Introduction: After family medicine’s famous beginnings in the early 60’s through introduction of the world’s first family medicine specialization, with transitional changes and war also come changes in former Yugoslavia’s healthcare systems. Aim: The primary aim of this article is to analyze frequency and causes of sanctioning of family physicians by Health insurance funds in the countries of former Yugoslavia. The secondary aim is to evaluate frequency and types of workplace violence family physicians experienced due to insurance boundaries for patients. Methods: The comparative, cross-sectional survey was carried out from October 2017 to February 2018. Study participants were general practitioners’ (GPs), family physicians (FPs) and those without a specialty designation but providing family medicine services in one of the five Western Balkans countries: Croatia, Slovenia, Serbia, Macedonia and Bosnia and Herzegovina (B&H). The questionnaire was designed for the purpose of the study. Results: Forty-nine percent of participating physicians have been sanctioned by Health Insurance Fund and 77. 5% has been exposed to workplace violence. The most common type of violence was verbal (76.6%). Financial penalties according to the scale had the highest rates in Macedonia (73.9%) and Slovenia (43.9%). Conclusion: It is necessary to educate creators of healthcare policies, doctors and patients for the purpose of establishing partner relations which would lead to strengthening of primary healthcare, but also to a more efficient healthcare system.
Kod kojih tegoba se radi ekg dijagnostika u ambulanti GZZHMP Beograd Sažetak:Uvod: Ambulanta Gradskog zavoda za hitnu medicinsku pomoć Beograd (GZZHMP) pripada primarnoj zdravstvenoj zaštiti, opremljena je EKG aparatom. EKG dijagnostika se radi svakodnevno, kod različitih tegoba, a pacijenti koji dolaze u ambulantu su najčešće tu prvi put, nepoznati su lekaru koji radi i najčešće bez prateće medicinske dokumentacije. Kod pregleda pacijenti žele da znaju ne samo šta se sa njima dešava, već i da li je to ozbiljno. EKG dijagnostika često razrešava ovu dilemu. Cilj:Prikazati kod kojih tegoba je pacijentima raĎena EKG dijagnostika u ambulanti GZHMP Metodologija: Korišćena je elektronska baza podataka ambulante Gradskog zavoda za hitnu medicinsku pomoć Beograd, u periodu od 01-22 januara 2015.godine na pregledanih 206 pacijenata. Analizirane su upisane tegobe onih pacijenata kojima je uraĎena EKG dijagnostika; jedan pacijent je mogao imati više od jedne tegobe navedenu u istraživanju.Rezultati: Kod 206 pacijenata u datom periodu, u skoro 50% pregledanih je uraĎena EKG dijagnostika. Razlog rada EKG dijagnostike su žalbe na: povišeni arterijski pritisak, bol u grudima i druge tegobe u grudima, kao npr. gušenje, kašalj, ubrzan ili nepravilan srčani rad, potom glavobolja i vrtoglavice, trnjenje ruku, mučnine, povišena telesna temperatura. UporeĎujući tegobe iz različitih grupa, koje najčešće ukazuju na bolesti kardiovaskularnog respiratornog i centralnog nervnog sistema, nema statističke razlike meĎu njima.Zaključak: Najčešća pojedinačna indikacija za EKG dijagnostiku kod naših pacijenata je povišen krvni pritisak, potom bol tj. tegoba u grudima, nepravilan i ubrzan rad srca, glavobolje, odnosno vrtoglavice. Samo 10% pacijenata je upućen internisti na dalju dijagnostiku u lečenje.

Полипрагмазија је озбиљан проблем због својих многобројних нежељених последица по здравље људи. За полипрагмазију не постоји јединствена и јасна дефиниција. Истраживачи се, углавном опредељују за број истовремено употребљаваних лекова. У раду смо полифармацију дефинисали као истовремено коришћење шест или више лекова у задњих седам дана, повремено или редовно.Циљ рада. Утврдити број истовремено коришћених лекова у полупрагмазији и повезаност са полом и годинама старости испитаника.Метод. У београдској Хитној медицинској помоћи један лекар је од 04.03.2008. до 29.03.2013. године анкетирао на терену пацијенте о лековима које су користили редовно или повремено у последњих седам дана, укључујући и минерале, витамине, помоћна лековита средства и препарате у оквиру традиционалне медицине и те податке уносио у Упитник. Сви добијени подаци су уношени у ексел табелу и статистички обрађени. Коришћен је програмски пакет SPSS 11.0 for Windows.Резултати. Анкетом је обухваћено 1516 испитаника оба пола (649 мушкараца и 867 жена), просечне старости 65,84±17,69 година. Просечан број лекова који су испитаници истовремено узимали је 6,82±3,79(интервал од 0 до 25) и нема статистички значајне реазлике у односу на пол.Шест и више лекова користило је 847 испитаника (349 мушкараца и 498 жена). Просечна старост за мушкарце износила је 70,89±12,79 година, а за жене=74,0±11,29. Највећи број испитаника (60,10%) био је старости 66-85 година. Највећи број испитаника је узимао 6-10 лекова( 70,60%). Десет и више лекова је користило 39,79% испитаника оба пола, а међу онима који су користили ≥10 лекова чак 80,18% је старије од 65 година.Мушкарци су користили у просеку 9,55±3,06 лекова, а жене 9,23±2,77 и нема статистички значајне разлике (p=0,136). Просечан број употребљаваних лекова повећавао се са годинама и био је највећи у групи 66-75 година. Статистички значајна разлика постојала је само између млађих и оних старијих од 65 година.Од укупног броја испитаника са полипрагмазијом, 74,97% су старији од 65 година. Они су употребљавали више лекова у односу на млађе и између њих постоји статистички значајна разлика (p=0,008).Закључак. Више од половине наших испитаника је било у полипраг-мазији и исто толико у ексцесивној полипрагмазији. Полипрагмазија је значајно повезана са старењем. Objective. Examine clinical manifestations, PCR test results, and visit outcomes in patients in COVID-outpatient clinics. Method. The survey was performed using the questionnaire, in February 2021. in patients in four COVID-outpatient clinics in Belgrade and one in Subotica. We got the data about their jobs, gender, age, present symptoms and comorbidities, PCR-testing, and visit outcomes. The data were processed using the software statistical package SPSS 17. Statistical significance was defined for level p<0,05. Results. We included 676 participants, of both genders, with an average age of 49. They visited the doctor 1-3 days after symptoms onset. The most common symptoms were fever and cough, followed by fatigue and muscle aches. Hypertension and obesity were the most common comorbidities, mostly in patients over 65 (p=0,000). We tested 92% of the participants, using PCR tests, and out of 676, 60,95% tested PCR positive, mostly those over 50 (p=0,010). Due to the disease severity, 17,3% of the participants were referred to the hospitals, mostly those aged 51 to 65, but the difference wasn't statistically significant (p=0,183). Conclusion. The most common symptoms of the COVID-19 were high temperature and cough, and the participants visited the doctor 1 to 3 days after symptoms onset. Hypertension and obesity were the most common comorbidities, more often in participants over 65. The PCR tests were positive in more than half of the participants, mostly in those over 50.

Утврдити заступљеност насиља, узроке и последице на професионални рад лекара и које би биле мере превенције.Метод. Упитник је подељен лекарима који су присуствовали обучавању 28-29. марта 2015. у Београду.Резултати. Обухваћено је 411 лекара опште медицине, оба пола, 86,37% жена. Највише испитаника је старости 51-60 год. (45,25%), просечне старости 49,27±9,32 година, просечни стаж 21,10±9,87 год. Највише је специјалиста опште медицине (ОМ) -62,30%. Током стажа 85,40% лекара је доживело неки вид насиља и нема разлике у односу на пол и квалификацију. У претходној години је 62,3% испитаника доживело насиље -вербално 82,97% а физичко 8,83%. Нема статистички значајне разлике у односу на пол (p=0,859), радно место (p=0,097), дужину стажа (p=0,640) и специјалност (p=0,537). Починиоци насиља су у 83,2% случаја пацијенти, а у 40,2% чланови њихових породица. У случајевима насиља, лекаре најчешће нико није заштитио (44,2%), чешће мушкарце (р=0,05). Најчешћи повод је незадовољство пацијената -60,4%, потом дуго чекање на прегледе (37,0%) и када је пацијент под дејством алкохола или опојних дрога (35,0%). Најчешћа последица је смањено задовољство послом који обављају (53,6%).Мере за превенцију насиља: мања преоптерећеност бројем прегледа (56,0%), увођење статуса службеног лица за здравствене раднике (55,60%), физичко обезбеђење (49,3%) и оштрије казне за прекршиоце (50,0%).Закључак. Насиље над лекарима је веома заступљено, најчешће је вербално, починиоци су најчешће пацијенти, узрок је незадовољство пацијента због неиспуњених жеља, а најчешћа последица је смањено задовољство послом који обављају. Насиље као израз незадовољства животом и друштвом, па и здравственим системом: дуго чекају на прегледе, сумња да скупа истраживања више користе лекарима него пацијентима, сумња у везу лекара и фармацеутске индустрије, тако да пацијенти и њихове породице можда у насиљу виде једину могућност да изразе незадовољство здравственим системом уопште и према појединачном доктору 3 . Велики број лекара у својој каријери искуси насиље од стране својих пацијената или њихових породица , вербални и физички напади чак и киднаповања и убиства, а и сексуалне нападе 2,4 .
