Recent studies have suggested that a relationship could exist between 25-hydroxyvitamin D [25(OH)D] deficiency and erectile dysfunction (ED). The present study evaluated the relationship between 25(OH)D levels and ED in male patients with type 2 diabetes mellitus (DM). The study included 98 patients with type 2 DM aged between 18-80 years. The International Index of Erectile Function (IIEF-5) Questionnaire was administered. The patients were divided into three groups according to IIEF-5 scoring: IIEF-5 score between 5-10, severe ED; IIEF-5 score between 11-20, moderate ED; IIEF-5 score between 21-25, no ED. Biochemical parameters, 25(OH)D and hormonal analysis tests were obtained in all patients. All parameters were compared between these three groups. Of 98 patients included in the study, 32 had severe ED, 45 had moderate ED and 21 had no ED. The mean age was 55.12 ± 9.39 years and the mean 25(OH)D level was 13.69 ± 8.15 ng/ml. When the three groups were compared, 25(OH)D levels were significantly lower in patients with the IIEF-5 score between 5-10 (p = 0.020). There was a moderate positive relationship between IIEF-5 score and 25(OH)D level (r = 0.21, p = 0.038). The patients with severe ED have considerably lower 25(OH)D levels.
We evaluated the relationship between erectile dysfunction (ED) and IL‐6 levels in males with COVID‐19. The study included 80 male patients aged 30–45 years who were hospitalised due to COVID‐19. The International Index of Erectile Function (IIEF‐5) questionnaire was used to assess erectile function. The IIEF‐5 questionnaire was re‐administered at a 3‐month control visit after discharge, and the change score from baseline was recorded. The patients were divided into three groups according to the IIEF‐5 score at 3 months as Group 1 (severe ED), Group 2 (moderate ED) and Group 3 (no ED), and into two groups according to IL‐6 level at the time of admission as Group A (IL‐6 ≤ 50 ng/ml) and Group B (IL‐6 > 50 ng/ml). The change in the IIEF‐5 score (p < .001) was significantly greater in Group B than in Group A. There was also significant difference in IL‐6 between Group 1 and Group 2 (p = .008). The correlation analysis revealed a moderate correlation between IL‐6 level and the change in IIEF‐5 score and D‐dimer level (r:0.529, p < .001) and a weak correlation between IL‐6 level and FSH (r:0.309, p = .005). The present study suggests that elevated IL‐6 levels in male patients hospitalised due to COVID‐19 might be related to the risk of developing ED.
We found a close relationship with Framingham cardiovascular risk score in diabetic patients with very low serum vitamin D levels. Cardiovascular risk as assessed by the Framingham's scale increases with decreasing 25[OH]D levels. BNF score was negatively correlated with 25[OH]D levels.
Background: Troponin levels may be elevated in COVID-19 infection. The aim of this study was to the explore relation between troponin levels and COVID-19 severity. Materials & methods/Results: One hundred and forty consecutive patients with COVID-19 pneumonia were included. Diagnosis of COVID-19 pneumonia was based on positive chest computed tomography (CT) findings. Quantitative PCR test was performed in all patients. Only 74 patients were quantitative PCR-positive. Twenty four patients had severe CT findings and 27 patients had progressive disease. These patients had significantly lower albumin and higher ferritin, D-dimer, lactate dehydrogenase, C-reactive protein, and high-sensitivity cardiac troponin I (hs-cTnI). Conclusion: COVID-19 patients with severe CT findings and progressive disease had higher hs-cTnI levels suggesting the use of hs-cTnI in risk stratification.
Araştırmalar / ResearchesÖZET Türk toplumunda organik ve fonksiyonel dispepsili hastalarda etyolojiye yönelik analiz Amaç: Dispepsi gastrointestinal kaynaklı spesifik olmayan semptomlar topluluğu olarak tanımlanabilir ve başvuran hastalarda yakınmaların nedenini ortaya koymak için yapılan incelemelerde dispepsiye neden olabilecek patolojiler saptanabilirse bu tür dispepsiler organik dispepsi, lokal veya sistemik bir hastalık sorumlu olarak bulunamamış dispepsiler ise fonksiyonel dispepsi olarak tanımlanırlar. Amacımız Türk toplumunda organik ve fonksiyonel dispepsi prevalansını saptamak ve muhtemel etyolojik faktörleri incelemektir. Gereç ve Yöntem: Çalışmamız, prospektif olarak planlandı ve Haziran 2008 tarihinden itibaren dispepsi şikâyetleri ile başvuran ilk 200 erişkin hasta cinsiyet gözetilmeksizin ve 100 hasta ise kontrol grubu olarak çalışmaya alındı. Çalışmamıza dahil edilen tüm hastalar hemogram, biyokimya tetkikleri, gaita tetkikleri, üst gastrointestinal endoskopi, tüm batın ultrasonografi ile değerlendirildi. Ülser, gastrit ve safra kesesinde patoloji tespit edilen hastalar organik dispepsili olarak kabul edildi. Bulgular: Dispepsi şikayeti olan 200 hastanın 71'inde (%35.5) üst gastrointestinal endoskopisinde gastrit veya ülser, 33'ünde (%16.5) tüm batın ultrasonografisinde safra kesesinde kalkül veya çamur tespit edildi. Hem mide patolojisi hem de safra kesesi patolojisi olan hasta sayısı ise 12 (%6) idi. Organik dispepsi grubundaki hasta sayısı sonuç olarak 92 oldu. Geri kalan ne ultrasonografi de ne de üst gastrointestinal endoskopisinde bulgu tespit edilmeyen 108 hasta ise fonksiyonel dispepsi grubuna dahil edildi. Sonuçlar: Çalışmamızda dispepsili hastaların %46'sında organik bir sebep ortaya konabildi. Organik dispepsilerin en önemli sebebi üst gastrointestinal patolojilerdi (organik dispepsilerin %77'si). A triyal fibrilasyon (AF), süratli ve disorganize olan atriyal elektriksel aktivasyonlar ve koordinasyonu bozulmuş atriyalkontraksiyonlar ile karakterize bir ritm bozukluğudur. [1] Çarpıntı, anksiyete, göğüs ağrısı, nefes darlığı ve halsizlik tipik semptomlar arasındadır. AF'nin erken tanısı için ≥65 yaş olan her hastanın muayenesi fırsat bilinmeli ve nabız, EKG kontrolü yapılmalıdır. [2, 3] İnme, tromboemboli, kalp yetmezliği, yaşam kalitesinde düşüş ve bozulmuş kognitif fonksiyonlar en önemli morbitide ve mortalite nedenleridir. Tedavide ana başlıklar; hız ve ritm kontrolü, uzun vadeli Amaç: Çalışmamızda atriyal fibrilasyonlu (AF), varfarin kullanan hastalarda INR (international normalized ratio) farkındalığının ve bu hastalarda hedeflenen INR düzeyine ulaşma oranlarının belirlenmesini amaçladık. Yöntem: Çalışmamıza varfarin polikliniğine başvuran, AF nedeniyle varfarin tedavisi alan, 18 yaş üstü kadın (%40, n=120) ve erkek (%60, n=180) toplam 300 hasta dahil edildi. INR açısından 2-3 ve arası değerler efektif olarak değerlendirildi. Hastaların tamamına aynı anket uygulandı. Bulgular: Varfarin kullanan hastaların %57'sinin istenilen efektif INR düzeyinde olmadığını gösterdik. Yine hastaların büyük çoğunluğunda INR farkındalığını oldukça düşük saptadık. Nitekim varfarin kullananların %72.2'si INR'nin ne anlama geldiğini, %68'i ilacın yan etkilerinin neler olduğunu, %75.7'si günlük hayatta alması gereken önlemlerin neler olduğunu, %83.7'si ise K vitamini içeren besinlerin neler olduğunu bilmemekteydi. INR'nin anlamını bilen hastaların, efektif INR aralığında bulunma oranları daha yüksek saptanmasına karşın bu oranlar istatiksel olarak anlamlı saptanmadı. Ayrıca eğitim düzeyi, medeni durum ve INR baktırma sıklığı ile INR hedef değerlerine ulaşılması arasında da istatiksel olarak anlamlı bir ilişki bulunmadı. Sonuç: Varfarin tedavisi başlangıcında hekim tarafından kar-zarar dengesi iyi kurulmalı, hasta ve yakınlarına ilacın etkileri, yan etkileri, etkileşimleri, takibi, günlük hayatta alınması gereken önlemler vb. konularda direkt, anlaşılabilir bir şekilde bilgi verilmelidir. İzlem için çeşitli modern yöntemler her şekilde devreye sokulmalı ve INR düzeyi istenen düzeyde olmayan hastaların takibi daha yakından yapılmalıdır.
Introduction Diabetic nephropathy and diabetic retinopathy are serious microvascular complications of diabetes mellitus. Recent studies have demonstrated that neutrophil-gelatinase-associated lipocalin (NGAL) may be accompanied by these complications during and before the appearance of microalbuminuria. In this study, we set out to research the role of NGAL in patients with diabetic nephropathy and diabetic retinopathy.Material and methods Eighty-two patients with type 2 diabetes were enrolled in our study. Urinary microalbumine and NGAL levels were measured in urine samples over 24 hours. We also studied NGAL levels in serum. All patients went through an ophthalmologic examination. The results were evaluated based on the presence of microalbuminuria and retinopathy.Results There were no significant differences in serum and urine NGAL levels between normoalbuminuric (n = 66) and microalbuminuric (n = 16) patients. We also did not find any significant difference in patients with retinopathy (n = 16) or without retinopathy (n = 66). ConclusionThere are controversial findings about the role of NGAL in diabetic patients in medical literature. Standard values of urine and serum NGAL levels have yet to be determined. Our study suggests that NGAL is not a useful marker to differentiate microalbuminuric patients from normoalbuminuric subjects. We also did not find a relationship between NGAL levels and the presence of retinopathy. Additional studies with larger sample sizes will be required to confirm or refute these findings.
