IntroducciónEn la actualidad, la hipertensión arterial es una enfermedad frecuente y, en muchas ocaciones, es controlada con diversos fármacos, entre los cuales tenemos: Betabloqueadores (atenolol, metropolol), inhibidores ECA (captopril, enalapril), inhibidores de angiotensina II (candesartan, valsartan), bloqueantes de canales de calcio (nifedipina, amlodipina, diltiazem), entre otros.1 El uso de fármacos bloqueantes de canales de calcio, ya que son útiles para el tratamiento de diversas patologías cardiacas e hipertensión 2,3; sin embargo, como efecto secundario, pueden provocar agrandamiento gingival, el cual se ha reportado también por el consumo de ciclosporina y fenitoína.Este aumento del volumen gingival produce problemas, no solo estéticos, sino también dificultades para el cepillado dental, lo cual agrava la condición periodontal. En algunos casos, puede ocasionar problemas funcionales, ya que, si el aumento es severo, dificulta la masticación4El agrandamiento gingival (AG) tiene diversas etiologías que pueden estar relacionadas a condiciones sistémicas patológicas (leucemia, tumores, etc.) o fisiológicas (alteraciones hormonales durante la pubertad, embarazo, menopausia)4,5, pero en el caso del agrandamiento inducido por fármacos, se manifiesta solo en algunas personas con ciertos factores de riesgo, que pueden ser el consumo de dosis altas del fárma-co, factores demográficos (edad y géne-ro) e influencia de la higiene dental 3,2 .El objetivo del presente estudio fue determinar la frecuencia del agrandamiento gingival por bloqueantes de canales de calcio y evaluar su relación Agrandamiento gingival por bloqueantes de canales de calcioGingival overgrowth by calcium channel blockers ResumenObjetivo: Determinar la frecuencia del agrandamiento gingival en pacientes hipertensos que consumen fármacos bloqueantes de canales de calcio y compararlo con otro grupo de pacientes hipertensos que no consumen dichos fármacos. Métodos: En el estudio descriptivo transversal se evaluaron a 50 pacientes hipertensos que consumían bloqueantes de canales de calcio (grupo de estudio) y a 50 pacientes hipertensos que recibían tratamiento con otro tipo de antihipertensivos (grupo control). Se les realizó una ficha de recolección de datos, periodontograma y fotografías de las lesiones por agrandamiento gingival. Resultados: Se halló que el 10% de los pacientes que consumían bloqueantes de canales de calcio presentaron agrandamiento gingival, lo cual fue estadísticamente significativo (p<0.05), en el grupo control no se halló agrandamiento gingival. Hubo una tendencia inversa con respecto a la edad, ya que mientras aumentaba la edad, disminuía la frecuencia de agrandamiento gingival, pero no se halló relación estadísticamente significativa. Conclusiones: El 10% de pacientes que usan antihipertensivos bloqueantes de canales de calcio presentaron agrandamiento gingival, lo cual fue estadísticamente significativo en relación al grupo control. Artículo originAlResumen El presente estudio evalúa la relación entre el pH salival y la enfermedad periodontal. Se evaluaron 60 pacientes que acudieron a la clínica de pregrado de la Facultad de Odontología de la UNMSM (30 pacientes con gingivitis y 30 pacientes con periodontitis). Asimismo, se contó con un grupo control de 20 estudiantes de odontología periodontalmente sanos. Se evaluó el estado periodontal y se tomó muestras de saliva antes y después de realizarse el tratamiento de fase I. Se encontró que a diferencia de los pacientes del grupo control que presentaron un pH salival promedio de 6,9; los pacientes con gingivitis y periodontitis presentaron valores de 7,3 y 7,9 respectivamente. Después del tratamiento los pacientes que respondieron positivamente presentaron una disminución en los niveles de pH salival en relación al grupo control que se mantuvo. Los pacientes con gingivitis presentaron una reducción promedio de 0,3 (de 7,3 a 7,0). En el grupo de pacientes con periodontitis el pH se redujo de 7,9. a 7,3. El estudio concluyó que el pH salival presenta valores mayores en pacientes con enfermedad periodontal y se reduce luego de realizarse un tratamiento periodontal adecuado. AbstractThe present study evaluates the relationship between salivary pH and periodontal disease. Sixty patients (30 gingivitis patients and 30 periodontitis patients) who went to UNMSM Dental School were evaluated. Also, we took a control group that included 20 dental students with healthy periodontum. We evaluated periodontal status and salivary samples were taken before and after phase I treatment. It was found a difference between control group that presented an average pH of 6.9 and gingivitis and periodontitis groups that showed values of 7.3 and 7.9 respectively. After treatment, patients who had a positive evolution presented a decrease in their salivary pH levels in comparison with control group that maintained its initial value. Gingivitis patients presented an average decrease of 0.3 (7.3 to 7.0). In periodontitis group patients, pH decreased from 7.9 to 7.3. This study concludes that salivary pH shows higher values in patients with periodontal disease and it decreases after the realization of an appropriate periodontal treatment. Odontología Sanmarquina
11La enfermedad periodontal materna como factor de riesgo del retardo de crecimiento intrauterinoThe maternal periodontal disease as a risk factor of intrauterine growth retardation Resumen El estudio evalúa la relación entre retardo de crecimiento intrauterino (RCIU) y la enfermedad periodontal (EP) materna. Este estudio tipo casos y controles de 120 madres, 38 casos (madres de bebés con RCIU) y 82 controles (madres de bebés sin RCIU) se efectuo en el Hospital Docente Madre Niño San Bartolomé. Se interrogó a las madres y se revisaron las historias clínicas. Se registraron datos como: grado de instrucción, estado civil, hábitos de fumar, alcohol y drogas. Todas se examinaron dentro de los dos días posteriores al parto, un solo examinador evaluó los componentes de placa, gingival y periodontal mediante el índice de enfermedad periodontal (IEP) modificado que observa la profundidad al sondaje, nivel de inserción clínica, sangrado al sondaje, inflamación gingival y presencia de placa. Se calculó la distribución de frecuencias y la prueba de Odds Ratio. Se encontró que la enfermedad periodontal materna aincrementan ligeramente la probabilidad de tener niños con RCIU [condiciones de placa OR 1,49 (IC 95% 0,13-1,96), gingivales OR 1,36 (IC 95% 1,06-1,74) y periodontales OR 1,32 (IC 95% 0,09-1,82)]. Se concluye que la enfermedad periodontal materna no representa un factor de riesgo perceptible del retardo del crecimiento intrauterino. AbstractThis study evaluates the relationship between the intrauterine growth retardation (IUGR) and the maternal periodontal disease (MPD). This case-control study of 120 mothers, 38 cases (mothers of babies with IUGR) and 82 controls (mothers of babies without IUGR) at the "Madre Niño San Bartolomé" Teaching Hospital. The mothers were interrogated and their medical histories were reviewed. Data was recorded, such as: the education level, marital status, smoking, alcohol and drug habits. All the mothers were examined within two days after the delivery. Only one examiner evaluated the plaque, gingival and periodontal components through the modified periodontal disease index (PDI) that observes the probing depth, clinical attachment level, proving bleeding, gingival inflammation and plaque presence. The frequency distribution and the Odds Ratio test were calculated. We found out that the maternal periodontal disease increases a little the probability of having children with IUGR [plaque conditions OR 1.49 (IC 95% 0.13-1.96), gingival conditions OR 1.36 (95% IC 1.06-1.74) and periodontal conditions OR 1.32 (IC 95% 0.09-1.82)]. It can be concluded that the maternal periodontal disease does not represent a substantial risk factor for intrauterine growth retardation.

El tipo de estudio es de casos y controles con 69 madres, 19 casos (madres de nacidos con PPBP) y 50 controles (madres de bebés con más de 37 semanas y más de 2500 g) en el Hospital Docente Madre Niño San Bartolomé. Se interrogó a las madres y se revisó las historias clínicas. Se registraron datos como: grado de instrucción, estado civil, hábitos de fumar, alcohol y drogas. Dentro de las 48 horas posteriores al parto, un solo examinador evaluó el índice de enfermedad periodontal (IEP) modificado registrando los indicadores periodontales: profundidad al sondaje, nivel de inserción clínica, sangrado al sondaje, inflamación y sangrado gingival y placa bacteriana. El análisis estadístico incluyó distribución de frecuencias y la prueba de Odds Ratio. Se encontró que las madres con peores condiciones de placa blanda OR: 1,45 (IC 95 % 1,07-1,97); inflamación gingival OR: 2,14 (IC 95 % 1,35-3,39) y pérdida de nivel de inserción clínica OR: 2,8 (IC 95 % 1,55-5,33) se asocian a niños con PP y BP.
