Background: Chronic otitis media is an inflammatory process of the mucoperiosteal lining of the middle ear space and mastoid. The main aim of surgery of chronic ear disease is to eliminate disease process and to give the patient a dry safe and functioning ear. Interlay myringoplasty a newer technique has shown promising results with higher success rates than other conventional methods of myringoplasty. We aimed to study the hearing gain in terms of air bone gap and outcome of graft uptake. Materials & methods: This is a prospective study of 18 months duration from January 2013 to June 2014 carried out in ninety (90) patients of chronic suppurative otitis media (CSOM) with large central perforation (more than 50% of tympanic membrane). All patients underwent through interlay myringoplasty after clinical examinations, audiometric tests & routine investigations. Patients were called for regular follow up for 16 weeks. Results: Pre operatively mean air bone gap was 27.50±5.53 dB. Post operatively after 16 weeks mean air bone gap was 13.67±5.56. On last follow up at 16 weeks, maximum numbers of graft rejections were observed in 6 patients (6.7%). Success rate was 93.3%. Conclusion: Myringoplasty is a safe and effective technique to improve the quality of life of patients. The interlay technique had a better graft take up and hearing improvement and also showed promising results in terms of limited follow up period and limited number of cases involved.
A prospective study of lipid profile was done in 60 patients of vertigo at E.L.M.C. Lucknow from 2011 to 2014. All components of serum cholesterol were analyzed. Serum cholesterol and hyperlipidemia as an etiology of the atherosclerosis of all blood vessels also have a role in vestibulo-cochlear vessels. It was found that there were 34 females and 26 males and maximum number of patients (63.33%) in the age group of 31-50years. Appreciable difference (p<0.05) in the mean value of total lipid, total cholesterol, triglycerides and phospholipids in the control and study group was found but difference was not significant in the mean value of HDL, LDL and VLDL cholesterol level. Four cases of diabetes and ten cases of hypertension of 60 vertigo cases were having marked vertigo of longer duration. These findings were similar to Mehra Y.N. Thus we find that serum lipid studies are important in the patients of vertigo for the diagnosis and management.
Aim: Comparative study of microbiology of chronic rhinosinusitis (CRS) in smokers and nonsmokers. Materials and methods:This study was carried out on 700 patients diagnosed with CRS attending the ear, nose, and throat outpatient department (OPD) at Era's Lucknow Medical College, Lucknow, India, between January 2015 and June 2016. These patients were divided into two groups (smokers and nonsmokers). All patients underwent diagnostic nasal endoscopy. Two samples were collected and antimicrobial sensitivity test was done. The data were analyzed using Statistical Package for the Social Sciences (SPSS) version 23.0. Chi-squared test and independent samples t-test were used to compare the data. A p-value <0.05 indicated a statistically significant association. Results:Of 700 patients included in the study, smokers constituted 333 (47.57%) patients and nonsmokers constituted 367 (52.43%) patients. Out of the 700 patients, bacterial isolates of 585 (83.57%) were found to be positive, of which aerobes were 485 (82.91%) and the rest 100 were anaerobes. After antimicrobial therapy, all the symptoms were higher in smokers as compared with nonsmokers. Proportion of improvement in nonsmokers (90.19%) was higher as compared with smokers. Conclusion:Microbiology of CRS is highly influenced by smoking habit. On evaluating the treatment response in terms of repeat sampling after 3 months, we found that pathogen positivity rate was much higher in smokers as compared with nonsmokers, thus implying that smoking exposure in vivo does alter the efficacy of antibiotics.
BACKGROUND Chronic Suppurative Otitis Media (CSOM) is one of the most common diseases of younger age in middle class population of the developing countries. Loss of hearing due to CSOM has a role in learning and intelligence of the students. There are multiple factors, which affect the tympanic membrane repair, mastoidectomy is one. MATERIALS AND METHODS This study at Era's Lucknow Medical College and Hospital was planned to assess the effect of cortical mastoidectomy in safe type CSOM. For that, 120 patients were taken who were divided in two groups of 60 each. One group was subjected to tympanoplasty type 1 and other to cortical mastoidectomy and type 1 tympanoplasty. RESULTS After six months of follow up, it was found that graft rejection was 13 in group 1 compared to 12 in group2 and cortical mastoidectomy has almost no effect on graft acceptance and hearing gain in pure tone audiometry. CONCLUSION This study was planned to find the effect of cortical mastoidectomy in tympanoplasty type 1 in safe type CSOM. Two groups of 60 patients were taken. In one group, only tympanoplasty type 1 while in other cortical mastoidectomy along with tympanoplasty type 1 was done. After 6 months of follow up, no statistically significant difference was found in the graft rejection and hearing gain between the two groups.
