OBJECTIVE: To estimate the seroprevalence of SARS-CoV-2 in the state of Maranhão, Brazil. METHODS: A population-based household survey was performed, from July 27, 2020 to August 8, 2020. The estimates considered clustering, stratification and non-response. Qualitative detection of IgM and IgG antibodies was performed in a fully-automated Elecsys® Anti-SARS-CoV-2 electrochemiluminescence immunoassay on the Cobas® e601 analyzer (Roche Diagnostics). RESULTS: In total, 3,156 individuals were interviewed. Seroprevalence of total antibodies against SARS-CoV-2 was 40.4% (95%CI 35.6-45.3). Population adherence to non-pharmaceutical interventions was higher at the beginning of the pandemic than in the last month. SARS-CoV-2 infection rates were significantly lower among mask wearers and among those who maintained social and physical distancing in the last month compared to their counterparts. Among the infected, 26.0% were asymptomatic. The infection fatality rate (IFR) was 0.14%, higher for men and older adults. The IFR based on excess deaths was 0.28%. The ratio of estimated infections to reported cases was 22.2. CONCLUSIONS: To the best of our knowledge, the seroprevalence of SARS-CoV-2 estimated in this population-based survey is one of the highest reported. The local herd immunity threshold may have been reached or might be reached soon.
Background: Few population-based studies on the prevalence of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) have been performed to date, and most of them have used lateral flow immunoassays with finger-prick, which may yield false-negative results and thus underestimate the true infection rate. A three-stage cluster sampling stratified by four state regions was used. The estimates took clustering, stratification, and non-response into account. Findings: A total of 3156 individuals were interviewed. Seroprevalence of total antibodies against SARS-CoV-2 was 40.4% (95%CI 35.6-45.3). Population adherence to non-pharmaceutical interventions was higher at the beginning of the pandemic than in the last month. SARS-CoV-2 infection rates were significantly lower among mask wearers and among those who maintained social and physical distancing in the last month compared to their counterparts. Among the infected, 62.2% had more than three symptoms, 11.1% had one or two symptoms, and 26.0% were asymptomatic. The infection fatality rate was 0.17%, higher for males and advanced age groups. The ratio of estimated infections to reported cases was 22.2. Interpretation: To the best of our knowledge, the seroprevalence of SARS-CoV-2 estimated in this population-based survey was the highest and the closest to the herd immunity threshold reported to date. Our results suggest that the herd immunity threshold is not as low as 20%, but at least higher than or equal to around 40%. The infection fatality rate was one of the lowest reported so far, and the proportion of asymptomatic cases was low.
This text presents, in a practical way, five steps to transform public data sites into open data portals. These steps include (1) modeling and (2) extracting data from a web site; The (3) development and (4) documentation of a REST API; And the (5) deployment in a cloud service. Resumo Esse texto apresenta de maneira prática, cinco passos para transformar sites com dados públicos em portais de dados abertos. Esses passos incluem a (1) modelagem e (2) extração dos dados de um site web; (3) O desenvolvimento e (4) documentação de uma API REST; E a (5) implantação em um serviço de nuvem. 5.1. Introdução No Brasil o acesso à informação sobre os recursos públicos está previsto pela Constituição de 1988, mas foi reforçado através Lei de Acesso à Informação (Lei n o 12.527/2011 1). Em resposta a esta demanda, instituições públicas têm disponibilizado um grande volume de informações públicas na Web. Contudo, muitos destes sites foram desenvolvidos para serem acessados por pessoas, e não são compreensíveis por algoritmos computacionais, impedindo assim uma busca automatizada das informações. Tornando a extração de uma informação específica impraticável ou até impossível em alguns casos. Quando isso ocorre o interessado precisa entrar em contato com a instituição provedora, para requisitar os dados de interesse. Esse processo pode demandar um tempo maior que o esperado pelo solicitante, além de consumir tempo da instituição detentora dos dados. Dado este cenário, muitas instituições estão migrando para o paradigma de dados abertos.
Introdução: Uma nova opção terapêutica para a remoção de cabos-eletrodo endocárdicos permanentes desativados é apresentada, utilizando a radiofreqüência para facilitar a liberação do eletrodo a partir do endocárdio, permitindo a extração transvenosa do sistema. Os resultados obtidos nos 4 primeiros pacientes tratados são apresentados. Casuística: Quatro pacientes portadores de processos infecciosos em eletrodos transvenosos abandonados foram submetidos a procedimento para remoção do cateter. Todos os pacientes eram portadores de eletrodos de fixação passiva por aletas implantados no ventrículo direito de 8,5 a 18,8 anos (M = 12,9 ± 5,6) antes. Em todos os casos a técnica da tração contínua havia sido utilizada exaustivamente, não se conseguindo a remoção. Métodos: Utilizou-se gerador de radiofreqüência Radionics RFG3D com anodo de 85 cm2 justaposto ao dorso do paciente e usou-se como catodo o próprio eletrodo a ser removido. Concomitante à tração contínua do cabo-eletrodo a aplicação de radiofreqüência era iniciada. Para cada paciente foram realizadas até três aplicações de 50 W com um tempo máximo de aplicação de 60 segundos em cada aplicação. O tempo e a potência foram determinados empiricamente pela equipe médica, considerando-se insucesso a não liberação do eletrodo a partir do miocárdio após a aplicação do montante de energia acima descrito. Resultados: O cabo-eletrodo foi removido completamente em 2 pacientes. Em 1, a radiofreqüência dissolveu a solda do condutor à ponta do eletrodo, que ficou presa ao miocárdio e no último paciente não houve liberação do cabo após o uso da máxima energia preestabelecida. Não houve complicações ou óbitos. Conclusões: A radiofreqüência pode ser usada como opção terapêutica para a liberação transvenosa de eletrodos abandonados. Maior experiência deve ser obtida para determinar a energia que deve ser utilizada para maximizar os resultados.
Purpose: A new method for endocardial lead extraction, based on radiofrequency assistance, is proposed. The results obtained in the four initial cases are presented. Methods: Four patients with infected pacing leads were submitted to radiofrequency assisted lead extraction. Tined passive fixation leads were implanted in the right ventricle 8.5 to 18.8 (M = 12.9 ± 5.6) years before. Exhaustive continuous traction had been performed previously in four patients and failed to remove the leads. A Radionics RFG-3D Radio Frequency generator was connected to an anode fixed to the back of the patient. The cathode was the lead tip to be extracted. Up to three radiofrequency applications were performed in each patient. The energy released in each applications was up to 50 W during up to 60 seconds. This amount of energy was decided empirically based on the previous experience of the authors with radiofrequency ablation. No locking stylet was used. Os modelos de mudanças de uso e cobertura da terra utilizam um grande volume de dados. Esse artigo apresenta a integração de diferentes ferramentas e conceitos em uma metodologia que permite o reuso e recriação de bases de dados entre diferentes ambientes computacionais. A metodologia foi aplicada na construção e na publicação de uma base de dados para a Amazônia Brasileira. Nesse artigo usou-se um pequeno número de variáveis, mas a metodologia pode ser aplicada independentemente da quantidade de variáveis. O sistema de informação geográfica QGIS foi usado para demonstrar a metodologia. Porém, diferentes sistemas computacionais poderão ser usados com pequenas adequações. Todos os dados e códigos utilizados, estão publicados para permitir replicação dos experimentos descritos aqui.
