ÖzAmaç: Primer olarak yapılan total tiroidektomi ile tamamlayıcı tiroidektomiyi ameliyat sonrası morbidite açısından karşılaştırmayı amaçladık. Yöntemler: Haseki Eğitim ve Araştırma Hastanesi Genel CerrahiKliniği'nde yapılan tamamlayıcı tiroidektomi ameliyatları ile primer total tiroidektomi ameliyatlarındaki morbidite retrospektif olarak incelendi. Cerrah tercihine göre nöromonitorizasyonun kullanılmadığı, ardışık olarak ameliyat edilen hastalar değerlendirilmiştir. Bulgular:İncelenen parametrelerden hiçbirinde yaş (p=0.207), cinsiyet (p=0.998), hormon durumu (p=0.287), nodül mevcudiyeti (p=0.287), çıkarılan lob sayısı (p=0.695), multisentrisite (p=0.081) komplikasyon oluşumu ile çalışıldığında hiçbirinde istatistiksel anlam farkı saptanmadı. Göreli orantı yönünden (odds ratio) 11.9 kat daha fazla komplikasyon riskinin ikinci ameliyat için uygun sürenin dikkate alınmadığı hastalarda oluştuğu görüldü.Tüm hastalar ele alındığında komplikasyon varlığının dağılımı tamamlayıcı tiroidektomi yapılan grupta (grup 2), ilk baştan total tiroidektomi yapılan gruptan (grup 1) anlamlı derecede yüksek bulunmuştur (p=0.003). Grup 2'de komplikasyon gelişme olasılığı Grup 1'den 10.6 kat (odds ratio) fazla gözlenmiştir. Sinir ve paratiroid hasarlarının hiçbiri kalıcı olmamıştır. Sonuç:Günümüzde teknolojik gelişmeler ve ameliyat öncesi tanı yöntemleri tamamlayıcı tiroidektomi gereksinimini azaltsa da, yapıldığında komplikasyon olasılığı baştan total tiroidektomi yapılan hastalardan daha fazladır. Methods:The morbidity of primary thyroidectomy operations and completion thyroidectomy operations performed at Haseki Training and Research Hospital General Surgery Clinic were evaluated retrospectively. Consecutive patients, in whom neuromonitorization was not used according to the surgeon's choice, were enrolled for study and control groups.Results: There were no statistically significant difference in age, sex (p=0.998),hormonal status (p=0.287),presence of nodule (p=0.287),number of lobes removed (p=0.695), multicentricity (p=0.081)in regard to complications.It was seen that the risk ratio (the odds ratio) was 11.9 times higher in patients in whom appropriate time was not waited for the second operation.When all patients were evaluated, the distribution of complication was found to be significantly higher in the group with completion thyroidectomy (group 2) than in the group with total thyroidectomy (group 1) (p=0.003).The probability of developing complications in Group 2 was 10.6 fold (odds ratio) higher than Group 1.Neither nerve nor parathyroid damage was permanent. Primer hiperparatiroidi, artmış parathormon salınımına bağlı hiperkalsemi ile seyreden endokrin bozukluktur.Tedavisi hastalıklı bez ya da bezlerin bilateral/unilateral boyun eksplorasyonu veya odaklanmış paratiroidektomi ile çıkarılmasıdır. Bu çalışmadaki amacımız primer hiperparatiroidi nedeniyle cerrahi uygulanan hastaların sonuçlarını değerlendirmektir. Yöntemler: Kliniğimizde 2013-2017 yılları arasında primer hiperparatiroidi nedeniyle ameliyat edilen hastalar retrospektif olarak incelendi. Hastaların demografik özellikleri, ameliyat öncesi parathormon (PTH), kalsiyum, fosfor,albümin, D vitamini seviyeleri, paratiroid sintigrafisi ve/ veya SPECT (Single Photon Emission Computerized Tomography), boyun ultrasonografisi bulguları, görüntülemeler arasındaki uyum, ek görüntüleme veya tetkik yapılıp yapılmadığı,uygulanan cerrahi teknik, ameliyat sonrası PTH, kalsiyum seviyeleri, patoloji sonuçları, komplikasyon varlığı, ameliyat sonrası altıncı ay PTH ve kalsiyum seviyeleri, persistans veya rekürrens varlığı kaydedildi. Verilerine ulaşılamayan, sekonder hiperparatiroidi nedeniyle ameliyat edilen hastalar çalışma dışı bırakıldı. Bulgular: Elli dokuz hastanın yaş ortalaması 56±11,2 yıl, kadın/erkek oranı 43/16 idi. Ameliyat öncesi PTH seviyeleri 272±171ng/L, kalsiyum seviyeleri 11,8±0,68mg/dL idi. Ortanca takip süresi 22(3-56) aydı. 10 hasta (%16,9) semptomatik, 49 hasta(%83,1) asemptomatik idi. Hastaların %50,8'ine odaklanmış cerrahi, %1,7'sine bilateral, %18,6'sına unilateral eksplorasyon uygulandı. Ayrıca hastaların %18,6'sına eşlik eden tiroid patolojisi nedeniyle lobektomi, %10,2'sine bilateral total tiroidektomi yapıldı. 1 olguda paratiroid kanseri şüphesi nedeniyle lobektomi uygulandı. Sonuç: Ameliyat öncesi tanının doğru konulması, hastalıklı bezin yerinin tespiti primer hiperparatiroidinin cerrahi başarısını etkileyen en önemli faktörlerdir.Bizim serimizde de lokalizasyon çalışmaları titizlikle yapılmakta ve cerrahi başarı giderek artmaktadır.

Bulgular: Karın ağrısı ile başvuran ve akut apandisit düşünülen 209 hasta opere edildi, 108 hasta gözlem sonrası ameliyat olmadan taburcu edildi. Ameliyat olanlarda Alvarado skorunun duyarlılığı %92, özgüllüğü %28, pozitif kestirim değeri %93, negatif kestirim değeri %24, doğruluğu %87, ultrasonografinin duyarlılığı %84, özgüllüğü %39, pozitif kestirim değeri %94, negatif kestirim değeri %18, doğruluğu %80 olarak hesaplanmıştır. Ultrasonografi; ameliyat edilen 219 hastanın 180'inde (%82,2) akut apandisit ile uyumlu sonuç vermişken, 39'unda (%17,8) apendiks normal görünümdeydi. Ultrasonografi; ameliyat edilmeyen grupta 108 hastanın 92'sinde (%85,2) akut apandisit ile uyumlu sonuç vermişken, 16'sında (%14,8) apendiks normal saptandı. Methods: A total of 327 patients, who were referred to İstanbul Haseki Research and Training Hospital General Surgery Clinic for abdominal pain and who received the diagnosis of acute appendicitis using both compression ultrasonography and Alvarado scoring system were retrospectively evaluated. Results: Two hundred and nine patients with abdominal pain and acute appendicitis were operated and 108 patients who did not require surgery were discharged after being placed in observation. The sensitivity and the specificity of the Alvarado score were found to be 92% and 28%, respectively The sensitivity and specificity of ultrasonography were 84% and 39%, respectively. Ultrasonographic findings were consistent with acute appendicitis in 180 (82.2%) of 219 operated patients, whereas appendix was normal in 39 (17.8%). Ultrasonography showed acute appendicitis in 92 (85.2%) and normal appendix in 16 (14.8%) of the 108 patients in the unoperated group. Conclusion: Alvarado score is not superior to ultrasonography in reducing negative laparatomy rates, but it can be used safely in the diagnosis of acute appendicitis andsurgical decision making.

Intimal angiosarcoma is a rare vascular malignancy, and diagnosis is very difficult due to nonspecific symptoms. There are controversial points regarding the diagnosis, treatment and follow-up of intimal angiosarcomas. The purpose of this case report was to evaluate the diagnosis and treatment process of a patient diagnosed with femoral artery intimal angiosarcoma. Furthermore, in line with previous studies, it was aimed to illuminate controversial points. A 33-year-old male patient, who had been operated on due to ruptured femoral artery aneurysm, was diagnosed with intimal angiosarcoma with the pathology result. Recurrence was observed during clinical follow-up, and the patient was treated with chemotherapy and radiotherapy. Since there was no response to treatment, the patient underwent aggressive surgery including the surrounding tissues. No recurrence or metastasis was observed in the patient’s 10th month follow-up. Although intimal angiosarcoma is rare, it should be considered in differential diagnosis when femoral artery aneurysm is detected. The most important step in treatment is aggressive surgery, but adding chemo-radiotherapy to the treatment should be considered.
