Les personnes vivant avec le Virus de l’Immunodéficience Humain (VIH) présentent des complications métaboliques qui sont généralement multifactorielles. Elles sont associées à un risque élevé de morbidité et de mortalité cardiovasculaires. L’objectif de cette étude était d’évaluer le niveau de risque cardiovasculaire et les facteurs associés chez les personnes vivant avec le VIH. Il s’agissait d’une étude transversale, descriptive à visée analytique conduite sur une période de six mois allant du 1er Février au 31 Juillet 2016. Un recrutement systématique a permis d’inclure des patients âgés de plus de 15 ans, VIH positif, sous traitement antirétroviral et suivis à l’hôpital universitaire départemental de Borgou. Le risque cardiovasculaire (RCV) a été évalué selon le score de Framingham qui a permis de classer les patients en deux groupes : les patients ayant un RCV mineur (score < 10%) et ceux ayant un RCV majeur (score ≥ 10%). La régression binominale en analyse univariée et multivariée a été utilisée pour identifier les facteurs associés au risque cardiovasculaire. Tous les tests statistiques ont été réalisés avec un seuil de significativité de 5% et un intervalle de confiance de 95%. Au total, 215 PVVIH ont été inclus dans cette étude sur les 2365 patients suivis au CHUD Borgou. L’âge moyen était de 43,65 ± 11,38 ans avec les extrêmes de 19 et 75 ans. Le sexe féminin était prédominant (146 soit 67,91%) avec une sex-ratio (H/F) de 0,47. Un risque cardiovasculaire majeur était présent chez 56 patients (26,05%). Les facteurs associés au RCV majeur en analyse univariée étaient le sexe féminin (p = 0,019), la consommation du tabac (p = 0,003), le diabète (p < 0,001), l’hypertension artérielle (p < 0,001), le syndrome métabolique (p < 0,001) et l’exposition à l’association Lopinavir/ritonavir (p = 0,011). La prise de Zidovudine avait un effet protecteur (p = 0,008). En analyse multivariée, les facteurs associés au RCV majeur étaient le sexe féminin (p = 0,0001), le tabagisme (p = 0,02) et la présence du diabète (p = 0,0003). Les patients infectés par le VIH suivis à Parakou présentent plusieurs facteurs de risque cardiovasculaire. Ce risque doit être prévenu par la prescription des mesures hygiéno-diététiques et l’instauration d’un suivi cardiologique régulier. © 2019 International Formulae Group. IntroductionLa dyspnée est un symptôme qui peut être l'expression de pathologies potentiellement grave et urgentes. Notre objectif était d’évaluer la qualité de la prise en charge de la dyspnée en milieu hospitalier à Parakou.MéthodesIl s'agissait d’étude rétrospective portant sur tous les patients admis dans les centres hospitaliers de Parakou pour dyspnée non traumatique entre le 1er Février 2012 et le 31 Mai 2013. Les recommandations tunisiennes sur la prise en charge de la dyspnée aux urgences ont été utilisées comme référentiel d’évaluation. Chaque étape de la démarche médicale a été notée et la prise en charge était bonne lorsque la note obtenue au score était entre 51 et 100. Les autres variables utilisées étaient la qualification du soignant principal, les antécédents du patient et les données cliniques et paracliniques.RésultatsSur les 11101 patients reçus aux urgences on a dénombré 328 cas de dyspnée (2,9%). La PEC était assuré principalement par les cardiologues (55,9%) et les médecins généralistes (29,2%). La qualité de la prise en charge par les généralistes était bonne chez 73,2%. Les facteurs associés à la bonne qualité de PEC étaient: la recherche des antécédents médicaux (84,4% vs 15,6%; p < 10-4), la description des caractéristiques cliniques de la dyspnée (94,1% vs 5,9%; p < 10-4) et la réalisation de l'examen physique complet (86,8% vs 13,2%; p < 10-4).ConclusionA Parakou en 2013 près d'un généraliste sur trois gère mal la dyspnée. Cette mauvaise gestion est liée à une mauvaise démarche médicale.
Introduction: Rhythm disorders (RDs) or arrhythmias are poor prognostic factors during heart failure (HF). Our objective was to study the frequency of rhythm disorders presented by patients treated for HF in Parakou, Benin in 2017. Methods: This was a cross-sectional, descriptive and analytical study. It consisted of a systematic recruitment of all patients admitted for heart failure in the cardiology units of the city of Parakou from March to August 2017. The diagnosis of RDs was made by cardiac auscultation completed by electrocardiographic recording (standard and Holter over 24 hours). The Lown classification was used to describe the severity of ventricular TRs. The threshold for significance was p < 5%. Results: A total of 83 heart failure patients aged 61.47 ± 15.97 years with a sex ratio of 1.08 were selected. Arrhythmia was detected by auscultation in 20.48%; by standard electrocardiogram (ECG) in 25.83% and by Holter ECG in 97.59% of patients. Among the arrhythmias detected by Holter ECG, 81.48% were atrial, including 28.79% of atrial fibrillation. RDs was ventricular in 90.12% of which 47.95% were severe. Left ventricular systolic dysfunction was significantly associated with ventricular arrhythmias. On the other hand, the frequency of ventricular arrhythmias was not associated with the severity of the HF according to the NYHA classification. Conclusion: In Parakou, RDs are frequent in patients with HF. Holter ECG should be included in the follow-up of heart failure patients in order to improve their management.
Introduction: Peripartum cardiomyopathy (PPCM) is a dilated cardiomyopathy occurring in the last month of pregnancy or the first five months postpartum without pre-existing cardiovascular pathology. It is a major cause of pregnancy-related heart failure with high morbidity and mortality. In severe forms (10% to 15% of cases), thrombo-embolic complications are the main cause. The initial hemodynamic evolution is totally unpredictable and sometimes extremely brutal and fatal. The objective of this work was to show the often pejorative evolution of PPCM in our country. Methods: We report in this work three serious clinical cases revealing the complications of this PPCM among patients admitted to the cardiology department of the CHUD-B/A in 2022 for heart failure. The data were collected according to the Declaration of Helsinki. Patients and Observations: The first case was a PPCM with severe left ventricular (LV) systolic dysfunction complicated by spontaneous left intraventricular contrast and right superficial sylvian ischemic stroke.
