Objective:to describe the epidemiological clinical profile of tuberculosis and analyze the spatial distribution of cases in a municipality in the state of São Paulo. Method:descriptive and ecological study of cases of tuberculosis through the records in an information system. Descriptive statistics were used to calculate central tendency (mean) and chi-square test, with Yates correction or Fisher exact test, when necessary. The Kernel technique was also used to calculate the occurrence densities of tuberculosis cases, using a radius of 1000 meters. Type I error was set at 5%. Results:299 cases of tuberculosis were identified, with 290 (96.98%) being geocoded. The majority of these were male (n = 212; 70.91%), median age 40 years, and the pulmonary clinical form was predominant (n = 244, 81.60%). The distribution occurred in a non-random manner, observing important areas of the municipality with a higher density of cases of the disease. Conclusion:the study evidenced an epidemiological profile of tuberculosis cases similar to those in the literature; however, their distribution does not occur in a random manner, pointing to specific population groups that require greater management and planning of health services for the control of tuberculosis.
A tuberculose persiste como desafio para a saúde pública. Inúmeros esforços são empreendidos para o controle da doença, sendo o tratamento e o acesso do doente aos serviços de saúde entraves para a sua cura. Este artigo tem como objetivo analisar a satisfação dos doentes de tuberculose com relação aos serviços que atuam no controle da tuberculose. Trata-se de um estudo epidemiológico, do tipo inquérito prospectivo, com abordagem quantitativa e qualitativa. Os dados foram coletados por meio de um questionário semi-estruturado. Participaram do estudo 77 doentes. Os dados quantitativos apresentaram avaliação positiva e os qualitativos permitiram compreender a vivência dos doentes quanto ao acesso e tratamento. Aspectos como critérios para a realização do Tratamento Diretamente Observado e proximidade do serviço à moradia do doente influenciam na satisfação, o que leva à necessidade de reorganização dos serviços de saúde para que eles propiciem uma atenção adequada aos doentes de TB.
O objetivo do trabalho foi apreender como os enfermeiros dos Núcleos de Saúde da Família do Distrito Oeste - Ribeirão Preto compreendem a intersetorialidade e buscam articulação com os equipamentos sociais na assistência à saúde da população de suas áreas adstritas. No percurso metodológico utilizou-se o Discurso do Sujeito Coletivo como técnica de ordenação dos dados. Os resultados mostram que a intersetorialidade é um desafio a ser conquistado, pois os profissionais e setores ainda trabalham de forma fragmentada. Percebe-se, nos depoimentos, uma confusão conceitual sobre intersetorialidade pois tomam-na como responsabilidade individual a partir das demandas individualizadas dos usuários. Desta forma, identificam a intersetorialidade como um processo de trabalho penoso e solitário. Entendem, ainda, a necessidade da criação de uma rede de apoio para a efetivação desse princípio e acreditam que é preciso começar a entender e escutar a opinião de outros profissionais e setores.
Neste estudo analisamos a gerência das Unidades Básicas de Saúde no controle da Tuberculose em um município do interior de São Paulo. Participaram do estudo 14 gerentes; a coleta dos dados foi realizada por meio de um questionário fechado e uma questão aberta, e também por meio de entrevista com consentimento livre e esclarecido. Para o tratamento dos dados, utilizamos o Programa Statística 8.0 da Statsoft, e para os dados qualitativos, utilizamos a técnica de análise de conteúdo, modalidade temática. Fica explícita uma gerência técnico-burocrática, com debilidades nas dimensões do planejamento e organização das atividades dos serviços de saúde. Assim, os gerentes desta investigação necessitam incorporar aspectos do planejamento e organização como forma de viabilizar a política de controle da TB.
The study aimed to analyze the discourse of Primary Health Care managers about the search for respiratory symptomatics as an epidemiological surveillance action of tuberculosis. A qualitative study was undertaken, guided by the theoretical and analytical framework of French Discourse Analysis. Data were produced in May 2012 through semi-structured interviews with 14 subjects. Two discursive blocks were produced: marks of power in the execution of the search for respiratory symptomatics; resistance strategies in the search for respiratory symptomatics. Discursive positions were grounded in the traditional management model, making it difficult to incorporate the search for respiratory symptomatics as a participatory action that integrates health staff, managers and the community. Insufficient human resources, workload and rigor in achieving goals favor postures of immobility among the professionals. It is concluded that the managerial work outlined in the health surveillance model encourages the mobilization of practices that contribute to qualify the search for respiratory symptomatics in Primary Health Care.
In this study we analyzed the view of Basic Health Unit (BHU) managers about the Tuberculosis (TB) issue in a city in the interior of São Paulo state. Fourteen BHU managers participated in the study. Data collection took place through interviews that focused on "TB in the municipal agenda". A closed questionnaire was administered, and, at the end, an open question was also applied, which allowed for subjects to freely express the difficulties to control TB in the scope of local management. The interview was carried out after obtaining free and informed consent. Data analysis was performed using the Statsoft software Statística 8.0, which was also used to create simple frequency tables, and thematic content analysis was used for the qualitative data. Thus, three thematic categories emerged: 1) Managing the BHU in TB control: a field of challenges; 2) Managerial actions for TB control in Primary Care (PC); strengths and limits; 3) TB Control in PC: the managerial view. The study subjects have worked as managers for over five years and most consider TB a priority disease in the local health setting. In fact, 81.3% of them consider it a priority disease in the municipal health setting, and only 12.5% state that they sometimes consider it a priority. As to their participation in the discussion and definition oft TB control actions in the city, over 50% sometimes or almost never participate. This shows how deep these managers are imbued with this theme. We observed that TB is strongly focused in PC, i.e., in the campaigns and TB week. This characteristic of "focusing" on actions, present in the subjects' statements, recalls the present culture of the local health system organization, having an effect on the field of planning. Managers reported there is a delay in diagnosis, which implies that the patient comes back several times to the health unit searching for a solution to his or her symptoms. In addition to that delay, subjects also state the stigma, which also needs to be solved since both have a direct effect on the planning and organization of TB control actions in PC. The data show that 50% of the managers do not participate and/or do not know about the definition of specific lines for TB, in terms of transferring resources in the municipality. Underreported cases are a concern pointed out by managers who have little knowledge about the Tuberculosis Control Program (TCP), as well as TB cases in their area. Their statements clearly state a technicalbureaucratic management, deficient in the activity planning and organization dimensions at the BHU they manage. Hence, health care managers in the studied context need to recompose management aspects like planning and organization as ways to make the TB control policy feasible according to the legal precepts and regulations inherent in the TCP.
