprensivo. A partir de ese recorrido, se argumentó que la elaboración de indicadores fijos, estandarizados y construidos a priori se presenta como un contrasentido desde una propuesta de investigación/evaluación cualitativa anclada en el paradigma interpretativo comprensivo. Por el contrario, se destacó la importancia de recuperar la perspectiva de las personas situadas, mediante abordajes inductivos que pongan en tensión los derechos reconocidos en los documentos internacionales con las realidades, percepciones y sentidos que emergen en los contextos socio-históricos concretos en que ellos se materializan. Se concluyó que este tipo de evaluación contribuiría a recuperar en los derechos humanos su potencial político emancipador, al resignificar el protagonismo de las personas como titulares de derechos y situarlas en el centro de los procesos de monitoreo.
