Objective Currently, treatment of relapsed or refractory multiple myeloma is challenging. Although bortezomib-thalidomide-dexamethasone-cisplatin-doxorubicin-cyclophosphamide-etoposide (VTD-PACE), a potent combination of a proteasome inhibitor, immunomodulatory drug, and conventional chemotherapeutics, is a widely used regimen, its efficacy and safety are unclear. Methods We retrospectively analyzed the clinical data of 35 patients treated with VTD-PACE. ResultsThe overall response rate was 65.7% (complete response, 5.7%). The median progression-free survival (PFS) and overall survival (OS) were 8.0 [95% confidence interval (CI), 0.9-15.0] and 20.0 (95% CI, 17.5-22.5) months, respectively. Twenty-two (62.9%) patients developed grade 3-4 infections, and no therapyrelated deaths occurred. Sixteen of 25 patients (64%) underwent stem cell harvest successfully with more than 2.0×10 6 /kg of CD34 cells after VTD-PACE. Twenty-two patients underwent autologous or allogeneic stem cell transplantation (SCT). The response and survival durations were short in patients without SCT after VTD-PACE [median PFS: 4.0 (95% CI, 2.7-5.3) months; OS: 14.0 (6.9-21.0) months]; however, these responses significantly improved with SCT following VTD-PACE. The PFS was 8.0 (NA) months (p=0.024), and the OS was 21.0 (19.1-22.8) months (p=0.019). Conclusion VTD-PACE is an effective and tolerable salvage regimen and feasible bridging therapy for SCT.

Polycomb repressive complex (PRC) 1 regulates stem cell fate by mediating mono-ubiquitination of histone H2A at lysine 119. While canonical PRC1 is critical for hematopoietic stem and progenitor cell (HSPC) maintenance, the role of non-canonical PRC1 in hematopoiesis remains elusive. PRC1.1, a non-canonical PRC1, consists of PCGF1, RING1B, KDM2B, and BCOR. We recently showed that PRC1.1 insufficiency induced by the loss of PCGF1 or BCOR causes myeloid-biased hematopoiesis and promotes transformation of hematopoietic cells in mice. Here we show that PRC1.1 serves as an epigenetic switch that coordinates homeostatic and emergency hematopoiesis. PRC1.1 maintains balanced output of steady-state hematopoiesis by restricting C/EBPa-dependent precocious myeloid differentiation of HSPCs and the HOXA9- and β-catenin-driven self-renewing network in myeloid progenitors. Upon regeneration, PRC1.1 is transiently inhibited to facilitate formation of granulocyte-macrophage progenitor (GMP) clusters, thereby promoting emergency myelopoiesis. Moreover, constitutive inactivation of PRC1.1 results in unchecked expansion of GMPs and eventual transformation. Collectively, our results define PRC1.1 as a novel critical regulator of emergency myelopoiesis, dysregulation of which leads to myeloid transformation.

Objectives Patients with hematological cancer receiving chemotherapy have a high risk of thiamine deficiency due to accelerated thiamine usage by tumor cells. Mild or severe thiamine deficiency can lead to varying degrees of neurological symptoms. We evaluated the relationship between thiamine deficiency and neurological symptoms, including mild or nonspecific symptoms, and the influence of chemotherapy on thiamine serum levels in patients with hematological cancer receiving chemotherapy. Materials and Methods We retrospectively identified 42 patients diagnosed with hematological cancer at our hospital, using electronic medical records collected from March 2019 to March 2020. We evaluated the risk factors associated with neurological symptoms (mild-to-severe cognitive impairment, attention impairment, and mood or emotional disorder), the relationship between the presence of neurological symptoms and thiamine serum levels, and changes in thiamine serum levels after chemotherapy. Results Thiamine deficiency was significantly associated with neurological symptoms. The thiamine serum levels in the group with neurological symptoms were significantly lower than those in the group without neurological symptoms. The Wilcoxon rank-sum test showed that thiamine serum levels after chemotherapy were significantly lower than those before administration of chemotherapy. Conclusion Thiamine serum levels in patients with hematological cancer may be used as a reference to maintain neurological status during chemotherapy.

Background: 8%-10% of patients with advanced cancer have intracranial metastases. At present, the treatments mainly include surgery, SBRT, whole brain radiotherapy, chemotherapy et al, but once the patients cannot tolerate those methods, palliative care shown good effects. Plant-extracted drugs, which are important part of palliative treatment, play important roles in inhibiting tumor progress, reducing adverse reactions of radio-chemotherapy, and improving the quality of life.Methods: 54 cancer patients, who diagnosed in brain metastasis and were failure of multi-line therapy, were enrolled. The plant-extracts grugs (Xianhe Baijiang and Shenhuang capsules) were used, which contain Agrimonia pilosa Ledeb, Patrinia scabiosaefolia, ginseng and Astragalus et al. Intracranial site, clinical symptoms, median survival, KPS score, and side-effects were observed.Results: The percentage of supratentorial metastases, subtentorial metastases, and both metastases were 88.88% (48 cases), 5.56% (3 cases), and 5.56% (3 cases), respectively. Since the diagnosis at the first time, dizziness, headache, and decreased muscle strength were observed in 25 patients (46.30%), and 29 patients (53.70%) were symptomless. After treatment, KPS score was 43.70AE27.77 compared to 39.63AE23.95 (p¼0.028), which improved up to 51.85%. Meanwhile, according to CTCAE3.0, no level II or above drug-related digestive and hematology side-effects were observed. Last, the median survival (mOS) reached to 4.5 months since the patient received plant-extracted palliative care treatment, and the longest survival time was up to 102 months. Conclusion:In summary, large proportion of brain metastatic tumors occurred at supratentorial without intracranial symptoms, which led to miss early diagnosis and treatment opportunities. Also, the plant-extracted treatment to patients, who suffered in low KPS score and failure with multi-line therapy, could improve the quality of life and prolong the survival time.
